On follow up inspection found tcs food, a pan of burritos 41°F in the walk in cooler. Pic states they are reducing volume of food in the reach in cooler and storing burritos in walk in cooler. Violation corrected.

(Critical) On follow up inspection found chemical spray bottles properly stored. This violation was corrected on routine inspection, and reverified on follow up.

(Critical) On follow up inspection found all tcs food, (shredded cheese and salad mix) on prep table on a time control policy with time marks (which were all within 4 hour limits). This violation was corrected on routine inspection, and reverified on follow up.

(Critical) Found time/temperature control for safety (tcs) food, a canister of whipping cream 46°F in an insulated cream holder in the front counter area. Pic temporarily corrected by discarding whipping cream, and has been instructed to monitor and ensure whipping cream is held at 41°F, and place whipping cream in cooler if insulated container cannot hold whipping cream at 41°F. Found tcs food, 3 covered pans of burritos 44°F to 47°F in the middle to bottom shelves in the 2 door sectional reach in cooler. Pic temporarily corrected by discarding burritoes, turned cooler to a colder setting, and has been instructed to monitor and ensure tcs food is held at 41°F, as cooler shelves were stuffed and had poor air flow. ,

Found the ceiling in the back kitchen area with drain tray collecting water and various ceiling tile in the dining area water stained. To correct repair source of leak, repair damaged areas, and replace water stained ceiling tile.

Found the floor in the back storage room with pooling water from clogged floor drain. Pic states floor drain will be cleared. To correct have floor drain cleared/cleaned so water may drain properly.

Found several spray nozzles on the spray rinse arm of the dishwashing machine clogged with debris. To correct follow manufacturer's directions and clean spray rinse nozzles.

Found the areas surrounding the pop nozzle and ice chute of the pop machine in the drive thru soiled with splash and build up. To correct clean cooler, shelves, and pop machine areas, and clean on a more frequent schedule.

(Critical) Found the pop nozzles of the pop machine in the self service area soiled with heavy accumulation of debris. To correct follow manufacturer's directions and clean pop nozzles.

(Critical) Used a chlorine test kit and found all of the sanitizing buckets in facility with low or no chlorine. Pic corrected by remixing fresh buckets of sanitizer with proper chlorine levels of 100 ppm. Pic has been instructed to monitor and ensure sanitizing buckets are maintained at proper concentration.

(Critical) Found time/temperature control for safety (tcs) food, trays of burritos 48°F and steak wrap 47°F in the bottom portion of the 2 door sectional reach in cooler. Note: the top portion of cooler was filled and probably blocked air flow. Found tcs food, bulk bag of whole milk 51°F and skim milk 51°F in the coffee milk cooler. Note: observed this cooler's door would not close and seal properly. Pic corrected by discarding burritos, steak wraps, and bags of milk. Pic has been instructed not to use these coolers until repaired/replaced to hold tcs food at 41°F.

(Critical) Observed an employee take/handle an ice cream cone cup which had no protective paper band on the base of cone with bare hands, fill cone with soft serve ice cream and hand to customer. Customer rejected ice cream cone and explained to employee that her hands touched the cone and did not have protective band around ice cream cone. Employee corrected by taking another ice cream cone cup which had a paper band, fill cone and hand cone to customer touching only the paper band. Pic has been instructed to monitor and ensure ice cream cones have either paper band around base or use napkins to handle cones that have no paper band and employees touch only the paper band.

Found the door gaskets and interior bottom surfaces of some coolers soiled with accumulation and debris. To correct clean noted areas and clean on a more frequent schedule.

Found the tiled wall by the drive thru window missing tile. To correct repair/replace tiled wall.

(Critical) Found a sign (appears every 4 hours) on the smoothie machine indicating that pitchers need to be washed, rinsed, and sanitized, and employees continued to use machine. Pic corrected by having employee take pitchers to the 3 compartment sink and properly cleaned and sanitized pitchers. Pic has been instructed to monitor and ensure employees are washing, rinsing and sanitzing food contact surfaces (pitchers) every 4 hours.

Found cold tcs food liquid eggs 45°F and breakfast burrito 45°F in the bottom portion of the left two door sectional reach in cooler. Pic corrected by relocating tcs food in the bottom portion of the cooler to a working cooler and has been instructed not to use the bottom portion of the reach in cooler until repaired/replaced to hold tcs food at 41°F or lower. Note cooler shelves were filled inhibiting proper air circulation. Found hot tcs food, a tray of chicken bites at 115°F in the hot holding unit. Found a recently used (not rinsed) smoothie pitcher left under the smoothie dispensing spout. Pic corrected by rinsing out smoothie pitcher, and has been instructed to monitor and verify staff are following policy and rinsing pitcher after each use, and at least every 4 hours, wash, rinse, and sanitize pitchers.

Found the bulk cream tube in the cold hold cream dispenser cut longer than manufacturer specification. To correct cut bulk cream tube to manufacturer's specification, that is, at 45 degrees approximately 5/8 below the bottom of the valve.

Found a sign stating on the ice machine that blue gloves must be worn when scooping ice. To correct evaluate this policy as company's policy states in part " blue gloves must be worn when handling raw food products such as beef patties", ice is considered a ready to eat food and to reduce possible contamination blue gloves should not be worn when handling ice. To correct evaluate, and change policy to fit company's policy and to reduce chances of contamination.

Found the 2 door sectional cooler not holding tcs food at 41°F, and found the under the counter salad cooler under the prep table not working. To correct repair/replace coolers to hold tcs food at 41°F or lower.
