(Critical) Observed a large container of sliced pepperoni in the walk in cooler to be beyond its date of consumption markings by 3 days. Corrected by discarding this container of pepperoni into the trash. Corrected

(Critical) The following pieces of equipment on the clean equipment storage shelf were found to be soiled with small pieces of food debris: the tomato slicer and the green pepper slicer. The can opening blade at the rear prep table was also found to be soiled. Corrected by taking these pieces of equipment to the 3 compartment sink and properly rewashing them until they were all clean to the site and touch. Corrected.

Observed the cold water knob for the handsink at the front prep area to not be functioning. To correct, please have faucet repaired/replaced so the cold water knob properly functions.

Observed two bulk working containers of food without labeling. Corrected by properly labeling these containers with the common food name of its contents. Corrected

Please have an employee at this facility obtain their certified food manager certificate. This facility has 90 days to have one of their employees become certified.

Observed the lower conveyor for the pizza oven to not be operational. The person in charge indicated that this portion of the oven very recently broke (today) and a repair technician has already been scheduled to repair it. Please have oven conveyor repaired as soon as possible. Observed the condensate line in the walk in cooler where it runs perpendicular to the wall is not insulated. To correct, please insulated this line to prevent condensate from developing on the outside of this line and then drip onto food and utensils below it. To correct, please have above mentioned items repaired as stated above

(Critical) The pizza cutter, spatula and the storage tray for the pizza cutter had not yet been cleaned and sanitized for the day. It has been 4. 5 hours since the store opened. Corrected by cleaning and sanitizing these items. Ensure all staff are made aware of this requirement. Consider using a timer to remind staff this equipment needs cleaning during the day at intervals less than four hours

(Critical) Observed a chemical spray bottle hanging from the end of a wire rack shelving unit that is used to store single service items. Corrected by relocating the spray bottle to the back kitchen area on the bottom shelf with other chemicals

(Critical) No chlorine test papers are available in the facility for staff to verify the strength of the sanitizer

(Critical) Again, no paper towel is provided at the back kitchen prep / dish washing area. The manager does not have a key for the dispenser. Provide a replacement dispenser and maintain paper towel at this station at all times

Observed water weeping out of the top of the water softener. Repair as needed to eliminate the water on the floor

Observed a home-made wooden shelving unit in the back hallway to store misc. Items. All surfaces are required to be smooth and easily cleanable. Seal the exterior surfaces of this shelving unit (especially the bottoms of the legs) with a sealant such as polyurethane so it is smooth and easily cleanable or replace with a commercial metal/plastic shelving unit (the bottom shelf must be at least 6" off of the floor to facilitate cleaning)

Observed a partial mesh bag of onions resting on the walk in cooler floor. Corrected by relocating these onions off of the floor

(Critical) Paper towel is not stocked at the back dishwashing/prep room hand washing sink. Always maintain paper towel at this station

(Critical) Observed a small amount of a bleach solution in a spray bottle used for sanitizing food contact surfaces that was mixed way too strong or not diluted at all. This solution bleached the color out of the test paper. Corrected by diluting the solution to 175 ppm

(Critical) Observed a spray bottle labeled oven cleaner that actually contained blue glass cleaner. Corrected by removing the glass cleaner from the bottle. Ensure employees are careful when filling/re-filling chemical spray bottles that the correct chemical is added to a pre-labeled bottle and/or a proper label is applied to a new bottle when adding the chemical

(Critical) Observed an employee prepping lettuce for salads without single use gloves. Corrected by having the employee put on gloves for this task

(Critical) The back dishwashing/prep area hand washing sink does not have a soap dispenser. Provide and maintain soap at this hand washing sink.

