Observed the floors in the following areas to have an accumulation of food debris: 1. Under and behind the fryers. Exceptionally heavy accumulation of breading behind the fryers is about 3 inches deep. 2. Under the prep table and reach in next to the fryers 3. Under the prep table by the dry storage reach in cooler. Correct by cleaning these areas now and on a more frequent basis. Improvement has been made since the last inspection. However, the area under and behind the fryers needs more attention. This is a repeat violation from the last several inspections

Observed the following pieces of equipment in need of repair: 1. Reach in cooler next to deep fryer door seals are cracked. 2. Frigidaire freezer a. Duct tape used to cover lid b. Both hinges are broken c. Case is cracked and insulation is exposed correct by repairing each item. The frigidaire freezer should be replaced.

Observed no protective shielding on fluorescent light bulbs in the following areas: 1. Front counter deli case used for fish display 2. Front counter deli case used for luncheon meat and dessert storage. 3. Ceiling fixtures over the food preparation and cooking area. Correct by providing approved protective shielding for the above mentioned light bulbs. This is a repeat violation from several previous inspections

Observed several unnecessary items in the facility including: 1. A dough mixer 2. An ice machine 3. A food slicer 4. Various kitchen tools 5. Old refrigerator 6. Various construction supplies 7. Boxes correct by removing unnecessary items from the facility

At the main hood observed the filter with a heavy accumulation of grease. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease

Correct by repairing wall and replacing molding. 2. Observed loose ceiling finish material and holes in the ceiling over the food preparation and dishwashing area. Correct by repairing holes and loose material. 3. In the dishwashing area observed loose and separated floor tiles. Correct by replacing tiles to provide a smooth and easily cleanable surface. At the hand sink in the dishwashing area observed the drain basket to have a slow leak causing water to puddle on the floor. Correct by repairing or replacing the drain basket to prevent water leaking on the floor

I found clean utensils stored in a lexan container, that needs cleaning. Clean tongs and other utensils must be stored in a clean, dry location. This is to protect them from contamination. Clean this storage container and re-wash the utensils.

I found that paper towels and soap are not available at both hand sinks. Proper and frequent hand washing are critical in preventing the spread of harmful bacteria. Therefore, provide toweling and soap at times for each hand sink

(Critical) Observed no soap or paper towels at any handsink in facility. Facility did have 1 container of dish soap and a 1/2 roll of paper towels at 3 compt sink. Improper/lack of handwashing and employee hygiene are the most common causes of spreading foodborne illnesses. All handsinks must have soap, paper towels, and hot water at all times. To correct, please inventory all sinks daily and restock with supplies as needed. Pic corrected by sending employee to store at time of inspection. Facility now has soap and paper towels at all handsinks. Critical corrected.

Observed ventilation hood and filters above deep fryers in cooking area soiled with grease and debris. Pic stated that he hires company to clean hood every 6 months. Since this violation has been cited several times, you will need to increase the cleaning frequency of the hood and filters. 6 months is too long

Observed almost all coolers in facility missing thermometers. To correct, please provide as soon as possible
