(Critical) The following foods were found to be expired in the refrigerators: feta cheese (2-22), black beans & rice (2-26). Corrected by discarding these foods.

One of the back kitchen wiping cloth pails no longer had any chlorine residual in it. Corrected by providing a fresh solution of chlorine sanitizer at 100-150 ppm

Observed two containers of employee foods found in different refrigerators in the back kitchen stored above food for your customers. Either relocate these employee food containers to the bottom of the unit to protect the food for your customers or provide a separate, small refrigerator just for the storage of employee foods

The ice scoop handle was found to be resting on the ice in the ice storage container. Corrected by removing the ice scoop and storing it in a separate container outside the ice container along with the ice pick

Observed the interior lid to one of the chest freezers in the storage room to be damaged. It was previously repaired with tape, however, the tape is starting to fail. Consider removing this interior cover and replacing it with a piece of frp paneling

(Critical) Since the last inspection the espresso machine was installed and is in use. The water line serving this piece of equipment lacks a back-flow preventer. Install an a. S. S. E. 1012 or a 1022 device on the water line between the connection to the hand washing sink supply and the espresso machine

(Critical) Observed a partial one dozen container of brown shell chicken eggs. These eggs are purchased from the ann arbor farmers market. This container only has a small label with the name and address of the farm. Initial research through mda indicates this facility only has a license for the manufacture of and retail sale of jams/jellies. These eggs were discarded. As a means of correction either provide me a copy of this supplier's mda license for the processing of chicken eggs or purchase shell eggs from another approved (mda or federally approved) egg processor/supplier

Observed an employee with fingernail polish and bare hands working directly with cooked chicken (separating the meat from the bones for future cooking). Corrected by discussing this issue with the employee and by the employee putting on single use gloves to finish this task

Observed some staff with bracelets and a watch involved in food preparation. Food handlers need to remove this type of jewelry prior to working with foods

(Critical) Observed bread pudding stored out a room temperature. Discard this food and keep this product in a refrigerator at 41f or below

Observed an employee wearing multiple rings and bracelets on her fingers and wrists. Employees working with food and making drinks, etc. Must remove jewelry on hands/wrists

Observed a used spatula to be placed on top of the mixer. Corrected by relocating the spatula to a container for storage between uses

Observed two clay soup bowls with cracked/missing glaze. Corrected by removing them from the available bowl supply. These bowls may be used for decoration/display or non-food use

Observed several wiping cloth buckets where the sanitizer was observed to be too weak. Corrected by providing fresh sanitizer at 100 ppm

A small counter top deep fryer (open top) is being used on top of a low-boy hot holding box that is not located under an exhaust hood. Deep fryers must be located under an exhaust hood to remove heat, steam and oil

Observed coffee filters stored on top of a piece of equipment adjacent to the coffee machine. Please store these filters in a clean plastic bag or in a sealed container

Observed multiple pieces of previously used aluminum foil saved in the facility for future use. Recycle these foil sheets after their initial use

Health Report - 07/19/2011 (5 critical incident(s))

(Critical) Observed three large plastic tubs of cooked salsa on the back prep counter. Each container had one ice stick in it that was melted. The manager who had started the cooling process had left the facility for a while. At 1:40 pm the salsa was at 98f. At 2:40pm the salsa was 86f and at 3:30 the salsa was at 83 to 85f. Corrected by staff placing the salsa in a pot and reheating it to 165f on the stove top. Ensure ice sticks are replenished as needed, the product is stirred frequently and use smaller containers to reduce the volume per container. Bulk hot foods that are cooling need frequent attention. If the individual who is responsible for cooling the food has to leave, ensure remaining staff are trained and made aware of how to carry-on the cooling process

(Critical) Observed the following food contact surfaces to be unclean: 1) the interior of the customer self-service water cooler at the front counter, 2) several food storage tubs, 3) the can opener blade, 4) at least three of the insulated "coffee" pots & lids, 5) the beverage dispensing spouts on three beverage dispensers. Items 1 & 2 were cleaned immediately. Clean the remaining items prior to next use (they were sent to the dish washing station). Several bottle brushes of different diameters are needed to be able to scrub clean the spouts

(Critical) Observed a food grade hose (with a spray nozzle attached to the end of it) connected to the mop sink faucet without any back flow prevention. The end of the hose was hung over the edge of the sink with the end of the hose inside the sink. As a correction either eliminate the hose entirely leaving the original air gap between the outlet of the faucet and the overflow level of the sink), install a pressure vacuum breaker at the required height on the cold water line (install a tee fitting off of the copper supply line above the mop sink), or install a hose bib vacuum breaker between the faucet spout and the hose, and eliminate the spray nozzle (end of hose must remain open ended)

(Critical) Observed a small tub of salsa that expired the previous day (7-12 to 7-18). Corrected by discarding the salsa

(Critical) Observed an employee in the back kitchen cutting up watermelon for an ice beverage to not be wearing single use gloves. Corrected by the employee placing a glove on his left hand to prevent bare hand contact

Observed the shared dumpster to be overflowing. Have your landlord contact the solid waste hauler to have them increase the pick-up frequency

(Critical) Observed a spray can of "hot shot" flying insect killer for kitchen use to be in the facility. Although it is in small print the label does indicate this product is for home use only. Corrected by removing this insect spray from the facility

Observed a wall shelf located above a hot holding unit for cups/mugs, etc. To have a lot of construction dust/debris and small chunks of wall board accumulated on them. Ensure all equipment/food is either removed or properly protected when any construction is being performed (a small exhaust vent was recently installed in the ceiling). Remove all of these items to be washed and the shelf to be wiped down

Observed a part of the ice scoop handle (the underside of the handle where it connects to the scoop) to be unclean. The manager cleaned this scoop immediately

Ensure staff verify the chlorine sanitizer concentration with the provided test papers to ensure 50-200 ppm chlorine

Observed plastic eating utensils stored in a basket to be oriented in both directions. Please have an employee with clean hands organize these utensils so the handles are all in the same direction toward the customer

Corrected by placing the wiping towels in the sanitizer solution
