In the bottom portion of the 3 door prep top cooler. Pic states cooler coil was cleaned, and set to a colder setting. Violation corrected.

Pic temporarily corrected by dialing cooler to a colder setting and states employee may have bumped control when cleaning cooler. Pic has been instructed to monitor and ensure food is held at 41°F. I will return to verify that cooler holds tcs food at 41°F. ,

Found the 3 door prep top cooler in the kitchen not holding tcs food at 41°F. Although, pic has set cooler to a colder setting, verify and ensure tcs food is held at 41°F and repair if necessary.

Found various food stored in plastic grocery bags in the walk in freezer. To correct discontinue using plastic grocery bag to hold food, and replace with food grade storage bags or containers. Found cans being reused to hold food in the walk in cooler. To correct discontinue using cans that previously held food to store food.

Observed 2 employees working in the cooking/prep area not wearing hat/hair restraint. To correct provide hat/hair restraint and remind employees to wear them.

Found opened employee water bottles on the prep table. To correct either have employees drink in designated areas only or use cups that minimize contamination, such as cups with lids and straws in prep areas.

Found an employee's bottle of vitamins stored with mustard packets. Found an employee's bottle of herbal supplement stored on back kitchen prep area table. Pic corrected by relocating vitamins and herbal supplement to employee storage area.

Found the floor under shelves, corners and edges in the storage room soiled with accumulation and build up. To correct thoroughly clean floor and keep clean.

Pic corrected by discarding egg rolls and chicken and has been instructed to store tcs food in coolers. Found tcs food, cartons of raw shell eggs 50°F on the shelf under the prep table. Pic states eggs were purchased about 2 hours ago. Pic corrected by placing eggs in the walk in cooler to rapidly cool, and was reminded to place tcs food/eggs in cooler as soon as possible. ,

Found the storage room shelves and storage room platform soiled with flour and debris. To correct clean noted areas and clean on a more frequent schedule.

Observed employee move cooling fried chicken from a baking sheet to a plastic tub. Pic corrected by having employee move chicken back to baking sheet and place sheet trays of fried chickent in walk in cooler to rapidly cool. Pic was reminded if desired to place chicken in plastic tubs after chickent has cooled to 41°F.

Found the thermometer in the 3 door prep top cooler not working. To correct provide a working internal thermometer in cooler.

Found the floor (under equipment, edges and corners) in the entire kitchen soiled with accumulation and build up. Found the ceiling in the kitchen soiled with build up. To correct thoroughly clean ceiling and floor and keep clean.

Found cardboard lining shelves through out facility. To correct remove cardboard, and if desired use lining that is durable, smooth, and cleanable.

Found the tiled floor in the back prep area chipped and cracked. Found the wall by the mop sink faucet damaged and peeling. To correct repair noted areas and keep areas in good repair.

Found a wood knife/cleaver holder on the back prep table. Found meat and food products stored in plastic grocery bags. Found boxes used to hold fried wontons in the walk in freezer. To correct discontinue using wood knife holder, grocery bags and boxes to hold/store food/food utensils and replace with commercial grade utensils.

Found the unisex restroom door propped open. To correct keep restroom door closed.

(Critical) Used a chlorine test kit and found the chlorine sanitizing concentration in the tub in the 3 compartment sink very strong, clearly above 200 ppm. Pic corrected by having employee remix sanitizing concentration to manufacturers specification of 200 ppm. This is a repeated critical violation and a risk control plan, sanitizer concentration has been issued. ,

Found some grease on the cooking hood gutter and edges. Found the underside of the shelf above the steam table soiled with splash. Found the interior of the microwave soiled with splash and accumulation. To correct clean noted areas and clean on a more frequent schedule.

Found dented cans stored in a box without label. To correct provide/label box as "dented cans for return to supplier for credit. "

Found a cloth used to line the bottom of steam holding wontons. To correct discontinue using cloth to line steamer. Found plastic grocery bags used to hold food supplies in the coolers and freezers. To correct discontinue using grocery bags and use food grade storage bags. Found facility using a wood utensil with slots to hold cleavers. To correct discontinue using this wood utensil, as it is not designed to be easily cleanable, and store utensils in a suitable utensil such as a container or tray.

Found a towel left in the front counter handsink. To correct do not leave items in handsink, and keep handsink easily accessible.

Found the lights in ceiling partially working. To correct repair/replace lights to work properly to provide adequate lighting.

Found the tiled floor in the back kitchen prep area with chips and damaged tiles. To correct repair/replace damaged tiles.

Found gaps along the floor and back door. To correct repair/replace door sweep to eliminate door gaps.

(Critical) Found a nonpotable hose connected to a hose bibb, which then connected to another hose supplying water to the prep sink. Found a nonpotable hose running through a pipe attached to the ceiling apparently used to supply additional water to the 3 compartment sink. To correct remove all nonpotable water hoses that are used to supply sinks, properly repair plumbing system to adequately supply water to sinks. ,

(Critical) Found the chlorine sanitizing solution in a tub (of the sanitizing compartment of the 3 compartment sink) at a very high concentration. Pic corrected by diluting sanitizing solution to the proper range (50 ppm minimum and 200 ppm maximum). Observed water dripping from the fan cover in the walk in cooler. To correct repair to prevent water from dripping from fan cover.

Found a bag of frozen chicken thawing on a baking sheet on the speed rack. Employee corrected by placing chicken in the walk in cooler to thaw.

Found several floor tiles in the back prep area cracked. Found several waterstained ceiling tiles in the storage room. To correct repair/replace floor and ceiling.

Found fried chicken cooling in large covered plastic tubs in the walk in cooler. Pic corrected by having employee place fried chicken in baking sheets to rapidly cool. Pic has been reminded to monitor that employees are reducing size of food to be cooled, and that food is cooled from 135 to 70°F in 2 hours, and then from 70 to 41°F in 4 hours.

Found the edges of the cooking hood and filters soiled with greasy build up. Found the top and edges of rice warmer soiled with build up. Found the shelves under counter tops soiled with accumulation of debris. Found the door gaskets and edges of cooler doors soiled with accumulation. To correct clean as soon as possible, and clean on a more frequent schedule to preclude accumulation.

Observed cook deep fry chicken nuggets and place chicken in a "deep" tub to cool. Pic corrected by having cook place in a baking sheet (thin layer) to rapidly cool. Pic has been reminded to monitor that employees are properly cooling foods, that is placing food in shallow pans/baking sheets, and then rapidly cool food in walk in cooler/freezer from 135°F to 70°F in 2 hours, and from 70°F to 41°F in 4 hours.
