(Critical) Observed the following soiled food contact surfaces: 1. Interior ceiling of the both microwaves on the cook line, specifically the metal bar at the top. 2. Can opener blade. 3. Bin of dishes under prep table with rotting food in the bottom. 4. Tomato slicer blade. (a) equipment food-contact surfaces and utensils shall be clean to sight and touch. (pf) (b) the food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. (c) non food-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. To correct have the noted items washed, rinsed and sanitized.

(Critical) Observed the following food contact in poor repair: 1. Two ice buckets with holes in the bottom. 2. Robomax blade with chips. Multi use food-contact surfaces shall be: (1) smooth; (pf) (2) free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections; (pf) (3) free of sharp internal angles, corners, and crevices; (pf) (4) finished to have smooth welds and joints; (pf) to correct: discard the noted ice buckets and sharpen the blade to be free to chips

(Critical) Observed no paper towel at the cook/prep and dishwasher line hand sinks. Each handwashing sink or group of adjacent handwashing sinks shall be provided with: (a) individual, disposable towels;(pf) (b) a continuous towel system that supplies the user with a clean towel;(pf) or (c) a heated-air hand drying device;(pf) or (d) a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. (pf) to correct provide paper towel to the noted areas. Corrected: person in charge provided paper towels to the noted sinks

(Critical) Observed the following: 1. Dishwashing machine sanitizing at less than 10ppm chlorine. 2. Two sani buckets at 150ppm quat ammonia. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under # 4-703. 11(c) shall meet the criteria specified under # 7-204. 11 sanitizers, criteria, shall be used in accordance with the epa-registered label use instructions to correct 1. Repair the dishwashing machine to sanitize at 50-100ppm chlorine 2. Remake sanitizing buckets to 200-400ppm of quat ammonia. Partial correction: 2. Noted buckets remade to the correct concentration. Note: 1 is not corrected

(Critical) Observed the following with out date markings: 1. Opened tofu, opened on 5/24. 2. Several milks - all redated correctly. 3. Supperesta opened 5/23. 4. Capricola opened 5/23. Ready-to-eat, potentially hazardous food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded when held at a temperature of 5�c (41�f) or less for a maximum of 7 days. (pf) to correct redate all the noted foods for the correct open dates. Corrected: person in charge properly dated all the noted foods

(Critical) Observed the following past the facility use by date: 1. Kale x2 5/22. 2. Cream fresh 5/21, 5/24. 3. Sopperesta 5/19. 4. Capricola 5/19. 5. Cooked asparagus 5/24. 6. Shrimp ravioli 5/18. A food specified in # 3-501. 17(a) or (b) shall be discarded if it: (1) exceeds the temperature and time combination specified in # 3-501. 17(a), except time that the product is frozen; (p) (2) is in a container or package that does not bear a date or day; (p) or (3) is appropriately marked with a date or day that exceeds a temperature and time combination as specified in # 3-501. 17(a). (p) to correct discard all noted food into the garbage. Corrected: person in charge had the noted items discarded into the garbage

(Critical) Observed the following: 1. Blackberries with mold like growth. 2. Mascapone: 2 - 3/27. 12 5/15. - past date. 3. 3 butter milks 5/15. - past date. 4. Prosciutto 10/13/2014. - past date. 5. Two half & half 5/19. - past date. 6. Two whole milks 5/5, 5/19. - past date. 7. Bush mills whiskey had fly's floating around in bottle. 8. Dewars white label scotch had fly's floating around in bottle. 9. Cutty sark whisky had fly's floating around in bottle. Food shall be safe, unadulterated, and, as specified under # 3-601. 12, honestly presented. (p) to correct discard all noted items into the garbage. (Critical) Observed the following chemicals stored with food, togo containers and clean linen: 1. Deck cleaner. 2. Pipe weld. 3. Stainless steel polish. 4. Sanitizer hung on the pop tower. Poisonous or toxic material shall be stored so they can not contaminate food, equipment, utensils, linens, and single-service and single-use articles by: (a) separating the poisonous or toxic material by spacing or partitioning; (p) and (b) locating the poisonous or toxic material in an area that is not above food, equipment, utensils, linens, and single-service and single-use articles. To correct move noted items to the chemical storage areas. Corrected: person in charge moved all noted items to the chemical storage areas.

Observed bar fly's in all areas of the kitchens and bar. The premises shall be maintained free of insects, rodents, and other pests. The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the premises by: (a) routinely inspecting incoming shipments of food and supplies; (b) routinely inspecting the premises for evidence of pests; (c) using methods, if pests are found, such as trapping devices or other means of pest control as specified under ## 7-202. 12, 7-206. 12, and 7-206. 13;(pf) and (d) eliminating harborage conditions. Observed the following in need of repair: 1. Soap dispenser no longer attached to wall. 2. Table top sink no longer attached to table. Equipment shall be maintained in a state of repair and good condition. To correct repair all noted items.

