Found boxes of single service articles placed on dry floor of the basement. To correct should store single service articles at available shelving above the floor to protect them during storage
(Critical) Found the ice machine in the basement to have an accumulation of pink slime on the drip edge. To correct - take ice machine out of service and clean thoroughly now and keep on a more frequent service routine (inspecting and cleaning). I will return for a follow-up inspection within 10 days ( nov. 5th, 2013) to check the ice machine for cleanliness
(Critical) Found a container of house-made ranch dressing with a prep date of 9/21/13, over 1 month old. Corrected - ranch dressing was discarded. Staff was educated that house-made, phf items must indeed be datemarked, but they must be discarded after 7 days. Datemarking poster provided to staff
Found a rack of silverware - forks - to be stored such that their tines are upright. To correct - although washing forks in this position allows for more thorough cleaning, once cleaned and air dried, the forks should be removed with gloved hands and stored so that the tines are protected from handling
Found the sides of the under counter freezer on the line to have an excessive accumulation of greasy food debris on them. Also found the hood filter above the fryer to have an accumulation of greasy dusty debris clinging from the filter. Found crumbs and debris in the bottoms of the hotel pans that are used to store clean utensils on the line to the right of the char-broiler. To correct - clean these areas now and keep on a more frequent cleaning schedule
(Critical) Found open bags of chopped lettuce and shredded cabbage in the wic. Found sliced tomatoes and cleaned trimmed romaine in the prep unit. These items are now considered phf and therefore subject to datemarking. Corrected - pic dated all items. Kitchen just opened this past monday; took delivery of these items that day

However, this is the second time in the last two inspections that this violation has been cited. Because of this, a risk control plan must be completed and faxed to this office by may 6, 2013. Further incidence of this violation could result in fines and further enforcement

(Critical) Found several ready-to-eat foods in walk in cooler beyond their discard date; those including milk dated 10/5/11, sw sauce for wraps dated 8/25/11, house made ranch dressing dated 10/9/11, marinade dated 9/17/11, sliced tomatoes dated 10/4/11. Also found a to go cup of milk in bar reach in cooler with no date. To correct - discard all ready to eat food items that have expired dates. Corrected by removing the cups that were taped to the drain pipes. 2) the two copper drain lines for the basement ice machine (tan unit). 3) the drain line for the water softener discharge, and the two brine tank overflow drains
(Critical) Observed five liquor bottles (previously opened with pour spouts connected) at the bar to have small flies in them. Corrected by discarding. Ensure your open liquor bottles are protected from contamination
(Critical) Observed: 1) a very weak chlorine residual provided in the rinse water of the kitchen dish washing machine. Corrected by providing a fresh container of chlorine sanitizer and priming the system to provide 150 ppm chlorine. 2) observed two spray bottles (one in the kitchen and one at the bar) to be mixed way too strong (>>200 ppm). Corrected by diluting to 150-200 ppm
(Critical) 1) observed living flies & fly larvae in the floor sink drain located between the beer cooler and the vitamin water cooler. Thoroughly clean this drain to remove debris build-up and flies and perform frequent inspections of the premises for the presence of pests. Obtain some drain treatment gel for the elimination of flies from your food service supplier or your pest control contractor. 2) observed evidence of mice (mouse feces) on multiple basement dry storage shelves. Remove any foods and equipment, etc. In storage from the shelves so the shelves and equipment can be cleaned.
Observed a leak in one of the bar area four compartment sink drain line connections. Repair as needed to eliminate the leak

The overhead garage door for the basement shop/golf cart storage area is left open during working hours. Some gaps were observed in the wall that is common to this shop and the basement food/equipment storage room. Properly seal these gaps in the wall/ceiling to prevent pest entry to this food storage area
Ensure staff are trained on how to use a chlorine test kit, and that they are using one to verify each batch of sanitizer prepared and the sanitizer residual in the dish washing machine rinse water daily
Observed shut-off valves located down-stream from the atmospheric vacuum breaker on the mop sink faucet. Obtain and install an eco-lab "side kick" flow through back flow preventer (or equivalent device) between the mop sink faucet and the "y" splitter.
Observed several cracked 12" x 12" floor tiles at the bar area. Replace these damaged tiles (an repair the sub-floor as needed) to provide a smooth and cleanable surface that is impervious to water.
Observed the following areas to be in need of cleaning at this time and more frequently in the future: the floor under the mat at the kitchen dish washing station, the floor drain and floor drain grate under the mat at the kitchen dish washing station, and the floor sinks at the kitchen 3 comp. The grill brush utensil used to scrape grill is in poor repair and no longer smooth and cleanable. Replace with a new brush/scraper.
Coffee urns at the bar are stored on a cloth towel. Cloth towels are prohibited as they are absorbent. Replace with rubber mesh matting that is smooth and cleanable or equivalent.
Gaskets on the doors of the coolers are in need of cleaning. Food debris build up found on several door gaskets. Clean now and place on a more frequent cleaning schedule.
There is a new domestic faucet was installed on the 3c sink and is not approved. Remove and install a commercial faucet for the 3c sink. Correct asap
The ambient air temperature in the walk in cooler was found at 47 degrees f. Assorted ph foods temped at 45 - 46 degrees f. All ph foods discarded/removed at time of inspection. The unit is ordered out of service for the storage of ph foods until repaired/adjusted and capable of holding ph foods at 41 degrees f or below. Call for repair/adjustment of unit. Correct asap.
There are two microwaves in the kitchen and at the bar that are not nsf certified equipment. Replace domestic units with approved commercial equipment when in need of repair.
Thermoemter missing on the monster self serve reach in cooler which also stores condiments. Provide a thermometer for this unit in order to properly monitor the temperatures for this unit.
