(Critical) At the three compartment sink observed several large pots being washed by hand. The pot was washed and rinsed but not sanitized before placing on a storage shelf for use. Health Report - 11/06/2013 (1 critical incident(s))

(Critical) At the in use mechanical dishwashing machine observed the residual chlorine concentration of the final rinse at 0 ppm. The sanitizer reservoir is full. However, the tubing is improperly routed. The tubes for the detergent and sanitizer are connected to the wrong reservoirs (five gallon container). The pic indicates the reservoirs are moved each evening during routine cleaning and in doing so were accidentally switched. During the inspection the pic routed the lines to the correct reservoir. After priming and cycling the machine functioned properly. The residual chlorine concentration of the final rinse was measured between 50 and 100 ppm. Item corrected.

Outside at the rear of the facility near the oil recycle bin observed heavy oil and grease staining on the pavement. Correct by cleaning the area around the storage bin now and on a more frequent basis too prevent the accumulation of waste grease on the pavement. Also, keep the lid of the waste recycle bin closed.

Above the main grill area observed the ventilation filters with a heavy accumulation of grease. Correct by cleaning now and on a more frequent basis to prevent a heavy accumulation of grease

Observed cloths used for wiping counters stored in a bucket containing chlorine sanitizer solution with a concentration of 0 ppm. Corrected during inspection by having the person in charge make a new solution containing the proper sanitizer concentration.

Observed no paper towel installed in the paper towel dispenser in the employee restroom. Correct by providing papertowel at all times. An adequate supply of paper towel encourages proper handwashing

Observed can opener blade to be dull and scored. Correct by replacing blade

Observed the cover of the atmospheric vacuum breaker on the mop sink faucet to be missing. Vacuum breaker appears to be functioning properly and is not leaking water. Correct by repairing or replacing vacuum breaker

Observed no anti choking poster displayed in the establishment. Correct by obtaining an approved poster and displaying

1. At walk in freezer observed the light switch is broken. Correct by replacing light switch. 2. At the walk in cooler observed a 60 watt light bulb installed. Correct by investigating if lamp base can accommodate a higher wattage bulb. Increased luminosity would be beneficial for viewing items in the walk in cooler.

(Critical) Found working liquor bottle to have fruit flies inside. Corrected - by pic discarding liquor bottle. Use storage caps over open liquor bottle to protect spout from being an entry point for fruit flies

(Critical) Found what appeared to be a bucket of sanitizer on a lower shelf under the counter in the front prep area. Found the concentration of the solution to be extremely high, above 200 ppm. When showing the pic the test strip, he showed me what he was using in the bucket. The product was spic and span floor cleaner. Told the pic to discontinue use of this product for sanitizer bucket and to use approved sanitizer for food contact surfaces, corrected by pic discarding contents of bucket

(Critical) Found cooked chicken, beef, beans, shrimp, cheese dip, baked potatoes all holding at 44 dregrees f. To correct - pic called repair person; someone will be there today to check out the wic and repair. If repairman does not show up by 5 pm, all cooked foods must be discarded. Came back to establishment to find repairman working. Repairman stated that he adjusted the thermostat to 38 degrees and adjusted the timer of the unit. Possible short-term ower outage may have caused unit falfunction. Thermometer on outside of unit now reading 40 degrees f. Temperatures of cooked food inside unit now reading 41 -42 degrees f. Considered item corrected.

Found shelf within the ice machine to have an accumulation of slime at the edge. To correct - clean this postion of the ice machine on a increased frequency to prevent the build-up of slime

Found unlabeled squeeze bottle (pic identified as water for fajita dishes). Also found large tub of sugar syrup for sweet tea without a label. To correct - label all working containers with contents by common name. Also, in the case of these squeeze water bottles, water should be changed daily to prevent cross contamination

Found hand sink in bar area without soap for hand washing, to correct- provide soap at bar hand sink

Wic found at ambient temperature of 45 degrees f. Cooked food in wic found to be at 44 degrees f. To correct - have repair person check the unit and repair as necessary

Found rapid-cool sticks stored on the bottom shelf of the freezer where they could be exposed to potential contamination. To correct - store frozen sticks on an upper level shelf, away from potential contamination

Found clean pots and pans being towel dried and clean wet pots and pans stacked before they were dry. To correct - allow all clean pots, pans, utensils to air dry before stacking and/or storing for future use.

I noticed that some food containers are stored uncovered, inside prep units. Foods must be stored covered to protect them from contamination.

The thermometer in the unit, can not be seen. Corrected at the time of inspection. The thermometer has been re-positioned.

(Critical) Observed housemade rte guacamole, sour cream, mole sauce, housemade salsa, sliced tomatoes and shredded cheese in top and bottom of prep unit cooler in prep area with an internal food temp of 48-50f. Cooler is not functioning properly. All phf food must be cold held at 41f or below at all times. Do not use this cooler until properly repaired and inspected by the wash county health dept. I will check this item at follow up inspection. Correct within 10 days. Pic stated that all food in prep unit cooler was either prepped 1-1. 5 hours ago or transferred from wic less than an hour ago. Pic also stated that no food is left in prep unit cooler overnight. Observed prep unit cooler in prep area (top and bottom) not able to cold hold at 41f or below. Cooler thermostat is set to max cool setting and temp only reaches 48f.
