Pic states beef patties were just prepared and placed in the walk in cooler to rapidly cool. Pic corrected by discarding all tcs food above 41°F in this cooler, and was instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. Found tcs food, an opened carton of half and half 49°F, and an unopened carton of half and half 46°F in the 2 door under the counter cooler in the bar. (Critical) Used a quat test kit and found quat sanitizing solution flowing from quat dispenser and in quat buckets in cooking/prep areas below manufacturer's recommended range of 150 to 400 ppm. Pic corrected by calling for repair, a repair tech arrived during inspection, and repaired quat dispenser to dispense at 200 ppm. Pic states she had checked quat sanitizer in the morning. Violation corrected.

Observed the presence of flies in bar, waitstations, kitchen cooking and prep areas and dishwashing areas. To correct keep facility clean and dry, in good repair and continue fly control measures.

Asked employee how slicer is cleaned and found facility is washing slicer with soapy water and then rinsing with sanitizing solution without rinsing slicer with clean water. To correct retrain all employees to properly wash slicer, that is to wash slicer with soapy water, rinse slicer with clean water, wipe down all surfaces with sanitizing solution, and allow slicer to air dry.

Found clean food containers on clean storage shelves soiled with old labels. To correct remove labels prior to washing food containers. Found the door gaskets, top, sides and bottom interior surfaces of most coolers soiled. To correct thoroughly clean coolers. Found the hood filters in the main cooking soiled with burn on, and the hood filters in the back cooking hood soiled with dust build up. To correct properly clean hood filters.

Found the floor under equipment, edges and corners on the cooking and prep areas soiled with accumulation and build up. Found the air vent covers in the kitchen ceiling soiled with build up. To correct clean floor and air vent covers and keep clean.

(Critical) At the in use mechanical dishwashing machine observed the residual chlorine concentration of the final rinse at 0 ppm. The sanitizer reservoir is full and the tubing is properly routed. No sanitizer could be pumped after several minutes of priming the machine. The unit was put out of service. During the inspection a maintenance contractor was called to repair the machine. Contractor arrived and repaired a faulty pump tube. Item corrected

At the handsink in the food preparation area observed the drain pipe to be leaking a significant amount of water when the handsink is used. The water runs down to the floor and into a floor drain. No contamination of food was observed. Correct by repairing the drain so it does not leak

(Critical) At the main cook line observed a food worker preparing several heads of iceberg lettuce. Observed the same worker place a 1/2 sheet tray containing several raw hamburger patties on the same preparation surface next to the lettuce. Observed the same worker place a case of raw bacon on a food prep surface used for rte foods. Corrected by the pic during the inspection. Pic reminded the food worker on proper separation of raw animal products and rte foods. Also, the affected food preparation surfaces were washed rinsed and sanitized.

Observed the following floor areas with an accumulation of dirty water, food debris, and grease: 1. At prep area under ice cream cabinet, salad prep refrigerator, and prep table. 2. At main cook line under chargrill, stove top, and fryers. Correct by cleaning each area now and more frequently to prevent the accumulation of soil

At the reach in refrigerator portion of the prep top cooler across from the chargrill observed the doors of the unit to have loose hinges and broken door seals. Facing the unit the far left door has become so loose it does not seal properly. Correct by repairing or replacing the door hinges and door seals
