(Critical) The can opener blade was observed heavily soiled. Staff need to remember to wash, rinse and sanitize this after each use! corrected by doing such.

(Critical) I observed 4 cans of chafer fuel stored above breads on a speedrack in the back area. Store toxic items seperated from and always below foods or food contact items. Corrected by doing such

When doing food prep, all food staff need to have some form of hair restraint, such as a hat.

Health Report - 10/21/2011 (5 critical incident(s))

(Critical) Two pans of pasta salad were found at 55*f on the hot serve line. There was melted ice water under these pans. Improper set up. You are not permitted to serve any cold items in the hot wells. Corrected by discarding pasta salads.

(Critical) I found the deli meats and cheeses on the cold well unit at 48*f to 55*f. Must be holding below 41*f. The unit is not working properly. You are ordered not to use this unit to hold any potentially hazardous foods until it is repaired and confirmed to hold below 41*f by me. After repair, fill pans with water and leave on overnight and and have me out the next morning to check temps. All phf's were discarded.

(Critical) I found a pan of cole slaw sitting on the stove next to the grill and at 70*f. Slaw must be held 41*f or less. You cannot leave phf's out of cold holding. It must be kept in a working cooler or properly set up in an ice bath. Temporarily corrected by discarding slaw.

(Critical) I found hamburgers and chicken breasts in pans not on any heat source at 99*f. Hot items must be on heat and held at 135* or more at all times. Must keep on heat source. Corrected by discarding these foods.

(Critical) I observed a bin of clean utensils with stagnant water in it and the utensils in contact with the water. You need to be sure things are air dried before storing them. Corrected by cleaning the bin and utensils.

Large wastebasket was blocking access to the handsink on the cook line. Cannot keep this waste can there.

(Critical) I found salmon cooked from the day before at 45*f in the upright cooler on the main line. You need to safely cool hot foods as per code. Use the chart i left you to verify cooling procedures are in compliance with code. Have copies for me when i come back on june 2, 2011 to look at coolers. Corrected by discarding.

You need to change out your sanitizer solutions more often as i did not detect any sanitizer in bucket on the main cook line.

The paper towels dispenser was found empty on at the main cook line handsink. Always re-stock after depleting. Corrected.

The two top rail coolers on the cook line are not holding product at 41*f or less, as required by code. Staff are using time as health control as a temporary means. You must have these two coolers repaired by june 2, 2011. I will be back to verify they are repaired. See closing comments.

You need to clean the bins you store your kitchen utensils in more often.

Store your clean utensils with handles all in same direction.

(Critical) Found time/temperature control for safety (tcs) food, turkey 53°F, ham 54°F, and roast beef 49°F, tomatoes 50°F, and sliced cheeses 54°F in the service line display cooler. Pic corrected by discarding tcs food, and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F.

(Critical) Found an opened gallon of milk passed its use by date in the 2 door reach in cooler. Pic corrected by discarding milk, and has been instructed to check for proper rotation and check dates before using.

Found an in use ice scoop stored on top of the ice machine. To correct either store ice scoop in a clean container or in holder.

Found the kitchen back door leading to the loading dock area and the side door leading to the kitchen propped open. To correct keep doors closed.

Found a plastic squeeze bottle of water on the cooking line without label. To correct provide label

Found the display cooler on the service line not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F.

Found dead ants along the edges of the storage room floor. To correct thoroughly clean floors and clean as necessary

Found a bowl of sliced bread on the tray line of the service area without protection. To correct either store bread under sneeze guard or under a protective cover.