Observed ceiling just outside of the walk-in cooler to be damaged and have peeling paint. Physical facilities shall be maintained in good repair. To correct repair and paint the noted area

(Critical) Observed several items that were just made cooling in the top loading section of the cheese cooler at the basement entrance. Cooling shall be accomplished in accordance with the time and temperature criteria specified under # 3-501. 14 by using one or more of the following methods based on the type of food being cooled: (1) placing the food in shallow pans; (pf) (2) separating the food into smaller or thinner portions; (pf) (3) using rapid cooling equipment; (pf) (4) stirring the food in a container placed in an ice water bath; (pf) (5) using containers that facilitate heat transfer; (pf) (6) adding ice as an ingredient; (pf) or (7) other effective methods. (pf) (b) when placed in cooling or cold holding equipment, food containers in which food is being cooled shall be: (1) arranged in the equipment to provide maximum heat transfer through the container walls; and (2) loosely covered, or uncovered if protected from overhead contamination as specified under subparagraph 3-305. 11(a)(2), during the cooling period to facilitate heat transfer from the surface of the food. To correct discontinue cooling in the top loadng section and place below until the food is at 41f or below. Corrected: person in charge moved the noted items below to cool properly

(Critical) Observed the following damaged storage bins: 1. 4 lexan containers badly cracked and taped. 2. 2 ice transport containers with holes in the bottom. (Critical) Observed the sani bucket is empty and not sanitizing at the dish machine. Corrected: person in charge hooked up and new sani bucker and it was noted to be working

(Critical) Observed the following items holding above 41f: a. Cooked sausage doubled stacked in the top loading cooler on the cook line at 53f for an unknown amount of time. B. Garlic butter 50f, green goddess dressing 50f, tempora 50f located in the top loading reach-in cool in the server area for under 1 hour due to top loading section not being turned on. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct: a. Discard the noted food into the garbage. B. Turn on the top loading cooler and moved the noted food to the walk-in to rapidly cool to 41f or below. Corrected: person in charge: a. Discarded the noted food into the garbage. B. Turned on cooler and moved food to a working cooler to rapidly cool to 41f

(Critical) Observed cold medicine on the clean dish storage rack in the dishwashing area. (a) except for medicines that are stored or displayed for retail sale, only those medicines that are necessary for the health of employees shall be allowed in a food establishment. (pf) (b) medicines that are in a food establishment for the employees' use shall be labeled as specified under # 7-101. 11 and located to prevent the contamination of food, equipment, utensils, linens, and single-service and single-use articles. (p) to correct discard the noted items into the garbage. Corrected: person in charge discarded the noted item into the garbage

(Critical) No quat test strips available for use in the facility. A test kit or other device that accurately measures the concentration in mg/l of sanitizing solutions shall be provided. (pf) to correct provide the noted testing kit. ,

Observed the following equipment in need of repair: 1. All gaskets throughout the facility are badly torn. 2. Top loading cooler on the server line not holding food at 41f or below. 3. Right door does not shut correctly and does not properly seal at times located on the top loading cooler on the server line. 4. Lid missing on the top loading section of the reach-in cooler in the server line. Equipment shall be maintained in a state of repair and good condition. To correct repair all the noted items. Observed the following plumbing in poor repair: 1. Back hand sink prep not working properly and leaks when turned off. Note: facility had sink off at the time of inspection, no prep was taking place. 2. Atmospheric vacuum breaker on the mop sink leaks when turned on. A plumbing system shall be: (a) repaired according to law;(p) and (b) maintained in good repair. To correct repair the noted plumbing. ,

Health Report - 04/23/2014 (4 critical incident(s))

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, an opened gallon of milk, passed it's best by date in the bar cooler without 7 day datemark. Found rte, tcs food, an open package of capricola and summer sausage in the main level waitstation, 2 door prep top cooler without 7 day datemark. Found rte, tcs food, an opened package of feta cheese without 7 day datemark in the walk in cooler. Found rte, tcs food, and opened carton on half and half without 7 day datemark in the upper level waitstation 2 door prep top cooler. Pic corrected by discarding all noted rte, tcs food and has been instructed to monitor and ensure staff are properly datemarking appropriate items. This is priority foundation violation has repeated 3 of the last 3 inspections, and an adminstrative conference notice has been issued. A risk control plan must be submitted.

(Critical) Used a chlorine test kit and found the glasswasher in the bar not chlorine sanitizing glasses. To correct call for repair and use basement dishwashing machine to wash glasses.

(Critical) Found the drain line of the bar ice bin draining into floor drain without air gap. To correct provide an air gap of at least 1 inch from the drain line and the rim of the floor drain receptacle.

