Pic stated that cheese has been in warmer for around 3 hours, but she forgot to turn machine on today. All phf must be hot held at 135f or more at all times. Pic corrected by discarding cheese and turning on nacho cheese machine time of inspection. Violation corrected

Observed pop machine carbonator drain line under pop machine in front prep area with a heavy accumulation of mold and debris. To correct, please replace or remove drain line and clean as soon as possible

Observed old mice droppings on back storage room floor (near corner ducts and behind washing machine/hot water heater). Old mice droppings or dead insects may attract more pests and insects to your facility. To correct, please remove all old mice droppings from storage room floors (especially behind equipment) and then clean and disinfect storage room floor as soon as possible

Observed severely damaged/broken floor tiles under dry storage shelves in back kitchen area. To correct, please replace all damaged floor tiles as soon as possible so all surfaces are smooth and cleanable

Observed both wiping cloth buckets in front counter area (near pop machine and dipper well) with a concentration of 0ppm of bleach (buckets were not changed this morning). All in-use wiping cloths must be stored completely submerged in a properly mixed wiping cloth bucket (50-100ppm of bleach) to prevent the accumulation of bacteria on wiping cloths. To correct, please change wiping cloth buckets more often and verify with test strips as needed

Please remember that sanitizers can become toxic and excessive levels. To correct, need to follow manufacturers directions when mixing buckets (in this case 50-100ppm) and verify with test strips as needed. (Critical) Observed food employee eating a cookie in kitchen, then help customers at the front counter, then put on plastic gloves and prep food without washing hands first. Also, did not observe any food employee in facility wash his or her hands after an hour of observation. All food employees must wash hands before engaging in any kind of food prep or any time possible contamination of hands may have occurred.
