(Critical) Corrected: facility provided documentation showing the plumbing repairs that had been done and documentation from serve pro. Facility states that walls, cove base molding and flooring that has been damaged as a result of the incident will be repaired and or replaced
(Critical) Observed an active sewage leak in the facility at the time of inspection. The plumber on site stated that the sewage was due to a blockage further down the line. The facility has voluntarily agreed to close. Sewage shall be conveyed to the point of disposal through an approved sanitary sewage system or other system, including use of sewage transport vehicles, waste retention tanks, pumps, pipes, hoses, and connections that are constructed, maintained, and operated according to law. (p) to correct continue working with the plumber on site to repair the sewage leak and then hire a disaster clean up crew to clean and sanitize the entire facility, including the basement steps and flooring. ,
(Critical) Observed the hand sink being used to dump coffee product. (a) a hand washing sink shall be maintained so that it is accessible at all times for employee use. (pf) (b) a hand washing sink may not be used for purposes other than hand washing. (pf) to correct discontinuing using hand sink for anything other than washing hands. Corrected: person in charge instructed employee to only use noted sink for washing hands
(Critical) Observed 6 gallons of milk between 44-45f in the upright one door reach-in cooler for an unknown amount of time. Potentially hazardous food that is be cold shall be held at 41f or below. (Critical) Observed no unit thermometer in the one door upright reach-in cooler. In a mechanically refrigerated or hot food storage unit, the sensor of a temperature measuring device shall be located to measure the air temperature or a simulated product temperature in the warmest part of a mechanically refrigerated unit and in the coolest part of a hot food storage unit. To correct: purchase a unit thermometer for the noted cooler and place at the bottom.
Observed the following: 1. One door upright cooler was over packed and blocking air flow to the bottom shelf. 2. Gasket torn on the left door of the two door reach-in cooler under the espresso machine. 3. Gasket torn on the one door upright cooler behind the counter. 4. Basement freezer has excessive frost build up. Equipment shall be maintained in a state of repair and good condition. To correct: 1. Unblock air flow to the noted upright cooler. 2,3. Repair or replace the gaskets. 4. Defrost freezer. Partial correction: 1. Cooler was rearranged to allow proper air flow to lower shelves. Note: 2-4 not corrected.
Observed wiping clothes being stored on the counter outside of sanitizer solution. Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be: (1) maintained dry; and (2) used for no other purpose. (b) cloths in-use for wiping counters and other equipment surfaces shall be: (1) held between uses in a chemical sanitizer solution at a concentration specified under # 4-501. 114; and (2) laundered daily as specified under # 4-802. 11(d). (c) cloths in-use for wiping surfaces in contact with raw animal foods shall be kept separate from cloths used for other purposes to correct put wiping cloth into sanitizer and always keep in use wiping cloths in sanitizer. Corrected: person in charge put noted item into sanitizer bucket.

(Critical) Found inside of blender pitcher in use to have an accumulation of food debris on the lip of the pitcher and inside the lid of the pitcher. Corrected - blender pitcher was washed, rinsed and sanitized at the time of this inspection. Staff is reminded to thoroughly wash/scrub, rinse, sanitize and allow blender pitchers to air dry before putting them back into service. Having additional blender pitchers available will help ensure correct cleaning protocol is followed
(Critical) Found an almost full half gallon container of 1/2 and 1/2 in the prep unit holding @ 62*f. Corrected - this half gallon was discarded. All other milk products and phf were holding at the correct temperature. Staff is reminded to keep containers under refrigeration at all times when not dispensing from them
Found the blender bases to have an accumulation of debris on them which is then transferred to the underside of the blender pitcher. To correct - clean the blender bases now and keep on a more frequent cleaning schedule
Found clean syrup bottle spouts to be stored in an open coffee cup that had water and debris at the bottom of the cup. Also found boxes of single service items stored directly on the floor in the basement. To correct - clean spouts must be kept in a protected manner safe from splash or debris. A covered cup or zip top bag will work in this instance. Store boxes of single service items on shelving at least 6' above the floor
Found both outside doors to be propped open at the time of this inspection. Both doors were tied to the railing and look as though this practice is used often by the appearance of the knotted rope and scarf that are tied to the railings/doors. Temporarily corrected - doors were untied at the time of this inspection. Door openings must have screened, self-closing doors installed if ventilation is desired, or current doors must be fitted with self-closing devices. Doors are not to be propped open
Found the floor under the ice machine and near the floor drain to have an accumulation of debris on/around them. Also, the shelves in the basement when syrup bottles are stored are dusty and dirty to the touch. To correct - clean these areas now and keep on a more frequent cleaning schedule
Found the ceiling fan and the fixture over the espresso machine to have bulbs that are not protected. To correct - provide coated bulbs (bulbs may be coated in ceiling fan, could not tell due to height) or covers/shields in these areas
