(Critical) Observed insert pans of cooked rte rice and sliced watermelon on top rail (right side) of prep unit cooler on make line with internal food temperatures of 53-61f. All other phf in same prep cooler (left side of top rails and bottom portion of cooler) were cold holding at 40f or lower. Appears right rail of cooler not functioning correctly. Pic stated that this side of cooler is mainly used to store vegetables and they will not use it to store any phf. Pic corrected by discarding rice and watermelon at time of inspection. Violation corrected.

Observed the following areas in need of cleaning: *floors under grill and fryers on cooking line (accumulation of grease and food debris). *walls and ceilings in wic and keg cooler (accumulation of debris). To correct, please clean these areas now and on a more frequent schedule

All raw animal products must be stored below and away from all rte foods to help protect foods from possible cross contamination. (Critical) Observed opened 1/2 gallon of un-pasteurized orange juice on shelf in beer/keg cooler near ice machine. Juice is freshly squeezed and packaged at another facility (plum market). Cannot serve un-pasteurized juices in your facility as customers are not informed of potential risks associated with untreated foods. Observed faucet on food prep sink in back kitchen area in poor repair (repaired with duct tape). To correct, please replace faucet as soon as possible so all surfaces are smooth and cleanable

Observed 2 pans of cooked rice, a 5 gallon container of gravy, and a pan of french onion soup in wic with internal food temperatures of 95-140f. Rice and soup were made 1. 5 hours ago and gravy was made 1 hour ago per pic. All phf must be rapidly cooled from 135f to 70f within 2 hours and from 70f to 41f or below within a total of 6 hours. Need to utilize shallower pans and ice baths/ice wands before placing hot foods into wic. Whatever cooling method you employ, staff must be monitoring food as it passes through the temperature danger zone. Pic corrected by spreading rice onto sheet pans and placing ice wands into soup and gravy at time of inspection

Observed open box of coffee creamers and clean bowls stored in splash zone under towel dispenser at handsink in salad prep area. All food and clean dishware must be stored in a clean, dry location and protected from sources of contamination at all times. To correct, please relocate food and clean bowls away from towel dispenser as soon as possible

Observed low light intensity in corner of wic (near wif door). Staff are stacking boxes on top shelves of cooler racks (near only light) thus making it hard to see in corners (had to use flashlight to see what was on shelves). To correct, please provide more lighting in wic so all areas of wic are clearly visible at all times.

Observed walls behind dishmachine with an accumulation of mold and debris (area needs a good deep clean). To correct, please clean now and on a more frequent schedule

Observed whole corn beef roast (cooked) in pan in wic with an internal food temp ranging from 85f to 107f. Corn beef was loosely covered with saran wrap and has been in cooler for 1 hour per pic. Please remember that all phf must be cooled from 135f to 70f within 2 hours and from 70f to 41f or below within a total of 6 hours. If cooking thicker cuts of meat must rapidly cool before placing into wic. Need to cut meat into smaller portions so the thicker parts of the meat can cool faster. Recommend dividing meat up and placing in shallow pans if possible. May need to utilize ice baths if food is not cooling fast enough. Regardless of cooling method utilized, staff must be monitoring foods as they pass through the temp danger zone. (Critical) Observed several severely dented cans of pumpkin pie filling and cabbage in can rack in dry storage area. All food containers must be in good condition and not exposed to sources of potential contamination. All dented cans must either be discarded or completely separated from other food items and placed in distressed merchandise area of kitchen. All pop nozzles and pop machine components must be washed, rinsed and sanitized every 24 hours or as needed and must be clean to sight and touch. (Critical) Observed 2 pans of rte gravy in steam table on prep line with an internal food temp of 115-120f. Unit was working, but observed that steam table thermostat was not turned too low. All phf must be hot held at 135f or more at all times. Pic stated that food was in steam table for less than an hour so pic corrected by reheating gravies to 165f and turning up steam table thermostat to proper setting. Critical corrected.

Observed pans (with no lids) of cooked meats stacked on top of each other on wic shelf. All foods shall be covered with proper lids when storing in wic (except when cooling) to protect food from sources of possible contamination. Stacking pans on top of raw foods could cause possible cross contamination

Observed cleaning supplies (5 gallon buckets of cleaners) stored in front of handsink in employee restroom. All handsinks must be clear and accessible at all times for proper employee handwashing. To correct, please relocate chemical buckets to another location away from handsink

Observed an accumulation of dead insects in light shield in employee restroom. To correct, please remove light and clean as soon as possible. , observed several fly strips throughout facility saturated with dead insects. To correct, please change and remove fly strips from facility more often.
