(Critical) Observed the following holding above 41f for under an hour in the top loading coolers on the prep line: 1. Small pan of artichokes 45f. 2. All facility made dressings 47f. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct move the noted food to the walk-in cooler to rapidly cool it down to 41f or below. Corrected: person in charge moved the noted items to the walk-in to rapidly cool the product to 41f or below

(Critical) Observed 4 spatulas torn and melted in clean dish storage. Multi use food-contact surfaces shall be: (1) smooth; (pf) (2) free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections; (pf) (3) free of sharp internal angles, corners, and crevices; (pf) (4) finished to have smooth welds and joints; (pf). To correct replace the noted items and then discard the melted/torn ones. ,

Observed the following parts of the facility in need of repair: 1. Floors throughout the kitchen and front service line. 2. Walls throughout the kitchen. 3. Some ceiling tiles are sagging in the kitchen. Physical facilities shall be maintained in good repair. To correct repair the noted items

Observed food being stored in open metal cans. Materials that are used in the construction of utensils and food-contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: (p) (a) safe; (p) (b) durable, corrosion resistant, and nonabsorbent; (c) sufficient in weight and thickness to withstand repeated warewashing; (d) finished to have a smooth, easily cleanable surface; and (e) resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. To correct transfer the noted food to a food grade container. Corrected: person in charge transferred the noted item to a food grad lexan container.

Observed the atmospheric vacuum breaker leaking water when turned on at the mop sink. A plumbing system shall be: (a) repaired according to law;(p) and (b) maintained in good repair. To correct repair the noted item to eliminate the leak. ,

Observed both gaskets torn on the loading reach-in cooler nearest to the cash register. Equipment shall be maintained in a state of repair and good condition. To correct replace the noted gaskets

(Critical) Observed cooked onions and cooked green peppers that were just prepped at 50f, cooling in the top-loading section of the reach-in cooler located on the serve line. Cooling shall be accomplished in accordance with the time and temperature by using one or more of the following methods based on the type of food being cooled: (1) placing the food in shallow pans; pf (2) separating the food into smaller or thinner portions; pf (3) using rapid cooling equipment ; pf (4) stirring the food in a container placed in an ice water bath; pf (5) using containers that facilitate heat transfer; pf (6) adding ice as an ingredient; pf or (7) other effective methods. Pf to correct: move the noted food to the walk-in cooler to rapidly cool them to 41f or below and discontinue cooling food in the top-loading section of the reach-in cooler and start cooling the noted food in the reach-in cooler. Corrected: person in charge moved the noted food to the walk-in cooler to rapidly cool it down to 41f or below and stated that they would discontinue cooling food in the top-loading section of the reach-in cooler

(Critical) Observed multiple food items in the top loading reach-in cooler holding between 45-48f for about 2 hours due to a non-working cooler. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct move the noted food to the walk-in cooler to rapidly cool the food to 41f or below. Corrected: person in charge moved the noted food to the walk-in cooler

(Critical) Observed the food processor blending container and lid to have large cracks and breaks. Multi use food-contact surfaces shall be: (1) smooth; (pf) (2) free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections; (pf) (3) free of sharp internal angles, corners, and crevices; (pf) (4) finished to have smooth welds and joints; (pf) to correct: discard the noted item into the garbage and purchase a new one. ,

Observed the following: 1. Top loading reach-in cooler not holding cold food at 41f or below. 2. Ice build up in the display cooler in the lobby. Equipment shall be maintained in a state of repair and condition to correct: 1. Repair the noted cooler to hold cold food at 41f or below. 2. Repair the noted cooler to eliminate the ice build up. ,

Found the entire floor in facility with worn, cracked, chipped and missing tile in the dining area, cooking and prep areas. To correct repair/replace tiled floor to be smooth and cleanable.

Found the surfaces of the conveyor oven and hood, which is not used soiled with dusty build up. Found the floor in the walk in cooler soiled with accumulation of debris. To correct clean equipment surfaces and floor and clean on a more frequent schedule.

Found the internal hanging thermometer in the 2 door prep top cooler not indicating proper temperature. To correct replace thermometer with a properly functioning thermometer.

