(Critical) Repeat violation 2 times. Once gain, the pop nozzles and diffusers were observed with black mold build-up on them. Staff must be pulling them off and cleaning and sanitizing every 24 hours. Complete the risk control plan and fax to me within 10 days. I will be back in 2 weeks to verify the plan is implemented and working.

(Critical) I found ham, corned beef and roast beef not marked with use by dates. Need to be sure to date mark these items. Corrected by date marking.

(Critical) I observed pasta salad at 63*f and cilanto mayo at 59*f in the rear prep area. The cook was not using any temperature control on these products, which must be held at 41*f or less at all times. You need to keep in cooler between uses or properly set up an ice bath to hold product below 41*f. Corrected by discarding these items and developing a written sop for future expectations. I also found at quart of half and half left out at a front prep area and at 70*f. A small pan of cole slaw was found out of cooler and at 53*f at the sandwich prep station. All items were discarded. Staff must be held accountable to store foods at safe temperatures at all times. I will be back to verify that sop's have been written up and are being implemented in 2 weeks.

(Critical) Trays of raw shell eggs are stored right next to a case of avocados, creating a cross contamination risk. Store raw animal product below and seperated from ready to eat foods to reduce this risk. Corrected by doing such.

Found the walls, behind storage shelves, and around handsink sink area with damaged surfaces. To correct repair walls and finish walls to be durable, and easy to clean.

Found the floor (under equipment, edges, and corners) on the service line, prep area, and cooking line soiled with accumulation of debris. Found the walls and ceiling around the cooking hood soiled with dust and grease build up. To correct throughly clean floors and clean on a more frequent schedule.

(Critical) Observed employees drinking from beverage bottles with lids in food prep areas. Pic corrected by discarding all beverage bottles, and was reminded to use cups with lids and straws.

(Critical) Found time/temperature control for safety (tcs) food, country fried steak 100°F to 120°F, in the self service hot holding unit. Pic corrected by re-heating country fried steak to 165°For 15 seconds, and turned the hot holding unit to a higher setting. Pic has been reminded that hot tcs must be held at 135°F or above.

Found grease dripping on the sides and edges of cooking hood gutter. Found the crumb trays of toaster filled with bread crumbs. Found the door gaskets, doors and lids, and interior of coolers soiled. To correct clean as soon as possible, and clean on a more frequent schedule.

Observed thermometer in ice bath showing 40°F. To correct follow manufacturers directions and calibrate thermometer.
