(Critical) Observed the first aide kit stored on the cook line top loading cooler shelf. First aid supplies that are in a food establishment for the employees' use shall be: (a) labeled as specified under # 7-101. 11; (pf) and (b) stored in a kit or a container that is located to prevent the contamination of food, equipment, utensils, and linens, and single-service and single-use articles. (p) to correct move the noted item to the office to store it away from food, equipment, utensils, and linens, and single-service and single-use articles. Corrected: person in charge moved it to the front of the house away from all food, equipment, utensils, and linens, and single-service and single-use articles

(Critical) Observed employee medication stored on dry storage shelf in the back. (a) except for medicines that are stored or displayed for retail sale, only those medicines that are necessary for the health of employees shall be allowed in a food establishment. (pf) (b) medicines that are in a food establishment for the employees' use shall be labeled as specified under # 7-101. 11 and located to prevent the contamination of food, equipment, utensils, linens, and single-service and single-use articles. (p) to correct move the noted items to the office. Corrected: person in charge moved the noted items to the office

(Critical) Observed interior ceiling of both microwave to have a food debris build up. Equipment food-contact surfaces and utensils shall be cleaned: (1) except as specified in # (b) of this section, before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry; (p) (2) each time there is a change from working with raw foods to working with ready-to-eat foods; (p) (3) between uses with raw fruits and vegetables and with potentially hazardous food (time/temperature control for safety food); (p) (4) before using or storing a food temperature measuring device; (p) and (5) at any time during the operation when contamination may have occurred. Observed the front of the house beer cooler to be at 56f. Note: no potentially hazardous food items were being stored in the cooler. Equipment shall be maintained in a state of repair and condition to correct have the noted cooler repaired. ,

Observed a large amount of food being stored in reused single service containers (containers that came with food originally stored in them). Single-service and single-use articles may not be reused. To correct purchase reusable food grade containers for food storage

Observed the floors to be in poor repair. Physical facilities shall be maintained in good repair. To correct repair the floors

(Critical) Observed the spray arm hanging below the flood rim of the drain board on the dish line. An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than 25 mm (1 inch). (p) to correct provide an air gap of at least one inch between the end of the hanging spray arm and the very upper edge of the drain board

(Critical) Observed the following potential cross contamination: 1. Raw eggs being stored above ready to eat food in the walk-in. 2. Raw egg containers stored on top of lettuce. 3. Raw eggs containers with cutting boards stored on top of them. Food shall be protected from cross contamination by: (1) separating raw animal foods during storage, preparation, holding, and display from: (a) raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables,(p) and (b) cooked ready-to-eat food; (p) to correct: 1. Move raw eggs down and away from all ready to eat foods and stored by cook of temperature. 2,3. Discontinue using raw egg containers throughout the facility

(Critical) Observed the following soiled food contact surfaces: 1. Knives hanging in the cook line area on magnets. 2. Underside of the large mixers arm, the part that hangs over food. 3. Underside of shelves on steam table and cook line cooler. Equipment food-contact surfaces and utensils shall be cleaned: (1) except as specified in # (b) of this section, before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry; (p) (2) each time there is a change from working with raw foods to working with ready-to-eat foods; (p) (3) between uses with raw fruits and vegetables and with potentially hazardous food (time/temperature control for safety food); (p) (4) before using or storing a food temperature measuring device; (p) and (5) at any time during the operation when contamination may have occurred. Observed the scoops for the rice being stored in standing water. During pauses in food dispensing, food dispensing utensils shall be stored in a container of water if the water is maintained at a temperature of at least 135�f and the container is cleaned at a frequency specified under subparagraph 4-602. 11(d)(7). To correct maintain the water at 135f or store scoops in the food or stored noted utensils on a clean dry surface

Observed a large amount of food being stored in reused single service containers (containers that came with food originally stored in them). Single-service and single-use articles may not be reused. To correct purchase reusable food grade containers for food storage

Observed several food items being stored in plastic grocery bags and open metal cans. Materials that are used in the construction of utensils and food-contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: (p) (a) safe; (p) (b) durable, corrosion resistant, and nonabsorbent; (c) sufficient in weight and thickness to withstand repeated warewashing; (d) finished to have a smooth, easily cleanable surface; and (e) resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. To correct provide food grade bags and containers for food storage.

(Critical) Found the can opener blade and surrounding areas soiled with accumulation of debris. Found knife/cleaver soiled on the knife rack. Pic corrected by having employee wash, rinse and sanitize can opener and cleaver.

Found the floor on the cooking line soiled with grease and the back kitchen area soiled with accumulation. To correct clean floor and clean on a more frequent schedule.

Found several spray nozzles in the dishwashing machine clogged with debris. To correct clean spray nozzles and clean as needed.

Found the tiled floor in the entire kitchen with cracks and uneven surfaces. Found the wall in the dishwashing and prep area damaged with uneven surfaces. To correct repair floor and wall to be durable, smooth and cleanable. Found a gap/crack between the sink and wall. To correct fill gap/crack to be smooth.

Found water dripping from ceiling into 3 buckets in the back kitchen area in front of stair way to basement. Pic states ice build up on the back roof area has caused leak. To correct locate source of leak and repair roof as needed.

Found newspaper covering containers of vegetables in the walk in cooler. To correct replace newspaper with commercial food grade material. Found food stored in plastic grocery bags. To correct store food in food grade bags.

Found the hood filters soiled with greasy build up. To correct thoroughly clean filters and clean as needed.

Found ice scoop and handle laying on top of ice in the bar ice bin. To correct store ice scoop with handle up to minimize contamination.

Found plastic grocery bags used to store food in the walk in cooler and freezers. To correct discontinue using plastic grocery bags and use food grade storage bags. Found newspaper used to cover vegetables in the walk in cooler. To correct discontinue using newspaper and use food grade utensils to cover foods.

Found the floor edges under the cooking line soiled with some accumulation. To correct thoroughly clean floor and keep clean.

Found the tiled floor in the entire kitchen area with numerous cracks. Found the wall behind and under the 3 compartment sink damaged with rough and uneven surfaces. Found the wall besides the water heater in the basement with a large dent. To correct repair/replace floor and wall to be durable, smooth, and cleanable.

Found the back wall under the cooking hood soiled with grease build up. To correct clean the wall under cooking hood.

Found the tiled floor in the kitchen with numerous cracks, loose and uneven surfaces. Found the wall under the sinks and dishwashing area with rough, and uneven surfaces. Found the wall besides the stairs leading to the basement and besides the hot water tank with holes and damaged surfaces. To correct repair/replace the floors and walls to be durable, smooth and cleanable.

Found wiping cloth left on the counter top besides cutting board. To correct store wiping cloths completely submerged under sanitizer.

Found several frozen ducks thawing on the drainboard of the sink in the back kitchen area. Pic corrected by placing ducks in the walk in cooler to thaw.
