Observed the door for the walk in freezer and the wall area surrounding the door in poor condition. Correct by repairing or replacing the door and surrounding area. Pic indicates a master plan for facility renovations is being prepared.

At the main make line observed a majority of the cutting boards with deep grooves and discoloration. Correct by replacing the cutting boards

Observed the facility has no employee on staff who is currently certified under a personnel certification program accredited by ansi, utilizing the conference on food protection standards. Commonly known as a certified food manager. Correct by providing proof of certification for a member of staff who is regularly engaged in food preparation on a full time basis. Item shall be corrected within 90 days.

Under the bagel display case observed several open top cases of bagged potato chips. Several cups of toppings from the bagels have accumulated in the chip boxes. Correct by cleaning the chip cases now and on a more frequent basis to prevent the accumulation of toppings. An alternate storage method for the chips is suggested to prevent the accumulation of the toppings

(Critical) 1) the mounting surfaces for soda dispensing system diffusers are in need of cleaning. 2) many interior surfaces of the ice machine have a slime build-up on them. Fully disassemble this unit for a thorough cleaning and sanitizing per the mfg. Directions

(Critical) No air gap is provided on the drain line for the water softener. The current flexible hose terminates down inside of the floor drain located under a base cabinet. The end of this hose is quite contaminated from being down in this floor drain pipe. ***provide an entirely new piece of hose/tubing for this small diameter drain line and ensure a minimum of a 1" air gap is maintained at the termination above the top of the receiving drain

A small water leak was noted at a connection of the cold water line supplying water to the water softener in the back counter base cabinet. Repair as needed to eliminate the leak

The door gasket of the one door prep cooler near the bagel storage and blender station is torn. Replace this gasket to provide a cleanable surface that doesn't allow food/moisture to accumulate

No chlorine residual was detected in the wiping cloth storage container at the back counter espresso machine station. Corrected by providing a fresh solution of chlorine sanitizer at 100 ppm

(Critical) Observed fruit/vinegar flies in the bottle of amaretto syrup with a pour spout. Corrected by discarding the syrup. The manager indicated she will clean the outsides of all the remaining bottles, remove the pour spouts and only use the screw-on style manufacturer's caps as a physical barrier for fly entry

(Critical) Observed a metal stem food thermometer in a cup above the sandwich prep cooler to be reading about 20f too warm. Corrected by removing the thermometer to the office for calibration

(Critical) Back kitchen: the electric food slicer (behind the blade and the hinged blade guard) and the food processor blade used for cutting cabbage were observed unclean. Corrected by cleaning and sanitizing these pieces of equipment

Observed a few out of the way floor areas and floor drains with debris build-up. Please clean these areas now and more frequently (2-3 times per week) to remove this debris that can be fly breeding areas. Also, the counter under the espresso machine has a build-up of coffee beans

(Critical) No drain line air gap is provided above the flood rim of the receiving sink basin for the barista fria smoothie machine drain line. Elevate your smoothie machine as needed and reconstruct the drain line to provide the required air gap (vertical gap height = 2x the discharge pipe diameter

(Critical) The drain line under the back counter eastern hand washing sink is leaking at a couple connections. Repair as needed

(Critical) 1) the interior of the insulated beverage dispensing spouts for catering events were unclean. Ensure staff are disassembling these spouts for thorough cleaning when the rest of the unit is cleaned after each use. 2) interior surfaces of the ice making machine have some slime growth on them. As soon as possible, turn this machine off, discard the ice, disassemble the machine/hoses as needed and thoroughly wash rinse and sanitize this unit

(Critical) Observed a metal stem food thermometer in the two door display cooler and the one in the walk in cooler to be inaccurate by 5f and 30f, respectively. Corrected by re-calibrating these thermometers

Observed an employee with multiple jeweled rings on her hand to be dispensing bagels. When working with food employees must either wear a glove over their hand(s) or remove the rings. To prevent food from being trapped in the rings and also to prevent any of the jewels from falling into food that could become a choking or tooth hazard should a customer consume it

Walk in cooler (especially along the wall in common with the walk in freezer): observed spilled food and some mold growth on the floor at the base coving. Thoroughly clean this area. Walk in freezer: observed bagels on the floor, especially behind the rearmost bagel racks. Remove spilled food from this unit

1) observed the sanitizer solution for the wiping towel at the espresso machine to be to weak. Provide a fresh sanitizer solution. 2) observed a couple red sanitizer buckets at the front counter area to have soap in them. Wiping cloth pails should only have sanitizer in them. If a soap solution is desired, provide a separate pail (industry standard for soap solutions is a green pail) for the soapy water, and use the red pails for your wiping cloth storage in a sanitizer

Observed the temperature gauge to be inoperable on the dish washing machine. Water in the tank was at 83f, however, the gauge was registering 140f. Adjust, if possible, to be accurate or provide a replacement thermometer

Observed individually wrapped, grab and go rice crispy treats provided without required labels. Provide labels containing necessary information (product name, ingredients, weight, food allergens, address, etc. ) on these products. Please contact me for specifics if needed

1) observed the drain lines for the boiling kettles to have dropped down. Re-establish the required drain line air gap (vertical air gap distance = 2 x the discharge pipe diameter. 2) re-establish the drain line air gap above the front kitchen area sink for the barista fria smoothie machine. 3) observed a small amount of water dripping out of the back flow preventer (watts 9-d) for the espresso machine. Provide an air gapped drain line for this port at a nearby receiving drain for disposal. This will also eliminate the damp conditions in this base cabinet

Observed some crumbs in the bottom of the silverware tray. Please thoroughly clean this tray and maintain it clean

(Critical) Observed both of the fasteners on the interior of the ice machine bin that secure the deflector shield to the side of the bin to be covered in black electrical tape. This tape is not smooth and easily cleanable as evidenced by some growth on these surfaces. Provide replacement original fasteners that have a smooth, easily cleanable surface

Observed several plastic and metal food pans/pots stored at the front counter area were observed stacked while still wet. Allow enough time for your equipment to fully air dry prior to stacking in storage

1) a significant amount of water was observed on the floor behind the ice machine. Have someone trouble shoot the problem and make necessary repairs. 2) re-establish the drain line air gap for the "barista fria" smoothie machine drain line above the flood rim of the adjacent sink

Broken floor tile in a variety of locations are still present. Repair as needed to provide a smooth and easily cleanable flooring surface

Observed split and torn door gaskets on the three door sandwich make cooler. Install replacement gaskets as needed to provide a smooth and easily cleanable surface and to keep your cold air sealed in the unit

Dish washing machine: observed the stainless steel housing for the food trap screen to be in need of cleaning at this time. Please ensure staff clean this housing regularly to eliminate food buildup that can be a breeding area for pests.

Observed the potential for accidental contamination of foods in the top of the one door prep cooler that is located next to a hand sink. Install a splash guard that is smooth and easily cleanable to prevent the contamination of food (be sure to properly seal all joints to prevent debris/moisture accumulation with silicone sealant).
