Observed door gaskets on the following equipment split and in need of replacement: 1. Reach in cooler at the sub make area 2. Door on the walk in cooler 3. Reach in doors on prep cooler at pizza make area correct each item above replacing door gasket. This is a repeat violation from the last inspection. Also, at the pizza oven observed the fiberglass insulation for the exhaust gas line has split and become friable. Correct by replacing the insulation

(Critical) Observed mop water on ground outside of back door. Upon interview with pic, discovered that mop water is sometimes dumped outside facility into a storm drain. You must cease this practice immediately! mop water is defined as sewage and must be discharged to an approved sanitary sewer system. Corrected violation by educating all employees about proper mop water disposal and illicit discharge prevention practices. Pic indicated they will be dumping mop water utility sink in the located in the mechanical room. Item corrected

Observed several cases of pop stored on floor in front counter area. All food, beverages and single service items must be stored in a clean, dry location and stored a minimum of 6 inches off of ground at all times to protect from sources of contamination

Observed door gaskets on the following equipment split and in need of repair or replacement: 1. Reach in cooler at the sub make area 2. Door on the walk in cooler also the walk in cooler door does not seal completely when closed. A 3/8 inch x 4 inch long gap was observed on the latch side near the floor. For each instance above correct by repairing and or replacing door and or gasket

Observed floors at the following areas dirty with an accumulation of debris: (debris is far under the equipment near the wall floor juncture. Make sure when sweeping and mopping to reach all the way under equipment. ) 1. Under pizza make area prep cooler. 2. Under sub make area prep cooler. 3. Underneath the handwashing sink 4. Behind the pop tower and ice machine correct by cleaning now and on a more frequent basis

(Critical) Observed several pop nozzle barrels on pop machine with an accumulation of mold and debris. All pop nozzles and pop machine components must be washed, rinsed and sanitized every 24 hours or as needed and must be clean to sight and touch. Pic corrected by washing and sanitizing all pop nozzle barrels at time of inspection. Critical corrected.

Observed several interior door gaskets on reach in coolers throughout kitchen prep area split and in poor repair. To correct, please inventory all coolers and repair or replace any split gaskets as soon as possible

Observed no test strips in facility to measure sanitizer concentration. To correct, please obtain chlorine bleach test strips as soon as possible

Observed wet mop stored in mop bucket in back kitchen area. All mops must be hung up after use to allow for proper air drying

Observed interior of microwave oven in prep area (next to pizza prep unit) with an accumulation of debris. To correct, please clean now and on a more frequent schedule

Observed several cases of pop stored on floor in front counter area. Also observed sealed bags of cardboard pizza boxes stored on floor in dry storage room. Observed handsink in prep area (next to side door) not sealed to wall. To correct, please reseal/caulk as soon as possible

Observed the following areas in need of cleaning: dry storage rack in prep area (in front of wic), equipment behind pop syrup containers with an accumulation of flour, dust and debris, door handle area of upright freezer in dry storage room (sticky fingerprints). To correct, please clean these areas now and on a more frequent schedule
