1) the pre-filter for the ice making machine has a significant amount of dust accumulation on it. Clean as needed to provide proper air flow to the cooling system. 2) remove the deteriorating silicone at the end of the clean dish drain board and reapply silicone to provide a proper seal. 3) the back and right side of the hand washing sink splash guard is also in need of replacement silicone at the counter top surface

Some of the interior surfaces of the ice making machine are due for cleaning to remove the slime build-up. Empty the ice machine, disassemble parts as needed, thoroughly wash rinse and sanitize the ice machine per manufacturer's directions, reassemble and start up ice machine. Discard the first couple batches of ice

(Critical) The drain line for the middle dipping well no longer has an air gap at the termination above the floor sink. Remove the drain pipe extension and/or re-plumb the drain line to ensure the required air gap is maintained

Ice cream dipping cabinet across from the waffle cone maker: the light bulb tube is still in two pieces and one end cap is missing. Provide a replacement bulb tube and end cap to prevent glass shards from contaminating the food and the unit should a bulb break

Observed several small flies on the surfaces of the floor sink located in the vicinity of the cash register. Ensure staff are regularly inspecting the facility for the presence of pests so appropriate measures can be taken to eliminate breeding areas

Observed the following areas to be unclean: the floor under the ice making machine (pull out this unit so a thorough cleaning can be performed. The floor sinks (2 of them) in the front kitchen area. Thoroughly clean these areas now and 2-3 times per week in the future

Observed the pre-filter and the condenser coil for the ice making machine to be in need of cleaning to remove excessive dust. The handles and surrounding area of the interior doors for the two door master-bilt freezer in the back room are in need of cleaning at this time to remove food debris

Observed water all over the floor under the ice making machine and the three compartment sink. Initial observations indicate the water is coming from the dipper well drain line serving the front dipping cabinet (closest to the front door). I recall this drain line is air gapped behind the ice machine. Consider eliminating the air gap behind the ice machine and provide an air gap directly under the dipping well prior to the p-trap. Ensure the ice making machine drain line(s) are properly secured and terminate with an air gap

(Critical) Observed the waffle batter to be stored out on the counter. The directions on the dry waffle batter package indicate to keep refrigerated once prepared. Corrected by covering and placing in the refrigerator

Observed the following areas to be unclean: the side-walls of the floor sink under the dish washing basins, and the depression in front of the toe-kick area at the hand washing sink. Please clean these areas now and more frequently in the future to maintain them clean

The one door front counter cooler is lacking an air thermometer. Provide a new unit to monitor refrigeration temperatures

Observed a significant gap/depression located in front of the toe kick area of the hand washing sink. Re-grout / seal as needed to be smooth and easily cleanable

(Critical) Observed a spray can of raid max ant and roach spray on a shelf by the back door. This product is labeled for household use only. Corrected by discarding. In the future, if a pest problem is detected either contact a professional pest control company or only purchase and utilize and insecticide that is approved for use in a food facility

The floor sink basin located under the back counter hand washing sink has excessive black slime build-up on the side-walls. Clean this drain now and at least 2-3 times per week to eliminate this accumulation

Ice cream dipping cabinet across from the waffle cone maker: one of the two dipping well water spouts was not flowing water even though the water supply valve was opened fully. Trouble shoot the problem and repair as needed

Observed some slime / mold growth on the sealing gaskets for cover of the ice cream dipping cabinet (the older one closest to the parking lot). Have someone thoroughly clean the affected surfaces (in a manner that protects the food) at this time and more frequently in the future as needed

Dipper wells: observed the water flow to only be coming out at a fast drip. Corrected by opening the water valves slightly to provide a steady stream of water. Ensure enough water flow is provided to flush away ice cream debris from the utensils

Remove rings prior to starting shift or utilize single use gloves to prevent food from soiling jewelry or jewels falling into food

The deteriorated shelf containing wood is still present above the three compartment sink. Provide a replacement shelf that is smooth, easily cleanable and does not contain wood or if wood cannot be avoided ensure the wood is properly sealed to prevent moisture damage

