(Critical) Observed the first aide kit stored above food and food products on the cook line prep area. First aid supplies that are in a food establishment for the employees' use shall be: (a) labeled as specified under # 7-101. 11; (pf) and (b) stored in a kit or a container that is located to prevent the contamination of food, equipment, utensils, and linens, and single-service and single-use articles. (p) to correct relocate the noted kit down and away from all food, food products and clean linen. Corrected: person in charge moved the noted item above the soiled side of the dish line

(Critical) Observed dish washing without dish detergent. When used for ware washing, the wash compartment of a sink, mechanical ware washer, or wash receptacle of alternative manual warewashing equipment as specified in # 4-301. 12(c), shall contain a wash solution of soap, detergent, acid cleaner, alkaline cleaner, degreaser, abrasive cleaner, or other cleaning agent according to the cleaning agent manufacturer's label instructions. (pf) to correct provide dish detergent to the noted machine and check to ensure it is working. , observed dish washing without dish detergent. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct: discard the noted food into the garbage. Corrected: person in charge discarded the noted food into the garbage

Observed a pipe leaking in the basement on the right side of the walk-in cooler. A plumbing system shall be: (a) repaired according to law;(p) and (b) maintained in good repair. To correct: repair the noted pipe to eliminate the leak. ,

Observed the cooler holding food between 45-48f located in the far left cooler in the outside cooking area. Equipment shall be maintained in a state of repair and good condition. To correct: repair the noted cooler to hold cold food at 41f or below. ,

Observed the ice machine at the bar with a mold like build up on the interior part of the machine. In equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment: (a) at a frequency specified by the manufacturer, or (b) absent manufacturer specifications, at a frequency necessary to preclude accumulation of soil or mold. Correct by emptying the noted machine and then wash, rinse and sanitize the interior.

Observed several light shields in the facility with a build up of dead insects. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. Correct by cleaning the light shields to remove the dead insects.

Observed the following: 1. Multiple torn gaskets throughout the facility and outside grill area. 2. Two door glass cooler at the bar is pooling water. Equipment shall be maintained in a state of repair and good condition. Correct the following by: 1. Repairing or replacing all the torn gaskets throughout the facility. 2. Repairing the noted cooler to eliminate the pooling of water.

(Critical) Found dish machine not sanitizing adequately. To correct - contact a service technician for repair. In the interim, the sanitizing must be done in the 3 compartment sink, as required. A follow-up inspection will be performed later this week to check on progress

Found the consumer advisory to be the wrong wording in conjunction with the use of the asterisks*. To correct - at next printing, provide menus, grill and event, with the wording as outlined in the michigan food law. With the following wording, the asterisk is not required: " ask your server about menu items that are cooked to order or served raw. Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness".

Found several openings in the walls in the basement where pests can enter. To correct - fill openings around pipes that are exiting the basement walls. Also, the outdoor grill area floor and walls should be painted or finished to add to ease of cleaning in this area. This should be done prior to season opening in 2013

Found water softener discharge to not be air gapped at the sump pump. It appears that the air gap was initially there, but has slipped down through the zip ties. To correct - re-establish the air gap by securely bracketting the drain line above the sump

Found the 3 compartment sink to be set up with paper towels and soap for hand washing. It is noted that staff was using the hand sink at the bar for hand washing. To correct - a proper, separate hand wash sink must be provided in the dish/prep room. A recommendation would be to move the dish machine to the left of the 3 compartment sink as this is the more a appropriate location for this equipment to avoid bringing dirty dishes across the sanitizing side of the 3 compartment sink. With the dish machine on the left, a new hand wash sink can easily be installed and plumbed to the right of the 3 compartment sink

Found the outer door to the grill area to be missing a self-closing device and is also in need of a new door sweep as daylight can be seen under the door providing an entry point for pests. To correct - provide required self-closer and door sweep on this door

Found single service items stored at server station outside of the protective plastic wrapper. To correct - always store single service items covered to prevent contamination from splash and dust

(Critical) Followed the drain lines of the ice machine which drain to the basement condensation pump, which then drains directly into a waste drain line. To correct provide an air gap of at least 1 inch from the condensation pump drain line and the rim of a floor drain. ,

(Critical) Found ready to eat foods, a covered container of guacamole and chili stored next to cases of raw beef patties. Pic corrected by moving ready to eat foods to a shelf above raw meats. Pic has been instructed to monitor that employees are properly storing ready to eat foods above and away from raw foods. ,

(Critical) Found various interior surfaces of the ice forming parts of the ice machine soiled with build up and accumulation. To correct follow manufacturers directions and clean all surfaces of the ice machine. ,

Observed the shelves in the basement appear to be made of particle board. To correct repair/replace shelves to be durable, smooth and easy to clean.

