(Critical) Found the hot water handle and assembly missing on the faucet in the front counter/prep area handsink. To correct replace entire missing hot water assembly and provide hot water so employees may properly wash hands.

(Critical) Found cold time/temperature control for safety (tcs) food, a canister of whipping cream 58°F in the insulated holder in the front counter area, and found another canister of whipping cream 49°F in the pocket on the door of smoothie cooler. Pic states employees may have left whipping cream out of insulated container and cooler. Pic corrected by discarding whipping cream and has been instructed to keep whipping cream in coolers at 41°F. Found cold tcs food, bags of milk 44°F in the bottom section of the 2 door reach in cooler, and noted door was not fully closed, as bags of milk were not fully placed in cooler and at the edge of cooler. Pic temporarily corrected by taking bags of milk to the walk in cooler to rapidly cool. Found cold tcs food, cream 53°F in the cold hold cream dispenser by the drive thru. Pic corrected by discarding cream and has been instructed not to use cold hold cream dispenser,until repaired/replaced to hold tcs food at 41°F. Found hot tcs food, beef pattie 124°F, and mushrooms 124°F in the edges of trays that were not fully placed in hot holding units (uhc). Pic temporarily corrected by discarding beef and mushrooms. To correct hold hot tcs food at 135°F and above, that is, properly place trays of hot tcs food in the hot holding unit (uhc).

Found the floor under equipment, edges and corners in entire facility soiled with accumulation and build up. To correct thoroughly clean floor and keep clean.

Found no waste basket for paper towel disposal in the back kitchen area handsink. To correct provide waste basket.

Found various shake flavorings and toppings stored in single use cups through out front counter area. To correct store food items in commercial grade storage containers.

Found light accumulation in the interior front shield of the ice machine. To correct follow manufacturer's directions and clean interior surfaces of ice machine and clean on a more frequent schedule.

Found the cold hold cream dispenser by the drive thru not holding tcs food at 41°F. To correct repair/replace cold hold cream dispenser to hold tcs food at 41°F. Found the shake toppings dispenser (m&m's) on top of the shake machine with deep cracks. To correct replace dispenser. Found no lamp guard surrounding heat lamps in the french fry hot holding unit. To correct replace missing lamp guard. Found a wire hanging on the interior edges of the cold hold french fry dispenser. To correct properly secure wire inside of french fry dispenser.

Found no working lights in the walk in cooler. To correct repair/replace lights and remember to reinstall light cover/shield.

Found the bottom of the milk cooler under the coffee machine soiled with liquid. To correct clean cooler and keep clean. Found the top and edges of the french fry hot holding unit soiled with accumulation. To correct clean french fry hot holding unit.

(Critical) On follow up inspection pic demonstrated company procedure to cook beef patties, that is to properly place beef patties on grill and set grill to cook beef patties. I verified beef patties were all under the clam grill and cooked to 172°F and observed no raw or red parts of beef patties. Pic states she has retrained and verified employees are properly cooking beef patties. Violation corrected.

(Critical) Observed employee cook hamburger patties (8 patties) on grill and found edges of hamburger patties not fully cooked (120 to 138°F). The edges of patties were bright red, and used thermometer to break into patties edges and found them completely red. Pic corrected by discarding entire batch of hamburger patties, and has been instructed to retrain employees, and monitor and ensure beef patties are fully cooked to at least 155°F for 15 seconds. ,

Found the areas surrounding shake machine and orange juice spouts soiled with splash and accumulation. Found the top of the french fry hot holding unit soiled with salt residual. Found the shelf under and round the microwave soiled with accumulation of debris. Found numerous old stickers and residual on the bottom of breakfast microwave ovens. To correct clean all noted areas, check that staff are cleaning per schedule and monitor and ensure cleaning is taking place.

Found cardboard on top of fryer hood and freezers used as liners. To correct discontinue using cardboard to line equipment.

Note: this cooler was overloaded and had poor air flow. Pic states all foods were recently portioned. Pic temporarily corrected by moving all tcs foods to the walk in cooler for rapid cooling, and has been instructed to monitor and ensure coolers are not overloaded and holding tcs food at 41°F. Found tcs food liquid eggs 60°F and canadian bacon 50°F sitting on shelf and cooler by the grill. Pic states employee forgot to put breakfast items back in cooler. (Critical) Used a chlorine test kit and found no or very little chlorine residual in sanizing bucket in the entire facility. Pic corrected by having employee empty and remix sanitizing solutions to manufacturer specification of 100 ppm.

Found the drain line of the handsink at the end of the cooking line leaking. Found the cap on the atmospheric vacuum breaker on the utility sink faucet missing. To correct repair noted areas and keep facility in good repair.

Found no thermometer in the 1 door worktop cooler in the cooking area. To correct provide an internal hanging thermometer.

Observed some of the fryer baskets damaged with loose sides. To correct replace damaged fryer baskets.

Note: other items in both coolers were holding at 41°F and observed a container filled with ice which pic states is used to hold whipping cream during busy hours. Found tcs food, cream 46°F in the cold hold cream dispenser by the drive thru window.
