(Critical) Observed 2 pans of rte hommous and a pan of sliced tomatoes in top rail of a non-functioning prep cooler in front prep area with internal food temperatures of 50-55f. Pic stated that above mentioned food items were removed from wic 30 minutes ago and are normally put on ice for lunch, but they haven't prepared bus tub of ice yet. All phf must be cold held at 41f or below at all times. Pic corrected by placing hommous and tomatoes in bus tub filled with ice at time of inspection. Violation corrected. , observed pans of cooked rte rice and cooked rte chicken in cambro warmers in front prep area with internal food temperatures of 50-65f. Pic stated that above mentioned food items were removed from wic 30 minutes ago and are the backup pans for lunch. All phf must be cold held at 41f or below at all times. (Critical) Observed interior food contact surfaces of the juicer, the blender, the lemon press and the juicer mixing cups in front prep juice station area soiled with dried on food debris (equipment was not washed, rinsed and sanitized after use yesterday). All food contact surfaces must be washed, rinsed and sanitized after every use and must be clean to sight and touch. Pic stated correct cleaning frequency of equipment, but it appears that staff did not follow this procedure last night. (Critical) At the front juice bar/service counter observed several small flying insects that appear to be drain flies. Correct by having a licensed pest control company eliminate the pests from the establishment using an approved method. Also, eliminate harborage conditions at the service area by: 1. Removing the case of lemons containing decaying fruit 2. Removing the used rag storage bag 3. Thoroughly cleaning the area to remove juicing residue 4. Mechanically cleaning the drains the pic indicates he has contacted a pest control company. The company will be out within a week to complete a thorough pesticide application. ,

Observed a sheet tray of frozen skewered chicken thawing at room temperature. All parts of the chicken were still frozen. The pic placed the tray into refrigeration. Item corrected

In the kitchen observed several refrigerator units with cracked or broken door seals including: 1. The salad refrigerator 2. The continental unit 3. The true unit 4. The unit with the quiznos sticker correct by inspecting each unit in the kitchen for broken door seals and repairing. The pic indicates he has already contacted his refrigeration maintenance company to install new door seals

(Critical) Observed potentially hazardous food (phf) stored above 41 degrees f. Lentil soup was observed at 55 degrees f. , house made dressing at 58 degrees f. , marinated cooked chicken at 52 degrees f. These items must be stored at or below 41 degrees f. Pic discovered the electrical circuit for the reach in cooler had tripped off. Item corrected during the inspection by discarding potentially hazardous food and replacing with new proper temperature food. The circuit serving the cooler was reset. The cooler operated properly and had a measured air temperature at the end of the inspection of 40 degrees f. Pic noted two refrigerators had been plugged into the same circuit. Each refrigerator was plugged into a separate circuit. Also, pic contacted an electrician during the inspection to confirm the circuit was okay.

Observed ice scoop at front counter service area stored inside the ice bin. To help prevent contamination of the ice the scoop shall be stored on a clean protected surface outside of the ice bin.
