(Critical) Repeat violation 2 times in a row. Complete the risk control plan and fax or scan to me within 10 days. Once again, i found foods on the main prep line above 41*f due to improper procedures being implemented by staff. I found a pan of tomatoes "double panned" on the catering cold rail and at 51*f. I also found creamers that require to be held at 41*f or less packed in plastic cups and stored on the cold rail of catering and found at 51*f. The creamers will need to be stored inside the drawers where there is air flow to keep product at safe temps. You cannot double pan product or it won't hold safe temps. I also found 3 pans of sliced deli meats at 45*f in the sandwich prep rail. Staff did not keep in freezer in basement long enough to bring temp down below 41*f before stocking on the line. Managers need to be sure staff are following proper procedures. I will be back in 2 weeks to verify managers have staff following store procedures. All product out of temp was discarded.

Provide a small wastebasket at each handsink for ease of paper towel disposal.

Your catering prep cooler rail needs to be defrosted more frequently, so it is easily cleanable.

Staff need to be keeping plastic cutlery with the handles all in the same direction.

The floor drain crocks have a black slime growing in them. You need to clean them more frequently to prevent this slime from growing.

(Critical) The dishmachine was found not sanitizing. Staff must be using test papers to verify early in the first shift that machine is dispensing 50-100ppm chlorine. Changed out to a new bucket of sanitizer and got 100ppm. Corrected.

(Critical) I found sliced tomatoes at 49*f and a chicken salad at 51*f in the sandwich prep cooler. These items were prepped this morning and not given enough time in either the walk in cooler or freezer to cool down to 41*f or less before stocking on the lines. These items are all prepped in the basement prep area. Keep a thermometer down there and staff need to temp product before going upstairs to assure it is 41*f or less. Corrected by stocking out of temp product in walk in cooler.

(Critical) Paper towels were not available in the male restrooms(both the public and staff) and in the public female restroom. Staff must be going through restrooms and assuring sink supplies are stocked. Corrected by stocking.

Clean the top of the dishmachine more frequently.

Your dumpster area was found to be very dirty with several bags of garbage on the ground. This is a shared dumpster and all users must work together to keep the area cleaned, all garbage must be kept in the dumpster container. You may need a larger dumpster or more frequent pick-up if you don't have room to keep all garbage in container with the lids closed!!!

Replace broken thermometer in the single door upright by the walk in cooler.

(Critical) The pannini/catering cooler was found holding all product at 47* to 50*f in the top rail and in the reach in. I temp chicken, cheese, egg whites. Product must be at 41*f or less at all times. Staff need to be taking temps of multiple items in all coolers to catch if temps are unsafe. All potentially hazardous foods were discarded and you are not to use this cooler to hold any potentially hazardous foods until repaired. I will be back in 2 weeks to verify it is fixed.

Plastic forks and knives found stored with handles down in dispensers. Store them with handles up.

You need to keep your pastry rack covered as it is in a high traffic area and within the splash zone of the nearby handsink.

I observed a package of chicken thawing in a prep sink without being submerged and having cold, running water over it. Be sure staff follow correct procedures for thawing. Best practice is to pull and thaw in coolers.

(Critical) The salad prep cooler was found holding all product at 45-46*f. Must be holding product 41*f or colder. Unit is not to be used until repaired and holding 41*f or colder. All product was transported to a walk in until unit is repaired. Manager is to call me this afternoon with an update. I will be back to re-inspect this.

Provide a thermometer for the interior of the proofer walk in cooler.

You need to have functional mop holders to hang your mops up to dry after uses.

The storage room in the basement is disorganized. Get things off the floor and organized please.

There were no paper towels in the female restroom on the main level. Dispenser was broke. Stock napkins until new dispenser is installed. Corrected.

(Critical) One of the 2 cream pitchers was not time marked. Staff must be doing this. Corrected by discarding cream and marking a new pitcher.

Repeat violation 2 times in a row. Once again, the tableware storage trays are dirty. Staff need to be held accountable to clean and sanitize them before re-stocking.

You folks need to keep the prep areas cleaned throughout the day. Also, the interior of the coolers need to be cleaned once every 24 hours.

Mops must be hung up between uses to facilitate rapid drying.

(Critical) I observed chemicals stored next to the ice scoop by the ice machine. You need to keep toxic items seperated from foods/food contact items. Suggest mounting an ice scoop holder on the ice machine or on a wall near it. Corrected by cleaning ice scoop and moving chemicals to lowest shelf.

No wastebasket by the bread area handsink on the front line. Keep a basket (can be a small one) at each handsink.

Please remember to clean the tableware storage bins about once a week. Observed dirty today.

I observed plastic knives stored improperly and not protected in the self serve area. Keep handles all in one direction. Suggest putting them in a dishwasher safe cylinder and store with handles up.

(Critical) Used a test kit and found the sanitizing concentration in the sanitizing buckets through out facility either to low or high, and found the sanitizing concentration coming from the quat sanitizing in the 3 compartment sink at a very high concentration. To correct call for repair of quat dispenser, and use test kit to make sanitizing solutions to manufacturers specification of 200 ppm. Pic has been instructed to monitor that employees are checking sanitizing solutions with test kit, and changing solutions when necessary. , used a test kit and found the sanitizing concentration in the sanitizing buckets through out facility either to low or high, and found the sanitizing concentration coming from the quat sanitizing in the 3 compartment sink at a very high concentration. To correct throughly clean interior surfaces of dispensing tanks to preclude excess build up on interior sides and monitor that employees are following procedures when filling cold hold disensing tanks. , found the interior front shield, and the upper ice forming surrounding surfaces of the ice machine soiled with accumulation. Found bags of food for pick up on the floor in the roll in 2 door cooler. To correct store food orders for pick up off the floor.

Found employee clothes stored on top of pop storage rack. To correct remind employees to store their personal clothing in employee storage area.

Found the drain line of dishwashing machine vent hood, and a drain line draining from ceiling draining into the clean side of the dishwashing counter drain. To correct relocate drains to drain into the "soiled" side of the dishwashing counter drain

Found a stack of unwrapped coffee filters stored on top of coffee maker, and on top of coffee packets under the prep table. To correct store coffee filters in a more protected manner, such as in a covered box.
