(Critical) Used a chlorine test kit and found a very low to no chlorine residual in the front counter sanitizing bucket. Pic corrected by having employee remake fresh sanitizing buckets with the proper amount of sanitizer (100 ppm). Pic has been instructed to monitor and ensure sanitizer concentrations in buckets are maintained at proper concentrations at all times. ,

(Critical) Used a chlorine test kit and found 2 of 3 sanitizing buckets with low or no chlorine. Pic corrected by having employee remix chlorine sanitizing solutions to manufacturers specification of 100 ppm. Pic has been instructed to monitor and ensure sanitizing buckets are maintained at proper concentrations at all times.

Found the floor (under equipment, edges and corners) in the entire facility soiled with accumulation of debris. To correct thoroughly clean floor and keep clean.

Found the areas surrounding the pop nozzle of the pop machine by the drive thru window soiled with splash and build up. To correct clean areas surrounding pop nozzle and keep clean. Found the door gaskets of the some coolers and freezer soiled with accumulation of debris. To correct clean door gaskets and clean on a schedule to preclude accumulation. Found stickers and sticker residual left on the edges of food containers on the clean utensil storage shelf. To correct remove stickers and sticker residual on containers.

Found the sanitizing fill tube on the sanitizing dispenser not long enough to reach the sanitizing compartment of the 3 compartment sink. To correct repair/replace fill tube on the sanitizing dispenser to reach the sanitizing compartment. Found the cold hold cream dispenser by the drive thru window not in use. To correct repair cold hold cream dispenser to hold tcs food at 41°F or lower.

Found the labels on time control foods not very clear or legible. To correct repair label machine to print labels that are clear and easy to read.

Found the areas surrounding the pop nozzle and ice chute in the drive thru pop machine soiled with splash and build up. Found a salty like residue on the heat lamp guard and surrounding edges of the french fry hot holding area. Found the edges of the french fry cooking hood soiled with greasy build up. Found the edges and corners of the hot holding (uhc) soiled with build up. To correct clean noted areas as soon as possible, and monitor and ensure areas are being cleaned on a regular schedule.

Found stacks of american cheese slices stored on the edges of other food containers on the sandwich prep table area. To correct store cheese slices in it's own storage container.

Found the towels in the sanitzing buckets stored above the sanitizing solution. To correct either increase the amount of sanitizer or decrease the number of towels stored in clean sanitizing buckets.

Found the thermometer in the 1 door salad worktop cooler not accurate. To correct replace faulty thermometer with a properly working thermometer.

(Critical) Used a chlorine test kit, and found dishwashing machine not chlorine sanitizing dishes. To correct call for repair, and use the 3 compartment sink to wash, rinse, and sanitize dishes. ,

(Critical) Used a chlorine test kit and found no chlorine sanitizer in all of the sanitizing buckets in facility. Pic corrected by changing all of the sanitizing buckets with fresh solutions, and verified proper concentration with chlorine test kit. Pic has been instructed to monitor and ensure proper sanitizing concentration in sanitizing buckets. ,

(Critical) Found time/temperature control for safety (tcs) food, sliced tomatoes and shredded cheese on the prep table without time control marks. Pic corrected by providing time control marks, and has been instructed to monitor and ensure employees are time marking items on time control. ,

(Critical) Found a container of raw shell eggs stored next to portioned containers of salads in the bottom portion of the 2 door sectional reach in cooler. Pic corrected by placing raw shell eggs on bottom shelf below and away from ready to eat food. ,

Found cardboard lining some of the shelves in storage area. To correct remove cardboard and if desired replace with lining that is smooth, durable, and cleanable.

Found several wiping cloths left on top of the soiled wiping cloth bucket lid and on counter tops. To correct either store wiping cloths submerged under sanitizer or follow companys policy and store soiled wiping cloths in soiled sanitizing bucket.

Found a stack of coffee filters stored on top of coffee filter package. To correct store coffee filters in a more protected manner, such as in original packaging or covered storage box.

Found the sliding doors of the dumpsters open. To correct close doors and lids of dumpsters and keep closed.

Observed employee wearing bracelet preparing sandwiches. To correct remove bracelet prior to working in food prep area.

Found the hose of the quat dispenser cracked and leaking sanitizer. To correct repair/replace hose.

Found handsink in the front counter area blocked with a waste basket, and back handsink partially blocked with a stack of bun racks. To correct relocate bun racks and waste basket and keep handsinks easily accessible.

Found 2 stacks of american cheese stored on top of lid in the sandwich prep table. To correct store cheese in container.

Found the exterior sides of the smoothie pitchers soiled with smoothie mixes. Found the door gaskets and surface sealing areas of coolers soiled. Found the exterior lid of the ice bin of the smoothie machine soiled. Found the areas surrounding the ice chute and pop nozzle soiled with splash and moldy build up. Found the heat lamp guard and surrounding areas soiled with accumulation and build up. To correct set up a cleaning schedule addressing all noted areas and ensure items are cleaned.

To correct clean the exterior sides of the pitcher, and clean shield/guard of the french fry hot holding unit to preclude accumulation.

Found a stack of uncovered coffee filters stored on top of the cream dispenser. To correct keep coffee filters in it's protective packaging or a covered storage box.

Found pallets on a hand truck left in front of the back kitchen area handsink. To correct relocate pallets and keep handsink accessible at all times.

Found no handsoap in the handsink by the front counter and grill. Employee corrected by refilling soap dispenser.

Observed employees working in the prep area not wearing hat/hair restraints. To correct provide hat/hair restraint and remind employees to wear them when working in food prep areas.

(Critical) Found a shelf of cleaning chemicals stored above the "clean" part of the dishwashing machine counter and also above the sanitizing compartment of the 3 compartment sink. Pic corrected by having employee re-locate chemicals to chemical storage shelf in the storage area. This is not a repeated critical violation as this is a different location and situation.

Found the bottom of the 2 door sectional reach in cooler soiled with accumulation of debris. Found the edge of the ice bin soiled with coffee. Found splash and build up around the pop nozzle and ice chute of the pop machine by the drive thru window. Found salt residual and accumulation on (top, by the heat lamp guards and trim) of the french fry hot holding unit. Found the interior of microwave soiled with accumulation and stickers. To correct clean as soon as possible, and clean on a more frequent schedule.

Found a stack of bread stored in front of the handsink in the back kitchen area. To correct relocate bread to a location that doesnot block access to handsink.

Observed water appear to be splashing out of the bottom corner of the dishwashing machine. To correct repair dishwashing machine to properly function.

Pic corrected by removing chemical bottles and placing them in chemical storage area. Pic has been instructed to monitor that employees are properly storing cleaning chemicals.

(Critical) Found raw shell eggs stored next to and above ready to eat foods in the 2 door sectional reach in cooler in the back prep area. Pic corrected by having employee move raw shell eggs to the bottom shelf, and relocate ready to eat foods to a shelf above eggs.

Found the lids and doors of the dumpsters left open. To correct keep lids and doors of dumpster closed.

Found mop left in an empty mop bucket. To correct hang mop when cleaning has been completed.
