Observed upper "clam grill" lift open and saw beef pattie in corner appear to be raw and under cooked, temperature was about 135°F, observed pic take action and instruct employee to waste all beef patties. Pic then re-calibrated grill and later found beef patties appear to be cooked with no raw like appearance on beef patties with a temperature of 170°F. Pic was reminded that beef patties should be cooked to a minimum of 155°For 15 seconds, and to monitor and ensure grill is cooking beef patties to proper temperature.

Found the areas surrounding the pop nozzle and ice chute of the pop machine in the drive thru window soiled with splash and build up. Found the top edges surrounding the shake machine lids soiled with spilled shake mix. Found the spouts of shake flavoring soiled with accumulation. Found the edges of cooking hood soiled with grease. To correct clean noted areas and clean on a more frequent schedule.

(Critical) Found time/temperature control for safety food (tcs) cut leaf lettuce 56°F, various salads and cheese 50°F in the under the counter salad cooler on the prep line. Pic temporarily corrected by discarding tcs food, and plugged in and turned on cooler after discovering cooler was recently cleaned. Pic has been instructed to monitor and ensure coolers hold tcs food at 41°F, and are plugged in and turned on after cleaning. I will return to verify cooler is holding tcs food at 41°F or lower. ,

Found the drain line of the ice machine/pop machine by the drive thru window no longer strapped to pipe and without adequate air gap. To correct resecure drain line and maintain an air gap of at least 1 inch above the floor drain receptacle.

Found the microwave oven under the hot holding units with a loose handle and with cracks and a hole on the bottom. Found the spray rinse nozzle in the 3 compartment sink wrapped with duct tape. To correct properly repair/replace noted items.

Observed water dripping from around the ceiling lights around the office. To correct repair source of leak, and repair/replace all damaged areas.

Observed an employee place frozen beef patties on the grill, close top of grill, and observed parts of beef patties not covered by the top of grill. Pic was been informed of this, and corrected by discarding beef patties, reminded employee on the proper placement of beef patties. Pic has been instructed to have "new employees" be properly supervised and trained, and ensure proper cooking temperatures and procedures are achieved and followed.

Observed numerous cases of food in the walk in freezer left open and food not covered. To correct cover all food, and store in a more protected manner.

(Critical) Used a quat test kit and found the quat dispenser dispensing quat solution above 300 ppm, above manufacturers specification of 200 ppm. To correct call for repair and have quat dispenser repaired /adjusted to dispense quat at manufacturers specification of 200ppm. Pic has been instructed to use quat test kit and adjust sanitizing solution to proper concentration when needed, and until dispenser is repaired. . , used a quat test kit and found the quat dispenser dispensing quat solution above 300 ppm, above manufacturers specification of 200 ppm. To correct call for repair and have quat dispenser repaired /adjusted to dispense quat at manufacturers specification of 200ppm.
