Pizza prep cooler: one of the two air circulation fans was inoperable. The owner called for repairs during the inspection. Asap repair this fan to provide proper air circulation. Also, a new fan guard is required for this inoperable fan
Observed the two utensil storage drawers for the prep counter to have food crumbs in them. Clean these drawers now and on a regular basis
Observed the mop sink basin to be very unclean. Also a rag was partially blocking the drain causing water to pool in this basin. Clean this basin asap and maintain clean on a daily basis

Observed doors on two, and a lid on one of the three shared dumpsters to be left open. Please maintain these lids and doors closed to keep pest out of the bagged food waste.

(Critical) The container used to portion and weigh potentially hazardous foods on the scale is not being cleaned at the required frequency. This container is cleaned at the end of the day. As it is used in contact with potentially hazadous foods it must be cleaned at least once every four hours. Corrected by discussion and cleaning or changing out container to a new cleaned container within four hours. Make sure that all employees are aware of this requirement.
Gaskets found on the doors of the dough cooler are torn. Once torn, they are no longer cleanable. Replace split gaskets