1) observed the floor to be in need of cleaning below and to the right of the two door low-boy cooler (out of serviced unit) in the back of the facility. 2) observed some dust accumulation on the ceiling air return grates. Please clean now and maintain these areas clean in the future

Observed a couple flies (likely fruit flies) back at the mop sink area. Ensure someone is monitoring the facility for the presence of pest. Appropriate steps need to be taken if pests are identified in the facility. Keep the facility clean and dry to prevent fly breeding areas

1) observed excessive water standing on the floor under the ice machine and three compartment sink areas. This is due to water overflowing the receiving drain located in an inaccessible space behind the ice machine (this drain line receives waste water from the ice machine and the western dipping well). As a short-term correction do not use this western dipping well. A permanent correction is needed for this issue. Consider providing a drain line air gap for the western dipping well located immediately below the well so a continuous length of solid drain pipe can be used behind the back counter where it is inaccessible from cleaning and monitoring. A separate drain line will be needed from the ice machine to the floor sink located under the hand washing sink. Provide a sloped drain line from the ice machine drain level to the floor (this drain line must terminate with an air gap). 2) the flow of water is too restricted coming out the bottom of the recently installed flow-through back flow preventer on the mop sink faucet. Contact your chemical supplier to repair/replace this unit so the water flows out of the bottom orifice as required

The interior of the ice making machine is overdue for cleaning. Please clean now and more frequently in the future

Observed: 1) the two floor sinks (under base cabinet at the cash register and in the base cabinet located under the hand washing sink) to be in need of cleaning. Clean these basins now and at least two times per week in the future. Keeping these areas clean will eliminate fly breeding areas. 2) observed an excess of spilled food and misc. Items on the floor of the walk in freezer. Clean this area now and more frequently in the future. 3) the floor under the ice making machine is also due for cleaning

Observed the interior bottom of the front counter delfield one door cooler to be in need of cleaning (to remove moisture and debris accumulation) now and more frequently in the future

Observed the bottom, front edge of the narrow shelf above the three compartment sink to be bare particle wood. This wood is swollen due to moisture from recently washed dishes dripping down the front of the shelf. Find a way to cover this wood with a water-proof material that is smooth and easily cleanable or install a replacement shelf that is not constructed of wood that can absorb moisture

Observed the toe-kick area (including behind the steel toe-kick panels) between the two ice cream dispensing units at the front counter to be in need of cleaning now and more frequently in the future

Observed the hand washing sink and the metal splash guard to be in need of removal, cleaning and re-sealing with silicone to eliminate the gaps where they contact the counter. Currently some sealant is missing and/or detached creating locations were moisture and debris can accumulate

Observed excessive moisture on the floor and mold growth (black in color) under the three compartment sink and ice machine at the back counter. Trouble-shoot this situation and repair as needed

Observed spilled food and misc. Debris accumulation on the floor in the walk in freezer (especially under the shelving units). Clean the floor regularly to remove spilled items

A new chemical feeding system has been installed at the mop sink since the last inspection. The built-in back flow preventer (an atmospheric vacuum breaker built-in to the mop sink faucet) is not compatible with down stream shut-off valves. Install a flow-through back flow preventer (such as a "sidekick" unit) on the end of the mop sink faucet prior to the "y" splitter that feeds water to the chemical dispenser and the mop bucket filling hose

The missing and damaged floor tiles are still present at the middle ice cream dipping cabinet and at the mop sink in the back room. Repair the flooring in these areas to be smooth and easily cleanable

Observed the dipper well to be turned off to one of the dipper wells with previously used ice cream scoops in them. Corrected by the employee washing all the items from this dipper well and turning on the dipper well once the utensils were returned and used for scooping ice cream

(Critical) Observed the cappuccino mix containing dairy ingredients to be expired in the one door prep cooler. Corrected by discarding this mix

Again observed several blending pitchers stored inverted on a solid shelf located above the three compartment sink. Please provide some bar shelf matting to facilitate the air drying, allow for air circulation and to keep these pitchers out of any moisture that accumulates on the shelf.

Observed the floor sink located under the three compartment sink to be in need of cleaning to remove debris build-up. Please clean this drain area now and at least a couple times per week to maintain it clean
