Hose with sprayer attached without a hose bib vacuum breaker. A plumbing system shall be installed to preclude back flow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and back flow prevention is required by law, by: (a) providing an air gap as specified under # 5-202. 13 (p); or (b) installing an approved back flow prevention device as specified under # 5-202. 14. (p) to correct remove the shut off valve from the avb and add a hose bib vacuum breaker. ,

Normal Good Repair and Proper Adjustm N/A N/A N/A Observed the following: 1. Gaskets on the middle cooler on the cook line in poor repair, doors are not properly shutting. 2. All three gaskets on the three door upright cooler in the prep area. 3. Gasket on the downstairs freezer. 4. Gaskets on the top loading cooler by the hand sink, near the dish line. Note: door is not properly shutting. 5. Cooler on the far left of the cook line needs the door liners repaired as they are loose and not easily cleanable. Equipment shall be maintained in a state of repair and good condition. To correct repair or replace all the noted items.
