(Critical) Observed fried rice in table top rice hot holding unit at 120 degrees f. Pic indicates food was cooked properly about one hour before the inspection. Hot holding unit does not appear to be working properly. Pic transferred fried rice to different table top rice hot holding unit. Temperature 30 minutes after transfer was 140 degrees f. Broken hot holding unit was placed out of service by pic. Item corrected
(Critical) Observed in the basement near the sump pump no backflow prevention on the water supply line. Correct by installing an asse 1011 hose bibb vacuum breaker to prevent potential water supply cross contamination
Observed the non food contact surfaces of the wok table, deep fryer, hood ventilation filters, and stove top to have an accumulation of grease. Correct by cleaning these surfaces now and more frequently to prevent build up of grease
Observed numerous sealed capped water bottles at various locations in the restaurant. Correct by relocating these bottles to the designated employee break area

Observed ceiling in kitchen area to have water staining. A one foot square area of the ceiling is falling down. Correct by repairing the ceiling. Pic indicates he has contacted the property owner who has promised to repair when the weather is warmer.
Observed several pieces of unused equipment and other items stored in the basement of the facility. These items shall be removed as time allows to prevent harborage conditions for unwanted pests.
Observed marked distressed merchandise storage area directly adjacent to in use merchandise storage area. Correct by providing more separation between the two storage areas to prevent confusion on which products are in designated distressed area.
Observed equipment needing repair: 1) the wok table has several substantial cracks and breaks. These breaks have been sealed but the table does not drain properly and still leaks onto floor. Correct by replacing wok table. The pic indicates the wok table will be replaced in 6 months to 9 months. Please see note at the end of this report about the wok replacement. 2) the seals on the reach in cooler at the cookline have numerous cracks. These need to be replaced so proper routine cleaning of the seals can be completed. Correct by replacing the seals. 3) can opener blade appears dull. Correct by replacing blade to prevent creation of metal shavings

Relocate the thermometer to a location that can be easily seen
I observed a dry rag stored on the slicer. As discussed, the slicer must be protected from contamination. Therefore, do not store any thing on the slicer as it may contaminate it

I observed an uncovered container of dried beans. Food must be stored covered to protect it from contamination. Make sure that the staff are frequently checking food storage locations to ensure that foods are covered.
