(Critical) Observed the following: 1. Cut tomatoes and cooked egg that were just prepped at 52f, cooling in the top-loading reach-in cooler on the back cook line. 2. Several sealed containers of cooked pasta that were cooked two hours prior at 52-55f and still in the cooling process, located in the walk-in cooler, in a deep, tightly sealed in a container. Cooling shall be accomplished in accordance with the time and temperature by using one or more of the following methods based on the type of food being cooled: (1) placing the food in shallow pans; pf (2) separating the food into smaller or thinner portions; pf (3) using rapid cooling equipment ; pf (4) stirring the food in a container placed in an ice water bath; pf (5) using containers that facilitate heat transfer; pf (6) adding ice as an ingredient; pf or (7) other effective methods. Pf to correct: 1. Move the noted food to the walk-in cooler to rapidly cool them to 41f or below and discontinue cooling food in the top-loading section of the reach-in cooler and start cooling the noted food in the reach-in cooler. 2. Re-do ice bath to rapidly cool the food to 41f or below and start cooling the noted food with one of the above, approved methods and continue the process until the food reaches 41f in the cooling time frames. Corrected: 1. Person in charge moved the noted food to the walk-in cooler to rapidly cool it down to 41f or below and stated that they would discontinue cooling food in the top-loading section of the reach-in cooler. 2. Person in charge had the noted food put on ice and stated that the food would be cooled with an ice bath and would ensure the food hit 41f before placing it into deep sealed containers.

Observed flour being stored in a color rubber maid container on the cook line in the back in dry storage. Materials that are used in the construction of utensils and food-contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: (p) (a) safe; (p) (b) durable, corrosion resistant, and nonabsorbent; (c) sufficient in weight and thickness to withstand repeated ware washing; (d) finished to have a smooth, easily cleanable surface; and (e) resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. Observed the flooring around the floor wall juncture throughout the kitchen in need of detail cleaning. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct have all the noted areas cleaned.

Observed the following: 1. Floor in front of the beer walk-in cooler to be sagging and in very poor repair, located at the bar. 2. Observed many missing cove base tiles throughout the kitchen. Physical facilities shall be maintained in good repair. To correct: have the noted areas repaired.

(Critical) Observed two trays of cooked rice that were cooked the night before at 55-58f and still in the cooling process, located in the walk-in cooler, in large portions. Cooked potentially hazardous food shall be cooled: (1) within 2 hours from 135f to 70f; p and (2) within a total of 6 hours from 135f to 41f or less. P discard the noted food and discontinue cooling food in large portions. Corrected: person in charge discarded the noted food and stated that they would cool the food in smaller portions

Move the noted food to the walk-in cooler to rapidly cool them to 41f or below and discontinue cooling food in the top-loading section of the buffet line cooler and start cooling the noted food in the walk-in cooler before putting them on the buffet line. Corrected: 1. Person in charge discarded the noted food and stated that the food would be cooled in smaller portions and would leave it loosely covered to allow the cold air to penetrate the noted food. 2. Person in charge moved the noted food to the walk-in cooler to rapidly cool it down to 41f or below and stated that they would discontinue cooling food in the top-loading coolers on the buffet line.

(Critical) Observed raw eggs stored with and above ready to eat foods in the grill area cooler with the two doors/drawers. Food shall be protected from cross contamination by: (1) except as specified in (1)(c) below, separating raw animal foods during storage, preparation, holding, and display from: (a) raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables,(p) and (b) cooked ready-to-eat food; (p) to correct move the noted raw food down and away from all ready to eat foods and store by final cook off temperatures. (Critical) Observed multiple foods in the buffet coolers 44-47f for less than an hour. Note: person in charge stated that noted coolers were turned on late. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct rapidly cool to 41f or below and ensure coolers are turned on before putting food into them. ,

(Critical) Observed no quat test strips available in the facility. A test kit or other device that accurately measures the concentration in mg/l of sanitizing solutions shall be provided. (pf) to correct provide the noted test strips for use in the facility. ,

Observed the follow not sealed to the wall: 1. Three compartment prep sink. 2. Entire dishwashing line. 3. 1 compartment meat prep sink and table in the grill area. 4. Hand sink in the lobby grilling area. 5. Hand sink and wash sink at the bar. Equipment that is fixed because it is not easily movable shall be installed so that it is: (1) spaced to allow access for cleaning along the sides, behind, and above the equipment; (2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch; or (3) sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage. To correct seal the noted items to the walls. ,

Observed the underside of the shelf over the rice warmers/cookers to have a heavy soil build up. Non food-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. To correct clean the noted items. ,

Found the floor by the main grill in the dining area worn with open seams. To correct repair floor to be smooth and easy to clean.

Observed employee cutting lemons wearing a single use glove under a slash resistant glove, without a single use glove covering slash resistant glove. Found a case of wrapped flat bread stored on the floor of the bar storage area. To correct store food on shelf at least 6 inches off the floor.

Found food (crushed peanuts, croutons, fried noodles, and sunflower seeds) in the self service area without protection or under sneeze guards. To correct store food items in a more protected manner, such as under sneeze guards.

Found the floor (under equipment, edges and corners) in the bar, kitchen, in the main level storage areas, and in the basement storage areas soiled with accumulation, and build up. To correct thoroughly clean floors and clean on a more frequent schedule. Found the floor under the main grill soiled with heavy accumulation of debris. To correct clean floor as soon as possible, and monitor that employees are properly cleaning floor around grill. Found the shared hallway leading to the back door soiled with accumulation of debris. To correct work with neighbors and keep area clean.

Found a container of recently made eggs in a plastic container cooling in back kitchen prep top cooler. Pic corrected by placing eggs in walk in freezer and then in walk in cooler to rapidly cool. Pic has been reminded to cool foods from 135°F to 70 in 2 hours, and then from 70°F to 41°F in 4 hours.

Observed flies in the bar and kitchen areas. To correct keep areas in good repair, clean and dry, and continue fly control measures.

Found plastic squeeze bottles of seasoning in the main grill area without labels. To correct provide labels.

Found several nozzles of the dishwashing machine clogged with debris. To correct clean nozzles and check and clean on a more frequent schedule

Found the hood in the back kitchen cooking hood not covering or above the range top burners. To correct move and replace equipment so all equipment is under cooking hood.

Found low water pressure (hot and cold) in the handsink in the bar. To correct repair/adjust water pressure in the handsink to be sufficient to wash hands.

Found water on basement floor and hose bibb leaking water in the basement storage area. To correct repair hose bibb.

Found the handles of the rice cooler lids missing. To correct repair/replace missing handles.

Found wiping cloths not completely submerged under sanitizer in sanitizing buckets in the entire facility. To correct monitor and re-train staff on proper storage of wiping cloths, and verify that wiping cloths are stored completely submerged under sanitizer in sanitizing buckets.

Found the employee restroom door propped open with door closer not connected to door frame. To correct re-attach door closer to door frame, and keep door closed.
