(Critical) At the clean dish storage rack observed forty five sheet trays not clean to sight and touch. The trays are used for proofing dough balls. Oil is applied to the tray. Not all of the oil is being removed during the dishwashing process. As a result the liquid oil and other food debris has created a film on each of the pans. The pans are stacked tightly transferring the oil from one pan to the next. On some of the pans the deposited film is two to three millimeters thick. The film is pliable and easily sloughs from the pan. Some of the oil appears to be deteriorating and rotting. A follow up inspection will be completed to ensure all of the pans have either been properly cleaned or replaced. Pic temporarily corrected by properly cleaning a selection of pans for immediate use.

(Critical) Observed the following tcs items without a discard date: 1. In the walk in cooler on the salad prep cart, nine house packaged salads 2. At the front counter area salad display cooler, two house packaged salads pic corrected during the inspection by properly datemarking the salads. Pic states he personally made the salads yesterday afternoon. Item temporarily corrected.

(Critical) Observed an infestation of pests at two locations in the facility: 1. Above and around the pepsi merchandiser cabinet near the telephone station 2. In the rear of the store by the mop sink correct by contracting with a licensed pest control company to remediate the situation. A follow up inspection will be conducted. ,

(Critical) Observed the dough sheeting process where oil is applied by a machine to the inside of deep dish pans. The deep dish pans are stacked next to the machine on an inverted stack of pepsi two liter holders. The bottom of the deep dish pan touches the unclean bottom of the two liter holders. After oiling the pans are restacked together in preparation for dough to be placed in the pans. During the inspection the pic directed staff to relocate the stack of awaiting pans to a clean preparation table. Item corrected

At the wic observed the floors with a heavy accumulation of food debris, old tape, container lids, and wrappers. Correct by cleaning now and on a more frequent basis

Observed the wet mop stored in the mop wringer inside the mop sink. Correct by hanging the mop to dry after each use. Hanging the mop to dry helps reduce harborage areas for insects.

In the wic observed a case of cherry tomatoes stored under the condensate drain line. Condensate is observed dripping on the unopened containers. Correct by relocating the case of tomatoes. Also, the condensate drain line needs to be rewrapped with insulation

At the rack adjacent to the three compartment sink observed several columns of pizza pans and lexan storage tubs stacked wet. Correct by air drying pans and tubs prior to stacking for storage

At the handwashing sink adjacent to the three compartment sink observed the following: 1. The fresh water flow volume is low at each the hot and cold. Correct by increasing the flow volume. 2. The drain pipe has a slow leak. During the inspection a trash can intended to contain the leaking water was observed under the pipe. Correct by repairing the pipe so it does not leak

At the pizza oven observed the hood filters with a heavy accumulation of dust and grease. Correct by cleaning no and on a more frequent basis to prevent the accumulation of dust and grease

(Critical) Observed several stacked topping insert pans/bowls with accumulated food debris caked on the surfaces located at the utensil storage shelf. Found the following food contact surfaces not clean to sight or touch: the interior of the berkel vertical cutter dough mixer. The blade of the can open soiled. Found the interior surfaces of the ice machine with black mold in the ice bin. To correct clean these food contact surfaces both now and in the future, by properly washing, rinsing, and sanitizing these items

(Critical) Found several mouse droppings located along floor wall surfaces of the dough mixer area of the kitchen. The outer doors were all found to be propped open. To correct seal all outer openings, provide self-closing outer doors and do not prop open outer doors, seal all outer openings pipe chases along exterior of the building. Contact pest control operator to service this facility for rodents.

Sanitizing agent is requied for use at the three compartment sink for proper washing, rinsing and sanitizing operations required for a licensed food service establishment. To correct obtain a sanitizing agent and appropriate chemical test kit to monitor the sanitizing solution concentration at the three compartment sink, and for wiping cloth sanitizing solutions

Found pizza pans being stored directly on the floor surfaces located beside the pizza oven. To correct store utensils, pizza pans above the floor on a clean and sanitized surface as required

Found the three compartment sink compartments with accumulatoin of debris along side walls of the sink compartments and back splash, in addition the overhead spray nozzle was found to be caked with food debris

(Critical) The following non food contact surfaces need cleaning. The prep unit, uprigh freezer interiors, shelves in the walk in cooler, outside of the ice machine and interior of the unit which is not in use. Clean now and place on a more frequent cleaning schedule.

The restroom door was ajar at time of inspection. Repair or adjust so that the door closes completely by itself tominimize access for insects.

The floors walls especially at floor wall junctures need cleaning. Clean now and place on amore fequent cleaning schedule.

There is not a certified food safety manager in the facility. Manager has agreed to sign up for servsafe within 14 ddays and provide confirmation of sign up. (Critical) I found a large amount of dried on food debris in the bottom of a food storage container, located on the clean items storage rack. Food storage containers must be effectively washed, rinsed and sanitizer to remove food debris. Failure to do this can result in the contamination of food stored in the container. The container has been discarded. A follow-up inspection will be made to ensure that this critical violation is corrected.

(Critical) The floor drain located under the rear ice machine has been clogged since the last inspection. This has resulted in the nearby hand sink being shut off. Additionally, the floor in the area has sunken creating a large gap in the floor. The floor drain must be cleared. The hand sink must be operational at all times. The floor repaired.

(Critical) A sanitizing agent is not available at the time of inspection. A sanitizing agent must be available at all times in the restaurant. Dishes, utensils and equipment can not be properly washed rinsed and sanitized without it. It appears that it maybe 1-2 days since chlorine has been available. Immediately obtain a chlorine based sanitizer.

(Critical) I found that a spray bottle filled with a cleaner is stored over single service items (cups). Store all cleaning products as directed above. Corrected at the time of inspection. The cleaner has been relocated.
