(Critical) Observed container of housemade rte tartar sauce on shelf in upright cooler with a use by date of 4/16/15. All potentially hazardous rte foods must be used or consumed within 7 days (day 1 is prep date). To correct, please rotate stock more often and discard all expired food as needed. Pic corrected by discarding expired tartar sauce at time of inspection. Violation corrected

Observed missing dumpster lids on 2 outside trash dumpsters at end of parking lot (birds observed inside dumpster). All dumpsters must be equipped with tight fitting doors and lids to prevent entry of unwanted pests, insects and birds. To correct. Please contact trash hauler as soon as possible and replace missing lids

Observed interior of rethermalizer oven in cooking area with an accumulation of burnt on food debris in bottom of oven. To correct, please clean now and on a more frequent schedule

Observed dry storage shelves in can storage area in poor repair (shelves are rough and some plywood exposed). To correct, please repair/resurface/repaint all dry storage shelves as soon as possible so all surfaces are smooth and cleanable

(Critical) A large standing fan was in front of the handsink blocking access for routine handwashing. Relocate fan and provide extension cord if necessary to prevent the fan from being directly in front of the handsink basin. Corrected at time of inspection by relocating the fan away from the handsink allowing access for routine handwashing.

(Critical) Hot water is not supplied to kitchen fixtures due to boiler is down. The boiler is in process of being repaired. Doo not do dishes until there is sufficient hot water

After proper manual dishwashing, utensil are to be airdried and may not be laid down onto of cloth towel to absorb the moisture. If necessary place on sheet tray and you may line the sheet tray with rubber mesh matting, but a cloth towel is not approved for this purpose. Discontinue this practice of setting wet utensils on top of cloth towel

(Critical) Potentially hazardous foods shredded cheese was received at 51 degrees f. Potentially hazardous foods were transported in a non refrigerated truck without cold holding. Received at 51 degrees f. Temped at time of delivery by health inspector. Corrected by discarding cheese at time of inspection. Potentially hazardous foods must be maintained at 41 degrees f or below during transport. Once received potentially hazardous foods received at proper temperature must be immediately placed into cold holding to prevent temperature rise. Frozen scrambled eggs were received partially thawed out and temped at 29 degrees f during same delivery. Potentially hazardous foods must be received frozen solid. Revise sops and provide approved cold holding to maintain potentially hazardous foods at 41 degrees f or below during transport. Frozen foods are to be transported frozen solid.
