Found the floor in the beer walk in cooler soiled with accumulation and build up. To correct thoroughly clean beer walk in cooler floor.

Found a partially covered container of ribs cooling on the shelf next to packages of sealed raw meats. Pic corrected by relocating ribs to a shelf above raw meats, and was reminded to cool foods on top shelves.

Found an uncovered carton of ice cream in the ice cream freezer stored below freezer door/lid which had water dripping down from possible condensation. Pic corrected by placing cover on top of ice cream and wiped lid of ice cream freezer. Pic has been reminded to keep food protected from contamination such as drips.

Found an opened case of menus stored in the employee women's restroom. To correct relocate menus to a more protected location away from restroom.

(Critical) Found the onion, lettuce and vegetable cutters soiled with accumulation of debris. Pic corrected by placing cutters in the dishwashing areas to be washed. This is violation has repeated in 3 of the last 5 routine inspections and an administrative conference notice has been issued. A soiled food contact surface risk control plan must be submitted. ,

(Critical) Found time/temperature control for safety (tcs) food, various salad dressing 45 to 49°F, shredded cheese 47°F and cut tomatoes 49°F in the 2 door prep top cooler. Found tcs food, various sauces 50°F in a cold hold well filled with water on the left side of the cooking line. Found tcs food, sauce 50°F and shredded cheese 50°F in double pans on the 2 drawer prep top cooler in the center of the cooking line. Found tcs food, sauce 54°F in a double pan in the right 2 door prep top cooler. Found tcs food, a container of shredded cheese 57°F sitting on top of a container of ice. Found tcs food, portioned cups of sour cream 50°F and cream cups 49°F in the 1 door domestic cooler. Pic corrected by discarding all tcs food above 41°F, and has been instructed not to use coolers to hold tcs food until repaired to hold tcs food at 41°F. Pic has been reminded that double panning tcs food hinders food from being held at 41°F, and if ice baths are used the ice must completely surround food container. ,

Found the 2 door prep top coolers, 2 drawer prep top cooler, and 1 door domestic cooler not holding tcs food at 41°F. To correct repair/replace coolers to hold tcs food at 41°F. Observed a heavy ice build up on the interior wall of the walk in cooler. To correct repair walk in cooler wall.

Found dispensing utensils in warm water 80 to 110°F on the cooking and prep line hot water dispensing wells. Pic corrected by replacing water with hot water and turned on hot water dispensing wells, and was instructed to monitor and ensure employees are filling wells with hot water and turning on hot water wells.

Found some of the microwaves on the cooking line soiled with splash and build up. Found the evaporator fan cover in the beer wal in cooler soiled with build up. To correct clean microwave and fan cover, and keep clean.

Found current food license not posted. To correct post current food license in a conspicuous area for public inspection.

Found the tiled floor through out kitchen with cracked, chipped tile and worn grout. To correct repair floor to be smooth and cleanable

Found the spray nozzles of the upper spray rinse arm of the glass washer clogged with debris. Pic corrected by cleaning spray nozzles.

Found no antichoking poster on the kitchen wall. To correct post antichoking poster on kitchen wall.

Found several clean, wet food containers stacked on the storage room shelf. To correct allow food containers to air dry prior to stacking and storing.

Found carpet like mats used on the floor and walkway between dishwashing area and back kitchen. Pic states these mats are cleaned and dried daily. To correct discontinue using carpet like mats and replace with mats that cleanable such as rubber mats.

(Critical) These food-contact surfaces found in need of cleaning: potato punch and ladle on clean dish storage rack. Food-contact surfaces shall be clean to sight and touch. Corrected at time of inspection by removing potato punch from wall and properly cleaning and sanitizing potato punch and ladle.

Observed several pans on clean storage shelf stacked wet. Please allow all clean dishes and utensils to air dry/drain prior to stacking

(1) some single-service items found stored improperly in various areas on shelving unit above cook line. Please store single-service items inverted or covered. (2) observed bagged clean linens stored improperly on floor next to beer wic. Please provide a shelf to store clean linens a minimum 6 inches above the floor

Two door traulsen on cook line under hood canopy found with an ambient air temperature of 52°F. There was no food stored in this unit at time of inspection. Refrigeration shall be capable of maintaining food at 41°F or below at all times. A follow-up inspection will be required when this unit has been repaired and capable of maintaining food at proper temperature. Refigeration repair person on site at time of inspection

Gaskets in poor repair on traulsen 2 door prep unit on cook line under hood canopy. Please replace to provide good seal and so smooth and easily cleanable

Please provide set screws for potato punch to remove easily from wall for cleaning. ,

Container of raw chicken stored on top shelf above seafood and container of vegetables and container of cooked noodles found stored below raw meat items. Corrected at time of inspection by relocating raw chicken to bottom shelf below and away from any ready-to-eat food items and below any raw seafood or raw beef items and relocating vegetables and noodles away from and above any raw animal foods

Observed some fruit flies near ice tea dispenser waitress station and in dry storage near bagged onions. Continue to work with your extermination company, eco-lab and maintain facility clean and dry. Keep floor drains clean. Consider storing onions in wic as discussed with pic

(1) beer wic drain line not centered over floor drain causing water build-up on floor in this area. Please re-attach drain line to terminate over drain; maintain proper air gap a minimum twice the pipe diameter above the flood rim of the floor drain. (2) one of the dipper wells at waitress station found with an air break. An air gap shall be provided at least twice the pipe diameter of the water inlet above the flood rim of the dipper well. Corrected at time of inspection. Consider cutting the drain line like the other dipper well to provide proper air gap instead of using a bracket. (3) spray nozzle attached to faucet with an atmospheric vacuum breaker from previous evening. The hose was not under pressure at time of inspection. Spray nozzle must be disconnected when not in use. ( this faucet does not have a mop sink underneath, cannot install a vented t-valve)

(Critical) Creamers found at improper temperature at waitress station. Critical corrected: at time of inspection individual creamers requiring refrigeration found at 59°F discarded. Creamers were being held in metal pan on ice at waitress station. Creamers are insulated away from the cold sides of the pan. Facility will look into purchasing "no chill" creamers; until then will store in refrigeration unit maintaining food at 41°F or below. Facility is using ice to maintain potentially hazardous food cold in a few areas throughout. Two of the units are insulated and surround the food with ice; these should not be an issue. The other two ice holding units with sour cream, whipped butter and shredded cheese do not surround the food with ice. Food items were maintaining at proper temperature since just removed from wic; facility serves dinner only; food is dispensed quickly. Management will consider using time as public health control or purchasing mechanical refrigeration or insulated ice bath for these potentially hazardous food items at the waitress station

(Critical) Two of the dipper well water inlets found without proper air gaps. Please provide a proper air gap twice the pipe diameter of the water supply inlet and not less than one inch above the flood rim of the receiving receptacle. Critical corrected: at time of inspection pic used twist ties to hold inlets up to provide proper air gap; will install more permanent fix instead of twist ties

(Critical) Open cup observed in rear kitchen. Food employees may drink from closed beverage containers. Critical corrected: at time of inspection discarded open cup. Please discuss with all employees use of closed beverage containers to prevent potential contamination. Sipper cup or cup with lid and straw as long as cup handled properly (do not guide straw to mouth)
