The excessively grooved cutting boards are no longer easily cleanable and need to be replaced.

You need to remember to clean the floor drain under the pop station, as there is a buildup of black slime there. Put on a more frequent cleaning schedule. Also, please clean the top of the dishmachine routinely.

The dumpster pad area is very dirty and needs to be kept on a frequent cleaning schedule. Work with the other tenants that use the dumpsters and keep it cleaned up.

The tableware storage bins on the prep line were observed dirty. You need to remember to pull the bins out and run them through the dishwasher at least once a week.

There is standing water in the dairy reach in cooler across from the smoothie station. Have unit serviced so water drains out of the unit.

(Critical) I observed one of your staff load up dirty dishes into dishwasher and then handle clean dishware without washing hands. Staff need to be washing hands before handling clean ware. Corrected through education.

(Critical) Two prep units were found holding product at 45*f. Must be holding 41*f or less. It was determined that product was at elevated temps due to staff leaving open spots on the cold rail. You need to have pans in all the slots to keep the heat from entering unit and elevating temps. Corrected by placing pans in the empty spots.

(Critical) Repeat violation 2 times in a row. Complete the risk control plan and fax into me within 10 days. Once again, i found the bagette bread in a basket without the protective liner and shield. You will need to get this recurring issue permanently corrected. I will be back in 2 weeks to verify this.

A speedrack with cookies on it was stored directly next to the front line handsink, exposing the foods to splash from handwashing. Find a safer location to store this rack. . . . Corrected by moving rack.

Please clean the pop syrup closet out more often.

(Critical) Bread was found not protected while on display. The bagette bread on the serve line not protected due to crack protected case. Get this replaced. Also, loaves of bread on a rack exposed to customer seating on the front line. You need to keep your loaves of bread on a speedrack and covered. Corrected by covering all exposed breads.

Smoothie straws stored directly under the prep sink drain lines. Store the sanitizer bucket in this location and move the straws over so they are not under the drain line. Corrected by doing such.

(Critical) The dishmachine was found not sanitizing. Upon further investigation, it was observed that both the detergent and sanitizer buckets were empty!!! managers must be checking the dishmachine supplies on a daily basis. This needs to be on your line checklists! also, staff need to be using test papers to determine that 50-100ppm chlorine in coming into the machine. Assure that this is happening! corrected by re-stocking, priming the lines and confirmed soap is coming in and sanitizer was at 100ppm.

(Critical) There is a large infestation of drain flies under the smoothie area around the floor drains. You need to have a licensed pest control operator out asap to treat all of your floor drains with a bio gel. I want you to have your pest company fax or e-mail me detailed reports of the progress of the mitigation of these pests for a month. I will be back in 30 days to re-inspect for the total absence of the drain flies. Do not store any food contact items drain lines. Keep straws in another location.

(Critical) I found cooked eggs at 120*f on the hot rail steam wells, due to triple stacking of pans. Product needs to be fully immersed within steam wells and never multiple pan stacked, or it won't hold product at 135*f or above. Corrected by discarding eggs and modifying staff habits.

Plastic forks stored with handles in both directions on the front line. Staff need to store with handles out!

Tableware containers were observed dirty. Staff need to be cleaning them in dishwasher more often. Also, keep the bins recessed under counter to protect from things falling into them.

You need to thoroughly clean the floors under the shelves in the walk in cooler and freezer.

Keep boxed foods off the floor in the walk in cooler. Also, don't use pop crates to store product on. Need 6" of clearance to floor.

You need to clean the top of the dishmachine more often.

Repeat violation 2 times in a row! you need to remember to clean the floor drain under the soda machine. Need to do this at least once a month.

You need to provide a lid for the brine tank of your water treatment system.

(Critical) Used a test kit and found the sanitizing concentrations in sanitizing buckets through out facility, which were coming from the sanitzing dispenser in the 3 compartment sink at a low concentration. Pic called for repair, sanitizing dispenser was repaired and manufacturers specification (200 ppm) was verified with test kit. Pic has been instructed to use test kit to verify proper concentration of sanitizer. , used a test kit and found the sanitizing concentrations in sanitizing buckets through out facility, which were coming from the sanitzing dispenser in the 3 compartment sink at a low concentration. Pic called for repair, sanitizing dispenser was repaired and manufacturers specification (200 ppm) was verified with test kit. Pic has been instructed to use test kit to verify proper concentration of sanitizer. On follow up inspection used a test kit and found the sanitizer solution coming from the sanitizer dispenser in the 3 compartment sink at manufacturer's specification of 200 ppm. This violation was corrected during routine inspection, and re-verified on follow up.

Violation corrected on both the 3 door sandwich prep top cooler and the 1 door worktop cooler.

Found the top of the conveyor toaster in the front counter area soiled with dust and accumulation. Found the shelves and countertops under equipment in the front counter area soiled with accumulation of debris. Found the pans holding fruit in the walk in cooler soiled with accumulaton. To correct clean as soon as possible and clean on a more frequent schedule.

Found the central area of the community dumpster enclosure damaged. To correct contact building management and have enclosure repaired.

Found the 3 door sandwich prep top cooler and the 1 door worktop cooler not holding tcs food at 41. To correct repair/replace cooler to hold tcs food at 41°F. Found the interior wall and plastic curtain door of the walk in freezer with excess frost. To correct repair/maintain walk in freezer.

Found the ceiling in the mens public restroom possibly water damaged with stains and mold. To correct repair source of leak, and repair and refinish ceiling.

Found bags of linen stored on the ground outside of the soiled linen receptacle in the dumpster enclosure area. To correct store soiled linen bags in soiled linen receptacle.

Observed a gap along the floor and door in the baking prep area. To correct eliminate gap.
