Observed the faucet on the handsink at the following two locations in need of repair: 1. Near the dish line 2. By the chicken cooking station at each location the faucet is loose from the sink and the connections are leaking water. Each faucet still operates and provides proper temperature water. Correct by repairing or replacing the faucets.

Portioning station at the reach in two door by the upright cooler observed the door seals on both doors to be ripped. Correct by replacing the door seals

In the kitchen on the floor in front of the warewashing machine observed two areas with broken tiles and missing floor grout. Correct by repairing or replacing the broken tile and missing grout. The pic indicates the tile throughout the whole store is scheduled for replacement this summer.

(Critical) At the mechanical warewashing machine observed no soap in the dispenser. During inspection the pic installed a new soap block in the soap dispenser. Item corrected

(Critical) In the kitchen on a cart near the saut� line observed an unlabeled plain opaque spray bottle containing a liquid. The pic indicates the liquid inside the bottle is a cleaning chemical. This bottle is identical to an unlabeled plain opaque spray bottle observed on the cook line. The pic indicated that other bottle contained cooking oil used for saut�ing food. During the inspection the pic labeled each bottle as to the contents. Also, the bottle was relocated to an approved location. All bottles shall be labeled as outlined above. Item corrected.

Observed several items located in the basement area that need to be organized and sorted. The area has become cluttered again since the last inspection. Correct by sorting items and if unnecessary to this food service operation these items should be scheduled for removal.

Observed the floor of the walk in cooler, especially at the floor wall juncture, with an accumulation of food debris. Correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris

In the basement, below the trench floor drain located by the main cook line, observed soapy/milky wastewater accumulating on the basement floor. The accumulation of water is intermittent. Evidence of moisture was observed around the floor penetration at this location. The source may be related to periodic cleaning activities or equipment use. Broken floor tiles and missing grout were noted in the area above the leak. No food storage or utensils are affected by the leak. The volume of water is only enough to create a small puddle. Correct by confirming the source of the leak and repairing or replacing the cause.

In the kitchen on the floor in front of the saut� line observed two areas with broken tiles and missing floor grout. These areas may be contributing to the leak observed in the basement and noted elsewhere in this report. The pic indicates a contractor is scheduled to complete replacement of the affected tiles and grout.

In the kitchen in front of the rethermalizer observed a section of floor with missing grout and a few broken tiles. Correct by repairing tile and replacing missing grout.

At the main cook line observed a spray bottle without a label as to the contents of the bottle. The pic indicates the bottle contains oil used for coating the saut� pans. The pic corrected during the inspection by labeling the bottle as to the contents.

In the basement, at two separate locations, observed soapy wastewater accumulating on the basement floor. The water appears to be from leaking from the first floor above: location 1: below the dishwashing area and walk in cooler location 2: below the trench floor drain located by the main cook line the accumulation of water is intermittent. No obvious active leaking pipe could be identified at either location. Evidence of moisture was observed around floor penetrations at each location. The source may be related to periodic cleaning activities or equipment use. No food storage or utensils are affected by the leak. The volume of water is only enough to create a dampness on the floor in most areas. Correct by confirming the source of the leak and repairing or replacing the cause.

Observed the dishwashing machine with a loose hose line at the top of the machine. During use of the dishmachine, water discharges from the loose hose line. The spray water from the loose hose connection splashing onto the walls of the dishwashing area of the kitchen. To correct the pic person in charge contacted ecolab to service the dishwashing machine at this facility. Observed the vacuum breaker for the garbage disposal missing a cap cover at the top of the vacuum breaker. To correct schedule service to replace the missing cap for the vacuum breaker located at the garbage disposal of the dishwashing area

(Critical) Observed fresh cut tomato in top section of preparation area refrigerator with temperature of the tomato found at 54 degrees f. Note this unit is designed to hold already properly cooled foods cold at 41 degrees f. And not to cool foods as was being observed with the nested insert pan of tomato. Note this refrigeration unit is operating properly at below 41 degrees f. As required. Pic corrected by relocating prepared tomato to proper refrigeration, and exchanging with properly cooled cut tomato. This item was corrected during this inspection. Pic corrected this item by providing tomato at a temperature of 39 degrees f. Acceptable temperature below 41 degrees f.

Obseved top section of refrigerated make unit at food cooking and preparation area of the kitchen to need mechanical repairs to function properly, pic has instructed employees to use ice as a method to hold potentially hazardous foods time/temperature control for safety foods at below 41 degrees f. As required. This is acceptable as a temporary correction as all foods in this unit were found to be cold holding at below 41 degrees f. As required. Repairs are required to be made to this unit to provide proper operation according to manufacturer specifications for cold holding of foods to 41 degrees f. Or below temperatures.