Found no internal hanging thermometers in the 1 door worktop cooler and freezer. To correct provide internal hanging thermometer in cooler and freezer.

Found the ceiling area on the side of the walk in cooler, walk way to the back basement area dripping water. To correct locate leak source, repair leak, and repair/replace surrounding area if needed

Found the floor of the dumpster soiled with accumulation of debris. To correct clean dumpster area floor, and keep clean.

Found the light in the kitchen above the dishwashing area missing light shield. To correct provide light shield.

Found several drawer knobs on the storage cabinets missing. To correct repair/replace drawer knobs.

Found the spray nozzles of the dishwashing machine clogged with debris. To correct clean spray nozzles, and clean on a more frequent schedule.

Found the interior of microwave soiled with splash and accumulation. To correct follow manufacturer's directions and clean microwave at least daily.

Found no "permanent type" handsink in the outdoor cooking/prep area, but found a "temporary" type handwashing station in the outdoor cooking/prep area. To correct provide a "permanent type" handsink in the outdoor cooking/prep area. Consult with our office prior to installing "permananent type" handsink in the outdoor cooking/prep area.

Found the metal shaker of seasoning on the outside cooking/prep area without label. To correct provide label.

Found the floor of the basement storage areas soiled with debris, and pooling water. To correct clean floor, and clean floor as needed.

Found no antichoking poster on kitchen wall. To correct provide and post antichoking poster on kitchen wall.

Found dumpster stored on top of dirt, loose surface. To correct provide a storage surface for dumpster that is smooth, durable, and sloped to drain.

Found hot dogs in original plastic wrapping being heated in the water of the table top hot holding unit. To correct either discontinue this practice of heating hot dogs in it's original packaging, or provide documentation that this plastic wrapping is safe to heat food in.

Found absorbent type liners lining the bottoms of drawers in the kitchen. To correct remove all absorbent type lining, and replace with nonabsorbent type liners. Found particle board shelving in the basement storage area. To correct replace particle board with shelving that is durable, smooth, nonabsorbent, and easy to clean.

Found a caddy of condiments stored on a shelf which was sitting on top of the floor. To correct raise shelf so it is at least 6 inches off the floor.

Found the light shield above the dishwashing area missing. To correct repair/replace missing light shield.

Found dumpster stored on dirt and rocks. To correct provide a dumpster storage surface that is constructed of nonabsorbent material such as concrete or asphalt and shall be smooth, durable, and sloped to drain.

Found a stack of unwrapped coffee filters on shelf in the kitchen. Found numerous beverage coolers stored on the floor of the back kitchen. Found single use gloves stored under and around the soap dispenser in the bar handsink area. To correct store utensils in a more protected manner, and at least 6 inches off the floor.

Found a hole in the drywall of the kitchen storage area, behind shelves. Found several gaps on the wall around the pipes exiting the walls of building. To correct properly repair/fill gaps and holes.

(Critical) Found ready to eat, time/temperature control for safety food (tcs), a container of turkey and diced cooked chicken with 8 day discard datemarks. Pic corrected by providing 7 day datemarks (day opened/prepared + 6 days). Pic has been instructed to monitor the datemarking of appropriate items. A risk control plan for datemarking has been issued. , found ready to eat, time/temperature control for safety food (tcs), a container of turkey and diced cooked chicken with 8 day discard datemarks. Pic corrected by providing 7 day datemarks (day opened/prepared + 6 days). Pic has been instructed to monitor the datemarking of appropriate items. A risk control plan for datemarking has been issued. On follow up inspection found tcs, a container of turkey with a discard mark of over 7 days. Pic corrected by having employee change datemark to proper datemark. Risk control plan has been been completed, and is being revised to work for operation.

Found employees in kitchen using a probe dial faced thermometer scaled to large (0-550°F, and scaled in 5 degree increments). To correct provide a thermometer which is appropriate for situation such as one that is scaled from 0 to 220°F , scaled in 2 degree increments.

Found the floor in the basement (under shelves, edges, and corners) soiled with accumulation of debris.

Found an absorbent type liner lining utensil storage drawers in the kitchen. To correct remove absorbent type liner and replace with non-abosorbent type liners.

Found dumpster stored on a dirt and rocks. To correct store dumpster on a surface that is nonabsorbent such as concrete or asphalt and shall be smooth, durable, and sloped to drain.

Found wet wiping cloths stored in sanitizing buckets that are to small to hold wiping cloths completely submerged under sanitizer.

Found no handwashing sign in the 3compartment sink which is also used as a hansink. To correct provide sign reminding employees to wash their hands.