(Critical) Observed employee wearing single use gloves handle phone and then continue handling ready to eat (rte) food, lettuce without changing gloves. Employee corrected by removing gloves, wash hands, and put on new single use gloves. Pic has been instructed to monitor and ensure employees are changing gloves when soiled.

Observed cups of beverages through out facility, in the basement prep areas, tables and shelves. To correct provide cups with lids and straws or equivalent and remind employee to use them in food prep areas.

Observed employee working in the basement prep area not wearing hat/hair restraint. To correct provide hat/hair restraint and remind employees to wear them.

Found opened cases of food, white fondue wafers, milk snaps and dark wafers on shelves not covered or protected. To correct either close bags and close cases protecting food or place food items in covered storage containers. Found cases of bread on the floor in the basement walk in freezer. To correct store bread on shelf at least 6 inches off the floor.

Found the door gaskets of most cooler doors torn, cracked, warped and loose. To correct repair/replace cooler door gaskets.

Found no handwashing sign in the bar handsink area. To correct provide and post a sign reminding employees to wash hands.

Found boxes of unused utensils on the floor under handsink by stairs and in the storage area. To correct remove unused utensils from facility.

Observed the presence of flies in the main level waitstation walls. To correct keep facility clean, dry, in good repair and continue fly control measures.

Found water leaking from the drain line on the basement, back prep area handsink. Found the floor drain cover in the dishwashing area missing. To correct repair drain line of handsink and replace missing floor drain cover.

Found the floor, under equipment, edges and corners in the entire facility, (bar, waitstations, and kitchen prep areas) soiled with accumulation of debris. To correct thoroughly clean floor and keep clean.

(Critical) Found the blade of the can opener in the basement prep table soiled with accumulation of debris. Found the interior front shield, the cover and sides of the ice forming assembly soiled with mold. Found the pop gun nozzle in the bar soiled with accumulation. To correct follow manufacturer's directions and clean can opener, ice machine, and pop gun nozzle. This is a repeated violation and a risk control plan has been issued.

(Critical) Found the atmospheric vacuum breaker on the faucet in the basement mopsink wrapped with a towel and leaking when water turned, with a y type shutoff attached to the sidekick device which was attached to faucet spout. To correct repair vacuum breaker and remove y type shutoff.

(Critical) Found an opened carton of half and half in the bar cooler without 7 day datemark. Found 4 opened cartons of half and half in the basement 2 door reach in cooler without 7 day datemark. Found an opened package of capicola and an opened gallon of milk in the middle level waitstation 2 door prep top cooler without 7 day datemark. Pic corrected by discarding half and half, milk and provided a 7 day ( day opened + 6 days) datemark for capicola. Pic has been instructed to monitor and ensure employees are datemarking appropriate items. This is a repeated violation and a risk control plan has been issued.

Found several open cases of chocolate on the storage shelf not covered. To correct pull plastic bags over food and close case to protect chocolate in storage.

Found the spout of the hot water machine soiled with mineral build up. To correct clean spout.

Found the floor in the upper level waitstation soiled with accumulation of debris. To correct thoroughly clean floor and keep clean.

Observed employee working in the prep area not wearing hat/hair restraint. To correct provide hat/hair restraint, and remind employees to wear them.

Found the door gaskets of the drawer and door prep top cooler cracked and warped. To correct repair/replace door gaskets.

Found an old sanitizing bucket with wiping cloths from previous day. To correct wash wiping cloths daily and remind staff at end of shift to empty sanitizing buckets and take towels to be washed.

Found the floor drain cover in the dishwashing area missing. To correct repair/replace missing floor drain cover. Found the ceiling in front of the walk in cooler door water stained, moldy and peeling. To correct repair source of leak, and repair and finish ceiling area. Found the corner wall trim in the upper level waitstation missing with exposed surfaces. To correct properly repair all noted areas and keep facility in good repair.

Pic corrected by discarding half and half and provided a 7 day datemark for the milk. Pic has been reminded to monitor and ensure appropriate items are datemarked.

(Critical) Found no quat test kit available to check and verify proper quat sanitizing solutions. To correct provide appropriate quat test kit such as (qt-40 test strips).

(Critical) Found the can opener blade in the basement, back kitchen area soiled with heavy build up. Pic corrected by taking can opener to the dishwashing area to wash, rinse and sanitize can opener. Pic has been reminded to monitor and ensure can opener is cleaned after use.

Found the basement storage area filled with old equipment and utensils which may no longer be needed for operations. To correct determine if equipment and utensils are needed and remove items no longer needed for operations.

Found the floor drain cover in the dishwashing area missing drain cover. Found the tile by the stair way leading to the basement cracked and loose. Found the edge of the entrance to the upper level waitstation damaged and chipped. To correct repair/replace noted areas and keep facility in good repair.

