(Critical) Observed a customer return food order (wrong order) to employee at front counter who then gave returned food to another customer. To correct re-train staff that food once served and in the possession of a consumer, and returned may not be offered as food for human consumption. Pic corrected by re-educating employees on how to handle returned food, that is food is to be discarded and not given to another customer.
Found the ice bin which is used to store onions soiled with onion skin and debris. Found the hood filters on the cooking hood soiled with greasy build up. Found the shelf and panels surrounding the battered vegetables soiled with splash and build up. To correct clean as soon as possible, and clean on a more frequent schedule.
Found the wall under the shelf by the 3 compartment sink soiled with accumulation. Found the floor under equipment and edges soiled with accumulation and build up. To correct clean wall and floor as soon as possible and clean on a more frequent schedule.
Observed the presence of flies in the basement prep area. To correct rinse returnable cans, keep areas clean and dry, and continue fly control measures.
Found the wall paneling around the basement 3 compartment sink worn and exposed surfaces. To correct finish wall paneling to be smooth, and nonabsorbent.
Found the lights in the basement prep area missing plastic tubes and endcaps. To correct provide plastic tubes and end caps for lights
Found the 1 door worktop cooler not used for cold holding. To correct repair/replace cooler to hold tcs food at 41°F if needed. Found ice build up in the basement 1 door reach in freezer. To correct defrost freezer.
Observed dead cockroaches on the floor of the corner behind the reach in freezer. To correct remove dead cockroaches, keep areas clean and dry, in good repair, and continue pest control treatments.
Found the ice machine in the front counter area not working. Found the paper towel dispenser in the front counter handsink not dispensing paper towels. To correct repair ice machine and paper towel dispenser.
Found the handsink on the end of the cooking line area with items stored in sink and items in front of the handsink. To correct remove items from handsink and keep handsink clear at all times.
Found no manager certification available. To correct provide manager certification. An administrative conference notice has been issued.
Found numerous plastic squeeze bottle of oils, water, and sauce without labels. To correct provide labels.
(Critical) Found time/temperature control for safety (tcs) food, milk 46°F, and soups 46 to 48°F in the 1 door worktop cooler in the kitchen. Pic corrected by discarding milk and soup and noted cooler was not set at proper setting. Pic has been instructed to monitor food temperatures in this cooler, and not to use this cooler to hold tcs food, until adjusted or repaired to hold tcs food at 41°F. On follow up inspection found tcs food, milk 41°F and sauce 41°F, in the 1 door work top cooler. Pic states cooler has been set at correct setting, and is holding tcs food at 41°F. Continue to check cold holding temperatures. Violation corrected.

Observed the presence of roaches in the glue boards in the basement storage areas. To correct work with neighbors to keep areas clean, dry, in good repair, and continue working with pest control company.

Found a roll of paper towel stored on top of paper towel dispenser by the cook line. To correct repair/replace paper towel dispenser to hold/dispense paper towels. Found the handle of the rice warmer broken. To correct repair/replace handle.
Found the sanitizing compartment of the 3 compartment sink soiled with accumulation. To correct clean 3 compartment sink and change sanitizing solution on a more frequent schedule.
Facility no longer has a certified manager on staff. Pic states he has taken manager certification exam is waiting for results. To correct provide manager certification.
(Critical) Observed an employee working in the basement prep area with an open cup on prep table. Pic corrected by discarding cup, and has been reminded to use a cup with lid and straw in food prep areas
(Critical) Found a small case of tofu passed it's sell by date in the 2 door reach in cooler. Pic corrected by discarding tofu passed it's sell by date, and has been instructed to monitor that employees are rotating stock, and checking dates when stocking and before using.
Found the hood filters soiled with greasy build up. To correct clean as soon as possible, and clean filters on a schedule to preclude heavy build up and accumulation.
Found the door of the ice machine broken. Found excess ice build up in the basement 1 door reach in freezer and the front counter under the counter cooler. To correct repair/replace ice machine door, and defrost and maintain cooler and freezer.
Found no handwashing sign in the basement hand sink area. To correct provide sign reminding employees to wash their hands at all locations where they may wash hands.
Found the ceiling light above the food prep area in the basement with out light shields and end caps. To correct provide light sheilding and end caps.
Found the faucet in the basement prep area handsink leaking. Found the drain line of the handsink in the main kitchen broken with a bucket catching waste water. To correct repair/replace faucet and drain line.
Found no thermometer in the 1 door glass reach in cooler. To correct provide thermometers in all coolers.
