At the main cook line observed the ventilation filters with a heavy accumulation of grease and grime. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease. This is a repeat violation
In the dining room observed the soda machine dispensing nozzles with an accumulation of crusty grime. It appears the nozzles are not being separated from the diffusers during routine cleaning. Correct by cleaning now and on a more frequent basis. The nozzles shall be separated from the diffusers during cleaning. Clean at a frequency as described above. This is a repeat violation.
At the mop sink observed the mop not hung up to air dry. Correct by hanging mop so it can air dry

(Critical) At two different prep top coolers observed the following phf items stored above 41 degrees f: 1. Pork belly at 48 degrees f 2. Breaded tofu 3. Shredded cabbage all other phf items in the prep top coolers were 41 degrees f or below. The food items are in containers which are overly full. The tofu was in a plastic 1/9 pan nested inside a 1/6 pan. Pic corrected by reducing the amount of food stored in the pans. Item corrected

In the rear prep area observed a 2 gal container of frozen beef submerged in hot water. Pic indicates the beef is thawing. Most of the beef is still frozen with visible ice crystals. Pic corrected by submerging the container under cool running water. Thawing shall occur as outlined above. Item corrected.
Observed the floors in the following parts of the rear prep area with an accumulation of food debris and grime: 1. Under the culinary sink 2. Under the dishwashing sink 3. Under the upright refrigerators correct by cleaning now and on a more frequent basis.

(Critical) At the reach in cooler on the main cook line observed the following foods between 55 and 60 degrees f. 1. Cooked chicken 2. Cooked beef 3. Cooked pork 4. Hard boiled eggs 5. Shredded cheese 6. Cooked onions and peppers the air temp thermometer inside the unit registered 53 degrees f. The thermostat on the unit was found to be set improperly (1 of 10) as to not provide proper cooling. Pic adjusted the thermostat to a cooler setting. After a 1/2 hour all temperatures were found to be at or below 41 degrees f. Items 1-4 had recently been stocked into the cooler from a properly functioning reach in cooler and were rapidly cooled to 41 degrees f. Items 5 and 6 were discarded and replaced with fresh proper temperature product from the reach in cooler. Item corrected

On the wire rack above and next to the three compartment dishwashing sink observed several stacks of recently cleaned and sanitized storage containers stacked without first completely airdrying. Correct by properly completely airdrying after cleaning and sanitizing prior to storage

(Critical) In the rear kitchen prep area at the reach in cooler observed trays of raw eggs stored above containers of previously cooked rte chicken, shrimp, and sausage. Item corrected during inspection by rearranging the cooler so rte foods were stored above the raw eggs
At the drain line for the soda dispenser ice bin observed the drain line has slipped down into the receiving drain. It appears items stored on the adjacent shelf cause the pipe to shift. Correct by reestablishing the air gap and moving the items to prevent the pipe from slipping down again
Observed no anti choking poster displayed in the establishment. Correct by obtaining and displaying an anti choking poster
