(Critical) On follow up inspection found tcs food. Portion cups of sauce 41°F in covered containers that were not overloaded. Pic states containers are not overloaded and covered to keep sauces at 41°F. Violation corrected. On follow up inspection found facility made salsas 41 f in the self service cold hold well. Pic states unit was repaired and they need to place ice in cold hold well to ensure tcs food is maintained ata 41°F. In addition pic states a new cold hold well has been ordered and should arrive in a few weeks. Violation is temporarily corrected. I will return to verify new cold hold well holds tcs food at 41°F. Cold hold risk control plan has been reviewed and should work if followed.

(Critical) Found no thermometer available to measure cooking, cooling, cold and hot holding temperatures. To correct provide a thermometer, preferably a thin probed digital thermometer. ,

(Critical) Found time/temperature control for safety (tcs) food, portion cups of pico and sour cream 45°F on the top portion of 2 door prep top cooler in overloaded containers. Pic temporarily corrected by placing portion cups in lower portion of cooler to cool back down to 41°F. Pic has been instructed to monitor and ensure containers are not overloaded, and tcs food is held at 41°F. Found tcs food, facility made salsas 46°F in the self service cold hold well. Pic states salsas were just restocked and temporarily corrected by placing salsas in an ice bath. To correct call for repair, and monitor and ensure tcs food is held at 41°F. These cold hold violations have repeated and a risk control plan must be submitted. ,

Found the lids of the 2 door prep top cooler stored on the floor and under the front counter. Found an opened case of single use utensils, food containers stored on the floor, under the counter. To correct store lids and utensils in a more protected mannuer at least 6 inches off the floor. ,

Found numerous wet food containers stored wet and stacked on the clean utensil storage shelf. To correct allow clean food containers to air dry, prior to stacking and storing.

Found time/temperature control for safety (tcs) food, recently prepared roasted corn pico, cooling covered in the 2 door prep top cooler. Pic corrected by placing roasted corn pico in ice baths, partially covered, and placed in walk in cooler for rapid cooling.

Found the wall behind the 3 compartment sink soiled with moldy build up. Found the kitchen floor under equipment, edges and corners soiled with accumulation and build up. To correct thoroughly clean wall and floor and keep clean.

Found the lids of the chips storage containers and bulk storage bins soiled with accumulation and build up. To correct thoroughly clean lids and clean on a more frequent schedule.

Found light in the cooking hood not working. To correct repair/replace light.

Found fryer barely under cooking hood. To correct move fryer to be clearly under cooking hood.

Health Report - 01/02/2015 (3 critical incident(s))

(Critical) On follow up inspection found tcs food, portion cups of pico and sour cream 45°F in containers on the top portion of the 2 door prep top cooler. Employee states they were portioned about an hour ago. Employee corrected by placing portion cups in lower portion of cooler to rapidly cooler, before placing in top portion of prep top cooler. These containers were not overloaded and need to be cooled prior to placing in the top portion. Violation corrected at this time. On follow up inspection found tcs food, cut lettuce 41°F in a container surrounded by ice bath. Violation corrected. On follow up inspection found tcs food, facility made salsas 39 to 40°F in the self service cold hold well. Pic states cold hold well was repaired. Violation corrected.

(Critical) On follow up inspection used a quat test kit and found sanitizer flowing from quat dispenser at about 200 ppm, between manufacturer's recommended range of 150 to 400 ppm. Pic states dispenser mixing tube was replaced. Violation corrected.

(Critical) On follow up inspection found the upper bin surfaces of ice machine reasonably clean. Pic states ice machine was cleaned. Violation corrected.

(Critical) Used a quat test kit and found sanitizer flowing from quat dispenser below manufacturer's recommended range (150-400 ppm). To correct: call for repair and use quat test kit to manually make sanitizer until dispenser is repaired. ,

(Critical) Found time/temperature control for safety (tcs) food, portion cups of pico and sour cream 47-51 degrees f in overloaded container on top portion of the 2-door prep top cooler. Pic corrected by discarding portion cups on top portion of container, and was reminded not to overload containers. Found tcs food, cut lettuce, at 46 degrees f in a container in inadequate ice bath. Pic corrected by discarding lettuce and was reminded to keep container completely surrounded by ice. Found tcs food, facility-made salsas, at 60 degrees f in self-service cold-hold well. Pc corrected by having employee discard salsas and set up ice baths to temporarily hold tcs food at 41 degrees f. To correct: repair cold-hold well to hold tcs food at 41 degrees f. ,

(Critical) Found upper ice bin surfaces of ice machine soiled with build-up. To correct: follow manufacturer's directions and clean ice bin surfaces and as needed to preclude accumulation. ,

Found lids of bulk chip storage containers soiled with greasy build-up. Found interiors of insulated delivery bags soiled. Found walk-in cooler floor soiled with accumulation of debris. Found bottom of self-service cabinets soiled with accumulation of debris. To correct: clean all noted areas and keep clean

Found the cold-hold well in the self-service area not holding tcs food at 41 degrees f. To correct: repair cold-hold well to hold tcs food at 41 degrees f or lower.

Observed several cleaned and sanitized pans stacked wet for storage. Correct by air drying dishware before stacking for storage. This is a repeat violation from the previous inspection. Pic indicates the warewashing area will be reconfigured to allow for better drying of dishware

At the warewashing area on the floor observed an old broken icemaker unit. The unit is sitting on the floor. Debris is collecting around the icemaker. Correct by removing this unit from the facility

(Critical) At the self service soda machine area observed a 1/6th pan of lemons and a 1/6th pan of limes without protection from contamination from the customer. The pans are open topped and have no lids or other guard. Pic corrected during the inspection by replacing the 1/6th pans with a roll top dispenser. Item corrected

(Critical) At the front make line, two areas with cold holding violations were observed: first a deep 2/3 pan of chopped romaine lettuce was found sitting on ice in a well built into the front make line. The well is not refrigerated. The lettuce temperature was 58 degrees f. The lettuce was stocked into the holding well at 11 am. Item corrected during the inspection by discarding the lettuce. The pic indicates the lettuce will be relocated to a different holding location. Second at the prep top cooler on the main make line observed the following cold holding food/temperature combinations: house made pico de gallo at 52 degrees f. House made chipotle ranch at 51 degrees f. These items had been in the cooler since at least 11 am. Other foods in the cooler were found to be 41 degrees or less. The food items were packaged in portion cups. The pic discarded the items and replaced them with fresh proper temperature product from the wic. A few portion cups will be placed in the prep top and the balance of the portion cups will be kept in the reach in cooler section. Both items corrected.

Vent cabinet to remove heat from cabinet to provide proper ventilation for refrigeration unit. Call for repairs to refrigeration unit if ventilation of the cabinet does not lower refrigeration temperature
