Observed bathroom doors not self closing. Except where a toilet room is located outside a food establishment and does not open directly into the food establishment such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door. To correct provide self closers to the noted doors.
Observed several pieces of equipment and items that are no longer in use in the facility. The premises shall be free of: (a) items that are unnecessary to the operation or maintenance of the establishment such as equipment that is nonfunctional or no longer used; and (b) litter. To correct go through equipment and remove unused equipment from the facility.
(Critical) Observed the following potential cross contamination: 1. Raw chicken stored on raw pork. 2. Ready to eat pan of strawberries stored on top of a box of raw bacon. Food shall be protected from cross contamination by: (1) except as specified in (1)(c) below, separating raw animal foods during storage, preparation, holding, and display from: (a) raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables,(p) and (b) cooked ready-to-eat food; (p) to correct rearrange shelf by final cook off temperatures and keep the ready to eat foods up and away from raw products. Corrected: person in charge arranged the cooler with raw foods stored by cook off temperatures and moved the ready to eat foods up and away from the raw product
(Critical) Observed non-pasteurized eggs being used in a retirement community. In a food establishment that serves a highly susceptible population: the following foods may not be served or offered for sale in a ready-to-eat form: (p) (1) raw animal foods such as raw fish, raw-marinated fish, raw molluscan shellfish, and steak tartare, (p) (2) a partially cooked animal food such as lightly cooked fish, rare meat, soft-cooked eggs that are made from raw eggs, and meringue; (p) and (3) raw seed sprouts. (p) to correct replace the noted eggs with pasteurized eggs. Corrected: person in charge removed and ordered new pasteurized eggs for use in the facility.
Observed raw seafood thawing at room temperature. Potentially hazardous food (time/temperature control for safety food) shall be thawed: (a) under refrigeration that maintains the food temperature at 5�c (41�f) or less; or (b) completely submerged under running water: (1) at a water temperature of 21�c (70�f) or below, (2) with sufficient water velocity to agitate and float off loose particles in an overflow, and (3) for a period of time that does not allow thawed portions of ready-to-eat food to rise above 5�c (41�f) to correct thaw under cold running water. Observed the floor and floor wall juncture soiled under equipment, especially the dish/prep line. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct clean the noted areas and maintain them clean.
Found opened cases of coffee packets stored on the floor under and beside the coffee machine. To correct store coffee packets on shelf at least 6 inches off the floor.
Found some of the hood filters in the cooking hood soiled with greasy build up. Found shelf under prep tables and drawers soiled with accumulation. To correct clean noted areas as soon as possible, and clean on a more frequent schedule.
Found the cold hold milk bulk dispenser heavily soiled around the spouts and not turned on. Pic states they no longer use cold hold milk dispenser. To correct determine if this cold hold dispenser is needed for operations, and remove if no longer needed.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, cut watermelon, cantaloupe, and honey dew, sliced turkey and cut ham, a container of tuna salad, and cottage cheese without 7 day (day opened + 6 days) datemark on the shelves of the walk in cooler. Pic corrected by providing datemarks, and has been instructed to monitor and ensure employees are datemarking appropriate items.
(Critical) Found the under side of the slicer blade and surrounding areas on the slicer soiled with accumulation of debris. Found the can opener blade soiled with accumulation. Pic corrected by taking items to the dishwashing area to be properly washed.
Found the evaporator coil in the cooler, under the cold hold unit on the service line with heavy frost build up. To correct defrost cooler and maintain in good working order.
Observed an employee with long hair in front of face and above food that was being portioned in the kitchen prep area. Pic corrected by having employee stop portioning food, tie up hair and wash hands before re-starting work.
Observed an employee bottle of water which requires employee to use hands to handle lid and drink from bottle possibly contaminating hands on the prep table in the cooking/prep area. To correct either use a closed beverage container that prevents contamination of hands, such as a cup with lid and straw in food prep areas or drink in designated areas only.

Found soup in cups cooling covered on the top shelf of the reach in cooler. Pic corrected by removing covers to allow soup to rapidly cool. Pic has been reminded that tcs food must be cooled from 135°F to 70°F in 2 hours and then from 70°F to 41°F in 4 hours.

Pic corrected by marking "do not use" on egg salad and placed in walk in cooler for return to supplier for credit, and discarded the cartons of orange juice passed its stamped date. Pic has been instructed to monitor that employees are checking dates when receiving , stocking, prior to use, and when rotating stock. ,

Refrigerated bulk milk dispenser not maintaining milk at proper temperature. Refrigeration shall be capable of maintaining food at proper temperature.
Please provide proper air gap on food prep sink in kitchen. Currently air gap is not twice the pipe diameter above the flood rim of the drain. Consider installing larger receiving receptacle and installing plexi-shield divider under sink if any splash onto clean dish storage area
