(Critical) Observed the lubricant used for the hobart stand mixer is not food grade. The lubricant is used in the cheese cutting attachment and may contact the food product during normal use. Correct by providing an approved food grade. A follow up inspection will be conducted for this item

(Critical) At the handwashing sink adjacent to the main make line observed pizza tray carts stored in front of the handsink. The handsink is not accessible. Corrected during the inspection by removing the items blocking the handsink to make the sink accessible. At all times keep the handwashing sink clear to make handwashing convenient

(Critical) In the walk in cooler observed the following items with no datemark: 1. A container of individually house portioned packages of lunch meat. 2. The chub portion of two different lunch meats 3. Four one gallon size bags of house cooked chicken wings. 4. Several preportion containers of hommus. The pic indicates the food items were prepared in the last one to two days and will be sold within one to two more days. Item temporarily corrected during inspection by pic who put a proper datemark on the foods. Please datemark all items as required above. A followup inspection will be completed for this item

At the following locations observed the ventilation hood filters with a heavy accumulation of grease: 1. Over the fryers 2. At the pizza oven correct by cleaning the filters now and on a more frequent basis to prevent the accumulation of grease this violation was observed on the previous inspection

In the rear prep area on the ceiling above the walk in cooler door observed missing area of drywall about 3┬░Feet by 5┬░Feet. Correct by repairing this missing area

At the pizza cut table observed a raw piece of osb plywood at one end and a raw piece of lauan plywood at the other end of the table. The raw wood is absorbing grease from the area. Correct by replacing the wood with smooth and easily cleanable surfaces.

On the main make line at the reach in portion of the prep top cooler observed all three of the door seals to be cracked and split open. Correct by replacing the door seals. This violation was observed during the several inspections.

At the deep fryer area observed a heavy accumulation of grease and food debris under the fryer and on the wall behind the fryer. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease and food debris

(Critical) Did not observe proper air gap at drains for single basin food prep sink, soda machine, ice machine in front area. Correct by installing an appropriate air gap. An air gap is equal to two pipe diameters of the equipment above the drain it drains into

(Critical) Observed pop nozzles of soda dispensing machine to be soiled. Corrected by pic during inspection by replacing nozzles with clean nozzles.

As discussed at the time of inspection, keep a supply of chlorine test strips, available to check chlorine sanitizer concentrations.

I found a large container of pop stored inside the ice bin. As discussed, ice used for beverages is a food, it can not be used to keep other foods cold. This practice may contaminate the ice. Immediately discontinue this practice.

You must have at least one serv safe certified employee. This requirement has been relayed to the staff several times. You must meet this requirement by july 19, 2011. If you do not have a serv safe certified employee by this date, an office conference notice will be issued.

The syrups toppings area, needs to be cleaned. There are drips on the counter surfaces. Frequently clean this area, to maintain good sanitation.

The bottom of the microwave oven below the plate needs to be cleaned. All parts of cooking equipment must be frequently cleaned.

All single service items, like the box of cups must be stored at least six inches off the floor, to protect them from contamination.

I observed that pie tins are being re-used for cooking other foods. As discussed at the time of inspection, immediately discontinue this practice.

Disposable paper towels must be available at the handsink, at all times. Corrected at the time of inspection.

As discussed, the toilet and hand sink need cleaning. The restroom must be frequently cleaned, to promote good sanitation.

The inside of the storage cabinet used for canned foods needs cleaning. Cleaned at the time of inspection.
