Pic states faucet base was resecured to counter providing adequate air gap. Violation corrected. On follow up inspection found the spray rinse nozzle on the left soup kettle with a solid piping attached to the faucet lifting spray rinse nozzle inches above the flood rim of the soup kettle. Pic states faucet has been modified to provide adequate air gap. Violation corrected.

(Critical) Found spray rinse nozzle used to fill and rinse right soup kettle several inches below the flood rim of the soup kettle, and noted spray rinse nozzle mounting was loose. To correct secure mounting and provide an air gap of at least 1 inch from the spray rinse nozzle and the flood rim of the right soup kettle. Found a spray rinse nozzle use to fill and rinse left soup kettle below flood rim of the left soup kettle, without adequate backflow prevention device, that is faucet only had an atmopsheric vacuum breaker. To correct either provide an air gap where the spray rinse nozzle is at least 1 inch above the flood rim of the soup kettle or install a pressure vacuum breaker on water supply line at least 6 inches above the highest point spray rinse nozzle can rest, or a reduced pressure vacuum breaker at any height (asse 1020, 1056, or 1013). ,

Found the floor in the storage room floor soiled with seasoning crumbs. To correct clean floor and keep clean.

Found the pop gun nozzle in the outer side of the bar soiled with light accumulation. Found some interior surfaces in the ice machine soiled with light accumulation. To correct follow manufacturer's directions and clean pop gun nozzle and ice machine, and clean on a more frequent schedule.

Found compressed paper towel used to plug drain on ice chill tank in the back kitchen area. To correct repair/replace drain plug on ice chill tank with a spigot/valve. Found a gap between hood filters on the cooking lines. To correct fill gap between hood filters with spacers or properly sized hood filters. Observed water dripping from what appeared to be the drain line of the pasta cooker directly on the cooking line floor. To correct adjust equipment so water drains into floor drain. Observed the control panel on the hot water sanitizing dishwashing machine showing wash temperature but no final rinse temperature. To correct verify control panel is working so hot water sanitzing can be visually confirmed.

Found the bottom of the 4 door sectional reach in freezer soiled with accumulation. To correct clean freezer and keep clean.

Observed water stained ceiling tile in the front door area. To correct verify facility is not leaking, repair if needed and replace damaged ceiling tile.

(Critical) Found a y type shut off valve downstream of an atmospheric vacuum breaker on the mop sink faucet. To correct install a hose bibb vacuum breaker downstream of the y type shut off valve. Found the drain line of the ice bin in the waitstation without air gap and an adaptor covering drain lines to receptacle. To correct remove adaptor and provide an air gap from the drain lines and rim of the floor drain. ,

(Critical) Found the upper interior surfaces of the ice machine soiled with accumulation. Found the blade of the can opener and surrounding areas soiled with build up and accumulation. Found the pop gun nozzles in the wait station and bar area soiled with heavy build up and accumulation. To correct follow manufacturer's directions and clean ice machine, can opener and pop gun nozzles. Pic has been instructed to monitor and ensure food contact surfaces are clean. ,

(Critical) Found time/temperature control for safety (tcs) food, chicken 50°F, sausage 50°F and cheese 48°F in the top portion of the 3 door prep top cooler. Pic states employees did not properly set up ice bath in the top portion of this cooler. Found tcs food, various desserts cheesecakes 53°F in cut greens 48°F in the 4 door sectional reach in cooler. Pic corrected by discarding tcs food above 41°F, and has been instructed not to use these coolers to hold tcs food, until repaired/replaced to hold tcs food at 41°F. ,

Observed employee wearing a long neck band with keys and various items preparing desserts. To correct remove neck band when preparing food to protect food from misc contamination.

Found the ceiling in the dining area waitstation water stained and with peeling paint. To correct repair source of leak and repair and finish ceiling.

Found the air vent covers and surrounding areas in the entire facility soiled with dusty build up. Found the floor under equipment, edges and corners in the kitchen, wait stations, and bar, soiled with accumulation and build up. To correct thoroughly clean ceilings and floors and clean on a more frequent schedule.

Found a stack of utensil containers used to hold silverware stored on the floor by the silverware table. To correct store utensil containers on table off the floor. Found cases and wrapped sleeves of single use utensils stored on the floor of the storage room. To correct store utensils on shelf. Found counters of food markers indicating doneness soiled with food debris on the cooking line shelf. To correct store markers in a more protected manner.

Found various food items on the floor in the storage room and walk in freezer. To correct store food on shelves at least 6 inches off the floor.

Found the top portion of the 3 door prep top cooler and the 4 door sectional reach in cooler not holding tcs food at 41°F. To correct repair/replace coolers to hold tcs food at 41°F. Found the paper towel dispenser in the back kitchen area not working. To correct repair paper towel dispenser. Found the trim plates on cooler drawers loose. To correct repair cooler drawers.

Found no handwashing sign in the bar handsink area. To correct provide a sign reminding employees to wash hands.

Found 2 packages of thawed beef 58°F in a container of standing water in the prep sink. Pic corrected by discarding beef and was reminded to thaw frozen items and either cook or place in cold hold immediatedly.

(Critical) At the reach in cooler on the main cook line observed the following raw phf at 49-50 degrees f. : 1. Chicken breast. 2. Breaded chicken breast 3. Strip steak 4. Beef tips 5. Salmon these items had been stocked, from the properly functioning walk in cooler, into the reach in cooler at opening (about one hour before). The unit was found to be unplugged and not providing proper cooling. Pic plugged the unit in and it started cooling. After an hour the unit's air temperature was found to be below 41 degrees f. The above mentioned tcs items were rapidly chilled with ice to below 41 degrees f. Item corrected