Currently no full-time manager (30 hours per week minimum) has a manager level food safety certification. Within three months a manager must obtain this certification

The door gasket for the upper dough cooler door is torn/missing. Provide a replacement gasket for this unit

Observed a few metal and plastic food containers stored stacked on the wire rack shelving while still wet. Please do not nest these pans until they have fully air dried

The following door gaskets are torn and in need of replacement: the left hand door of the pizza prep cooler, and both doors of the dough cooler. Replace these to provide a smooth and easily cleanable surface

Observed the floor drain covers and the piping below the floor level to be in need of cleaning at this time and more frequently in the future to remove debris build-up. Also, ensure the dust pan is cleaned out frequently to remove build-up

Provide a new rubber spatula to replace the current one that is showing some signs of wear

(Critical) Most of the "clean" dough trays in storage have an oily residue on them still. Re-wash these trays in a fresh solution of hot soapy water. The can opener blade was found unclean, and a 1/2 pan containing multiple utensils was found with a little standing water in the bottom of it that also had crumbs/debris build-up. These items were sent to the dish washing sink to be re-cleaned.

(Critical) A separate supply of salad toppings is not available in the refrigerator. Consequently, staff are using the pizza and hot sub sandwich toppings that they dispense with bare hands to make salads. Gloves are only worn to dispense these toppings when making salads. Correct by providing a set of small food storage containers with lids to store your salad ingredients. Staff will need to use single use gloves or a suitable utensil when prepping all foods and when dispensing ingredients from these small food containers for salads.

(Critical) Staff indicated the pizza cutting tool and its tray, and the food spatula at the pizza cutting table are only cleaned at the end of each night. Since these tools contact potentially hazardous foods (meat, cheese, cooked vegetables) and are stored out at room temperature they need to washed, rinsed and sanitized, or switched out with clean ones at least every four hours. Educate staff on this requirement and implement a system to keep track of the four hour time frame, such as a timer

Observed loose and damaged floor tile surrounding the floor sink located in the corner of the back dish washing area (adjacent to the walk in cooler). Currently water is pooling up around the drain. Stagnant water with debris accumulation can cause odors as well as be a breeding area for pests. Repair as needed

The dumpster is still missing the two covers. Contact your solid waste hauling provider and have them replace the lids and/or dumpster

An opening in the back wall (located in the furnace room) for pipes to enter the building is not properly sealed off to prevent pest entry. Seal this opening with a pest proof material

Observed a certificate of achievement for a manager level food safety course for the general manager (garrett), however his actual serv-safe certificate is not posted. Provide me with a copy of his certificate.

The dumpster is overflowing. Contact your solid waste hauler for an early pick-up. Your dumpster needs at least weekly pick-up to help control fly breeding and to keep the trash contained

Observed +/- 10┬░Fiberglass dough trays among many others in the two stacks of "clean" trays that have been damaged/worn through at the corners. Eliminate the use of these damaged trays

Observed a bucket on the back prep counter containing towels and water. No chlorine residual was detected. Provide a fresh chlorine sanitizer solution

1) the toilet is running water constantly. Repair the float, fill valve and/or tank flapper as needed to eliminate the water leak. 2) the condensate drain pipe for the furnace is leaking at the connection to the furnace causing water damage to the adjacent wall. Eliminate the water leak as needed

Although both employees present believed there were chlorine test papers in the facility, neither one could locate them. Purchase and/or locate your chlorine test papers so employees know where they are and store them in a location where they are accessible and will stay dry

No soap is provided at the back kitchen hand washing sink between the dish washing sink and the prep counter

Observed several gaps in the foundation adjacent to the back delivery door where pipes enter the building. One of the openings is partially covered by duct tape. Remove this tape and properly seal the gaps to prevent pest entry

Observed trash piling up above the dumpster. The owner indicated business has been slow up until recently and the dumpster is only emptied 2x per month. The dumpster is scheduled to be emptied in two days. Contact your waste hauler and have them increase the frequency of the pick-ups

Observed the floor sinks in the back kitchen (serving the three comp. Sink and the hand washing sink) to be overdue for cleaning. Please clean these drains now and at least 2x per week