Found the spray nozzles of the dishwashing machine clogged with debris. To correct clean spray nozzles of the dishwashing machine.

Found the interior of the microwave soiled with accumulation. To correct follow manufacturers directions and clean microwave.

Found cases of single use utensils stored on the floor of the back area storage room. To correct store single use utensils on shelves and tables at least 6 inches off the floor.

Found the door handle on the employee restroom door in the basement broken and missing handle. To correct repair door handle.

Found the cold water valve of the faucet in the basement back area handsink not working. Pic corrected by turning cold water supply valve on, and found water leaking. To correct repair cold water valve, so cold water is provided.

(Critical) Used a chlorine test kit and found the glass washer in the bar not chlorine sanitizing glasses. To correct call for repair and either use dishwasher in the basement or the 3 compartment sink in the bar to wash, rinse, and sanitize dishes. Chemical sanitzing violation has repeated and a risk control plan (rcp) has been issued. ,

Found slight accumulation in the pop nozzles of the pop machines in the upper and lower waitstations. Found slight accumulation on the corners and sides of the interior front shield of the ice machine. To correct follow manufacturers directions and clean pop nozzles and interior surfaces of the ice machine.

Found the door gaskets of the 2 door reach in cooler torn and loose. To correct repair/replace door gaskets. Found the pop gun holster on the bar counter broken and pop gun laying in the sink. To correct resecure pop gun holster on bar counter and temporarily store pop gun in a clean container or surface.

Found the floor drain cover in the dishwashing area missing. To correct repair/replace floor drain cover.

(Critical) Used a chlorine test kit and found the dishwashing machine not chlorine sanitizing dishes, and found the sanitizing bucket empty. Employee corrected by changing the empty bucket, ran several cycles, and later verified dishwashing machine was chlorine sanitzing dishes. Pic has been instructed to monitor that employees are checking the dishwashing machine with test kit.

Found no paper towels in the paper towel dispensers in the bar and upper level wait station handsink area. To correct fill paper towels and fill when empty.

Observed an employee wearing a loose bracelet preparing and portioning food in the basement prep area. Employee corrected by removing bracelet, and was reminded to remove jewelry prior to starting work.

Found the floors (under equipment, edges and corners) in the prep areas and wait stations soiled with accumulation of debris. To correct thoroughly clean floors and keep clean.

Found several spray nozzles in the dishwashing machine clogged with debris. To correct clean spray nozzles, and clean when clogged.

Found the prep area microwave soiled with splash and build up. Found the door gaskets and door sealing areas of the upper level 2 door prep top cooler soiled. To correct clean microwave and cooler gasket and clean on a more frequent schedule.

Found employee personal food in several shelves in the basement walk in cooler. To correct designate an employee storage area in cooler and remind employees to store their personal items in designated areas only.

(Critical) Found time/temperature control for safety ( tcs) food, a dispenser of vegetable broth 58°F on the shelf above the 2 door prep top cooler in the wait station. Employee corrected by discarding vegetable broth, and remade another batch. Pic has been instructed to follow company policy and store vegetable broth once cooled in cooler at 41°F. ,

Found the upper spray nozzles of the dishwashing machine in the basement clogged with debris. Found the upper and lower spray nozzles of the glass washer in the bar clogged with debris. To correct follow manufacturers directions and clean spray nozzles as needed.

Found a bag of towels and other items on the floor of the basement storage closet. To correct store items on shelf off the floor.

Found cases of food stored on the floor of the walk in freezer. To correct store food on shelf at least 6 inches off the floor.

Found wiping cloths stored in santizing bucket by the prep table not completely submerged under sanitizer. To correct store wiping cloths completely submerged under sanitizer.

Found a partially eaten sandwich and an opened bag of chips on the dessert prep table. Employee corrected by moving sandwich to an appropriate area, and pic corrected by discarding bag of chips. This is a repeated critical violation and an administrative conference notice and a risk control plan form has been issued for eating and drinking in a food prep area. , found an open cup of beverage on the steam table in the basement kitchen prep area. This is a repeated critical violation and a risk control plan form and an administrative conference notice for unsafe food has been issued. , found time/temperature control for safety (tcs) food, an opened container of milk, unopened cartons of yogurt, unopened cartons of buttermilk passed its use by date in the walk in cooler. Pic corrected by discarding food passed its use by date. This is a repeated critical violation and a risk control plan form and an administrative conference notice for unsafe food has been issued. On follow up inspection found no tcs food in the walk in cooler passed its use by date in the walk in cooler. This violation was corrected on routine inspection, and re-verified on follow up. Observed employees taking clean utensils from utensil rack, dipping them in hot water, and wiping them dry. To correct discontinue this practice, utensils once air dried may be polished with a clean and dry cloth.
