Found the wall in the north side of facility basement with peeling paint and water stained. To correct repair wall to be free of leaks and seepage. Found the wall behind the clean storage shelf with worn spots. To correct repair wall to be smooth and clean.

Found wood stirring utensil used to mix food and drinks. To correct discontinue using wood utensils and replace with commercial grade utensils.

Found the surface of the grill with worn surface spots. To correct in future replace grill surface or replace grill.

(Critical) Found time/temperature control for safety (tcs) food, 3 pans of cooked beans 48 to 50°F, made the previous day in the 3 door glass reach in cooler. Pic corrected by discarding beans, and found employees were in a hurry and placed beans in an ice bath and then placed beans in cooler before leaving without adequate time to properly cool. Pic states he has told employees to either make beans earlier in the day or a smaller quantity that can be easily cooled. Pic has been instructed to monitor and ensure beans are broken down into smaller quantities in shallow pans and cooled from 135°F to 70°F in 2 hours and then from 70°F to 41°F in 4 hours.

Observed the grill surface not completely under the cooking hood. To correct move/adjust cooking equipment to fit under the hood or replace with equipment that fits under hood.

Observed a container of food stacked on another uncovered container of food on the cooking/prep area shelf. Pic corrected by placing container of food on shelf, and instructed employees not to stack containers on top of uncovered food containers.

Found no thermometer in the 1 door domestic cooler in the front counter area. To correct provide internal hanging thermometer in all coolers.

Found a floor tile in front of the pop storage area missing. To correct repair/replace tiled floor.

Found the 1 door reach in freezer in the kitchen, and the ice machine in the basement not working. To correct repair freezer and ice machine to work properly.

Observed water pooling around the faucet base when water in employee handsink is turned on. To correct repair/tighten faucet to eliminate leaks.

Found a spray bottle of water hanging on the edge of the cooking grill, and metal shaker containers of seasoning without labels. To correct provide labels.

Found an absorbent type liner used as a lining under cutting boards and used to line storage room shelves. To correct remove absorbent type liners and replace with nonabsorbent type liners.

Found the hanging thermometer in the 1 door prep top cooler without liquid to indicate temperature. To correct replace hanging thermometer.

Health Report - 01/05/2011 (1 critical incident(s))

(Critical) The large plastic storage tub for cooked tortillas (two issues): 1) a large crack is present in the bottom, and 2) this tub is screwed/bolted to a home-made wooden cart with the bolt heads sticking up in the bottom of the tub. Purchase a new food grade tub. The new tub may not be attached to the cart (must be removable for cleaning in the three compartment sink. This cart could be modified to create a platform with low sidewalls that will hold the tub in place (if made of wood all surfaces must be sealed to prevent moisture problems and allow for easy cleaning).

Observed a couple of the water outlets for the dish washing machine spray arms to be plugged. Frequently remove the spray arms and then the caps at the ends of each arm so they can be flushed of any debris

Observed a couple wet wiping cloths stored on the counter in the kitchen. Corrected by placing the wiping towels in the sanitizer pail

Observed the air thermometer in the upper right corner of the one door reach-in prep cooler to be damaged. Provide a replacement air thermometer for this unit
