Found wiping cloths left on top of counter tops. To correct store wiping cloths completely submerged under sanitizer after each use.

Found employees working in cooking and prep areas not wearing hat/hair restraint. To correct provide hats and remind employees to wear them.

Found crab rangoon on a cardboard box in the walk in cooler. To correct replace cardboard box with commercial grade food pans and trays.

Found the floor (under equipment, edges and corners) on the cooking line soiled with accumulation and build up. To correct thoroughly clean floors and clean on a more frequent schedule.

Found several wiping cloths stored on the counter tops of the cooking line. Pic corrected by placing wiping cloths in a sanitizing bucket, and has been instructed to monitor and ensure wiping cloths are placed in sanitizing bucket after each cleaning task.

Observed employee cutting chicken, sharpen cleaver/knife and then attempt to resume cutting chicken without cleaning and sanitizing cleaver. Corrected by instructing employee to wash, rinse, and sanitize cleaver before continuing to cut chicken

Found the edges and surrounding areas of the cooking hood soiled with greasy build up.

Found the drain line of the steam table no longer on top of brick which elevates drain line and provides adequate air gap. Pic corrected by placing drain line on top of bricks which provide adequate air gap. Found the drain lines of the ice machine fallen off of stand and laying in floor drain receptacle. Pic corrected by resecuring drain lines on stand which provides adequate air gaps. Pic has been instructed to monitor and ensure air gaps are maintained at all times.

(Critical) Observed open cups on the counter tops through out facility, a mug of beverage on the steam table, cups (with out lids) on the counter top of the back steam table. Pic corrected by removing cups from food prep areas, and has been instructed to monitor that employees are either drinking in the dining area or provide cups with lids and straws if drinking is desired in the food prep area.

Found cardboard boxes use to hold various foods on the shelves of the walk in cooler. To correct replace cardboard boxes with food grade trays, pans, and containers.

Found the floor (under equipment, edges and corners) on the cooking line soiled with greasy build up. To correct thoroughly clean floor and clean on a more frequent schedule.

Found opened bags of rice stored on the floor in front of food storage shelves. To correct store food on shelves at least 6 inches off the floor.

(Critical) Found time/temperature control for safety (tcs) food, diced roast pork 50°F in the top portion and dumplings 47°F in the bottom portion of the left 2 door prep top cooler. Pic corrected by discarding pork and dumplings, and has been instructed not to use this cooler to store tcs food, until repaired/replaced to hold tcs food at 41°F or lower.

Found several large sacks of cabbage laying on the back kitchen floor. To correct place cabbage on shelf at least 6 inches off the floor.

Observed deep fried chicken nuggets on cardboard boxes. Pic corrected by placing chicken in plastic tubs, and was instructed not to use cardboard/boxes to hold food, but food grade utensils. Found cardboard being used thorough out facility to line items on shelves. Discontinue using cardboard as surfaces should be durable, smooth, and easy to clean.

Found the front door left in open position. Pic corrected by closing door. Found the back screen door damaged with a hole in screen. To correct keep front door closed and repair screen door.

Observed water spraying and dripping from the right faucet of the 3 compartment sink. To correct repair/replace faucet.

Observed employee working in the cooking/prep area not wearing hat/hair restraint. To correct provide hat/hair restraint, and remind employees to wear them

Found the left 2 door prep top cooler on the cooking line not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower.

Found the sides and edges of the cooking hood soiled with greasy build up. Found the door gaskets of coolers soiled with accumulation. To correct thoroughly clean notes areas and clean on a more frequent schedule.

Found the floor edges and areas around the deep fryer soiled with build up and accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

(Critical) Found the drain line of the steam table draining into floor drain without air gap. Pic corrected by placing drain line on bricks lifting drain line and providing adequate air gap. Found the drain line of the walk in cooler/freezer draining into floor drain receptacle without air gap. Pic corrected by securing drain line to the side of the walk in cooler lifting drain line and providing adequate air gap. Found the drain lines of the ice machines draining into floor drain receptacle without air gap. Pic corrected by securing drain line on support pipe, which provided an adequate air gap. Pic has been instructed to ensure air gaps are maintained at all times.

Found dented cans stored on a designated shelf with no sign indicating " dented cans, return to supplier for credit. " to correct provide sign identifying dented can storage area.

Found wiping cloths stored on top of counter tops. Pic corrected by setting up a sanitizing bucket and placed wiping cloths in sanitizing buckets.

Found no handwashing sign in the ladies and mens public restrooms. To correct provide signs reminding employees to wash their hands at all locations where they may wash thier hands.

Found the floor (under equipment, edges and corners) in the kitchen soiled. Found the light shields and ceiling tile around the cooking area soiled with a dusty, greasy build up. To correct clean as soon as possible, and clean on a more frequent schedule.

Found no drain stoppers for the 3 compartment sink compartments. To correct provide drain stoppers for all compartments of the 3 compartment sinks.

Found bags of rice stored on the floor of the storage area. To correct store rice on shelf at least 6 inches off the floor.

Found the center edge of the cooking hood soiled with grease. Found the surfaces of the rice warmer lids soiled with build up. To correct clean as soon as possible, and clean on a more frequent schedule.

Found no antichoking poster on the kitchen wall. To correct provide and post antichoking poster on kitchen wall.

Observed an employee not wearing hat/hair restraint cooking food on the cooking line. To correct provide hat/hair restraint and remind employees to wear them.

Found the spray nozzles of the dishwashing machine clogged with debris and build up. To correct follow manufacturer's directions and clean dishwashing machine and spray nozzles.

(Critical) Observed a couple types of medicines stored on a shelf above a food prep area and some single use items in the kitchen. Corrected by relocating these medicines to a personal item storage area at the front of the store

(Critical) Observed the owner to be drinking in the kitchen from an uncovered styrofoam cup. Corrected by discarding the cup and by informing the owner to utilize a covered mug for drinks in the kitchen.

(Critical) Observed food in the two, two door prep coolers to be 45 to 58f. Temporarily corrected by discarding foods above 50f, relocating foods in the 40's to another cooler and by adjusting the two units colder. Monitor and adjust these units as needed to maintain foods at 41f and below. Call a refrigeration contractor to repair them if needed

Observed a bag of whole onions and a bag of rice stored on the kitchen floor. Corrected by elevating these foods off of the floor.

Observed the dish washing machine wash and rinse arms at the top of the machine to be stuck in place due to lime build-up. Corrected by removing the top and bottom wash & rinse arms to de-lime them. Ensure this unit is maintained daily so it functions properly

Observed depressions in the tiled floor area near the one compartment food prep sink due to worn away tile grout. Remove remaining grout from these deteriorated areas, thoroughly clean and dry, then re-grout the floor.

Observed the wet mop to be stored resting in the bottom of the mop sink. Please hang up the mop to facilitate quicker drying

Observed no paper towel provided at the back kitchen hand washing sink. Temporarily corrected by providing a roll of commercial paper towel. This roll of towel does not fit the residential style paper towel holder provided. Install a commercial paper towel dispenser for this hand washing station

Observed a wet wiping cloth stored on the counter by the soup steam table, and observed a bucket with wiping towels in water near the server side stand. Corrected by providing 100 ppm chlorine sanitizer in the bucket and by placing the wiping towels in the bucket
