(Critical) Several surfaces of the ice bin & ice making machine and the soda dispensing spouts have a slime build-up on them. Clean and sanitize these surfaces now and more frequently in the future
(Critical) 1) the interior surfaces of the ice making machine have brown and black slime build-up on them. Have an ice machine contractor thoroughly disassemble this machine for a thorough cleaning. 2) the food processor blade for the hobart food processor still had cheese on it from when it was used yesterday. This attachment is stuck on the vertical spindle. Take necessary steps to free this attachment and ensure staff are removing it for cleaning in the three compartment sink after each use
(Critical) Observed a small container of re-fried beans in the two door upright cooler that expired two days ago (6-8). Corrected by discarding
1) the door gaskets on the dough and the pizza prep coolers are torn and collecting debris. Provide replacement door gaskets on these units so they are easily cleanable. 2) the chest freezer in the back prep room is severely damaged. At a minimum, provide a replacement lid or replace with an entirely new one freezer
Observed two employee hoodies hanging from a food storage rack in the back prep kitchen. Ensure staff are only hanging their garments in approved locations away from food and single service items
The fan guards on three fans (one floor fan in the back prep kitchen area and two shelf units by the salad cooler / pizza cutting station) to have a build-up of dust. Thoroughly clean these fan guards now and more frequently in the future
One of the two wet mops was observed resting in the bottom of the mop sink. After wringing out mop heads hang them up to facilitate faster drying
The microwave oven above the pizza prep cooler had spilled and splattered coney sauce in it. Corrected by cleaning this oven
Back storage rooms of facility: observed excessive amounts of old/outdated and broken items. Begin purging items (old printers, computer monitors, chairs/chair pieces, broken paper towel dispensers, etc. ) from these storage areas

Observed the two door raetone upright cooler to be operating at a slightly elevated temperature. This unit was adjusted to a colder setting. Closely monitor this unit and have it repaired/serviced as needed. Correct w/in two days

The handsink is turned off on the cold water due to plumbing issue. Repair so that the handsink can have cold water to temper the hot water for proper handwashing. In theinterim use the handsink on the outer wall of the dishroom

(Critical) Observed the following foods without use-by dates: feta cheese, rice, alfredo cheese sauce, artichoke hearts, meat balls, portioned pork rib tips, portioned roast beef, etc. These food containers were marked with last date of use based upon when they were thawed and/or opened from their original container. Observed the portioned chili sauce to be marked with a 11 day use time-frame (chili sauce portioned on 6-3 was marked with 6-13). These containers were re-marked with 6-9

1) observed the door gaskets on the following front of the house coolers to be cracked and in need of replacement: the dough cooler and the pizza and salad prep coolers. 2) observed the exhaust hood filters to be missing. Provide replacement filter sections asap to minimize oil/grease build-up inside the hood system that increases the risk of a grease fire.
Observed the interior of the two microwave ovens to be splattered with food. Corrected by cleaning
Observed loose and cracked tile/grout at the incline in the floor located in front of the dish washing area
Observed spilled food located under the pizza oven. Thoroughly clean this area now and more frequently in the future to remove debris build-up. The wood frame surrounding the base of the pizza oven is very unclean and it is not preventing dropped food from getting under the pizza oven. Remove this wood and maintain the floor clean
Observed the wet mop head resting in the mop sink. Ensure the mop head is hung up after use to facilitate faster drying

1) observed a flattened cardboard box located under some items on the bottom shelf near the dough press. Eliminate extra cardboard from the facility as it cannot be cleaned and/or can become a pest harborage area. 2) observed a bare wood 2x6 on the floor between the table with the deep fryer and the pizza oven. Remove and replace with a material that can't rot or absorb moisture or replace it with an new piece of wood that has been sealed on all sides with polyurethane or equivalent
The drains for the ice bin and the three compartment sink are overdue for cleaning. Please clean these areas now and more frequently in the future to maintain them clean
The catch tray underneath the utensil strainer needs cleaning clean now and place on a more frequent cleaning schedule
The faucet at the 3c sink is leaking. Repair leak. The drainline for the upright freezer is plumbed to a bucket. Provide pumps and drainlines as needed to plumb to the sewer system via an airgap.
Food in the upright 2 door freezer is uncovered. Keep all foods covered for proper protection
Torn door gaskets were observed on the following units: both doors of the salad prep cooler, and the right hand door of the dough cooler. Replace these gaskets to eliminate the difficult to clean areas now that they are split open

Sliding door track on the pop unit needs cleaning. The exterior of the dough machine, including the base, needs cleaning. The mop sink needs cleaning. Clean now and place on a more frequent cleaning schedule.

(Critical) Hobart mixer: the center of the large mixing bowl (supposed to be a raised conical surface) is smashed/flattened creating creases that are not being fully cleaned. Purchase a replacement mixing bowl

(Critical) Observed portioned meat (chicken, corned beef, pork, etc. ) and feta cheese in the walk in cooler and in the kitchen prep coolers to be missing dates of expiration. Some meat of an unknown prep date was discarded. Also, observed canned chili that was portioned out today to have 7-1 as a last date of use (11 days). The dates were revised to 6-27 (7 days)
1) observed dust/mold accumulation on the fan grates in the walk in cooler. Remove and clean these grates. 2) observed excessive oil/grease build-up in the exhaust hood from the pizza oven. Clean this area asap to prevent a hood fire and place this task on a more frequent cleaning schedule

Observed the non-stick coating on the metal pot from the cuisinart rice cooker to be scratched. Only utilize a plastic or rubber/silicone utensil for dispensing the rice from this pot, and not metal. Provide a replacement cooking pot
Observed the following areas to be in need of cleaning: 1) the floor under and behind the ice machine; 2) the floor sink that receives wastewater from the three compartment sink
1) all three of the pizza prep cooler door gaskets are in need of being replaced. 2) the condensate drain pan in the reach in portion of the pizza prep cooler is bent down in the middle and is not flowing to the back right-hand corner so it can drain to the evaporator pan. Repair as needed so the water flows to the drain

The product thermometers in the store are in need of calibration. Calibrate so that the food product temperature can be properly measured.
Observed the mounting surfaces for the soda dispenser spouts and nozzles to be in need of cleaning at this time
Observed the condensate catch tray in the reach-in section of the pizza prep cooler to be full of water (and slightly damaged/bent). The drain port at the far right side of the unit is clogged. Unclog the drain port and bend the tray as needed to eliminate the low spot in the middle so the water is routed to the drain outlet
Provide a coat rack/hooks in an area away from food/equipment storage areas for staff to hang their coats/purses. Today staff are hanging their coats on a shelving unit and on top of stacks of pizza boxes
1) observed some of the wire rack shelving in the walk in cooler (especially the one with the sauces on it) to be due for cleaning to eliminate spilled food/mold. 2) the interior of the pizza prep cooler is in need of a thorough cleaning to eliminate the spilled food that has accumulated
