Employee states they were portioned about an hour ago. Employee corrected by placing portion cups in lower portion of cooler to rapidly cooler, before placing in top portion of prep top cooler. These containers were not overloaded and need to be cooled prior to placing in the top portion. Violation corrected at this time. On follow up inspection found tcs food, cut lettuce 41°F in a container surrounded by ice bath. Violation corrected. On follow up inspection found tcs food, facility made salsas 39 to 40°F in the self service cold hold well. Pic states cold hold well was repaired. Violation corrected.