Found the pop nozzles in the self service pop machine sticky. To correct follow manufacturer's directions and clean pop nozzles.

Observed employee with beard washing dishes in the back kitchen area. To correct provide beard net and remind employees with beards to wear them.

Found various old equipment in the basement storage area floor. Found the conveyor type oven in the cooking area no longer used for operations. To correct evaluate if equipment is needed for operations and remove if no longer needed.

Found the surfaces of the range top and areas surrounding burners soiled with build up and accumulation. To correct clean range and keep clean. Found the pop nozzles of the self service pop machine sticky. To correct follow manufacturer directions and clean pop nozzles.

Found the ceiling in the cooking and prep area soiled with dusty build up. Found the stairs leading to the basement soiled with dirt and accumulation. To correct clean noted areas and keep clean.

Found the tiled floor in front of the self service pop machine with cracked tile and worn grout. Found the tiled floor in the entire cooking and prep area with chipped and cracked tile. Found the walls in the dishwashing and storage area with damaged surfaces. To correct repair floors and wall to be durable, smooth and cleanable.

(Critical) Found time/temperature control for safety (tcs) food, krab cakes 46°F, cashew sauce 46°F, cut kale 46°F, and spinach balls 46°F in the bottom shelf of the right 2 door prep top cooler. Pic temporarily corrected by discarding tcs food, and has been instructed not to use the bottom shelf of this cooler to hold tcs, until repaired/replaced to hold tcs food at 41°F. Note: foods in other areas of this cooler was holding at 41°F, and an administrative conference notice was not issued as colding holding violations were of a different nature. ,

Found the bottom shelf of the right 2 door prep top cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower. Found the paper towel dispenser in the back prep area broken with paper towels on top of dispenser. To correct repair/replace paper towel dispenser.

Found the hood filters, edges and sides of the pizza oven hood soiled with dusty build up. To correct clean hood filters and hood areas, and keep clean.

Found the stairs leading to the basement and the basement floor soiled with accumulation of debris. Found the basement floor soiled with accumulation of debris. Found the main level floor (under equipment, edges and corners) soiled with light accumulation. To correct clean all noted areas and clean on a schedule to preclude accumulation.

Observed water dripping from the drip tray under the pop machine carbonator in the dining self service area, and found the basement floor and ceiling areas under the pop machine water stained. To correct identify source of leak, repair leak, and properly repair affected areas.

Found the tiled floor surrounding the pop machine in the dining area cracked, loose, and with damaged grout. To correct properly repair tiled floor.

Found various equipment, utensils and other supplies no longer needed for operations. To correct determine if items are needed and remove items not needed for operations.

Observed the tiled floor in the back kitchen area in the process of being repaired, that is black mats placed above wood floorboards and rough edges on tiled floor. Pic states floor is being repaired and should be finished soon. Observed a black mat covering damaged part of the tiled floor in the front counter prep area floor. Pic states this area is to be repaired. Found the drywall surrounding storage shelves with dents, scratches and uneven surfaces. To correct repair all noted areas, and finish areas to be durable, smooth, and cleanable.

(Critical) Found time/temperature control for safety (tcs) food, black beans 48°F, rice 47°F, and sweet potato 46°F in the bottom right portion of the 2 door prep top cooler. Pic states cooler is new, and we observed the cooler's power cord was caught between door gasket not allowing right door to properly seal. Pic corrected by discarding tcs food, and has been instructed to monitor and ensure tcs food is held at 41°F. I will return to verify that this cooler is working properly. ,

(Critical) Found a container of caesars dressing passed it's 7 (day prepared + 6 days) day datemark in the 2 door prep top cooler. Observed the kitchen wall and tiled floor in various areas of facility with damaged surfaces. To correct repair wall and floor to be durable, smooth and easy to clean.

Found the cold water valve of the handsink in the front counter cooking/prep area turned off. Pic corrected by turning on cold water valve, and was reminded to provide hot and cold water to properly wash hands.

Observed employee working in the dishwashing area with a large beard without beard restraint. To correct provide beard net and remind employees with beards to wear them.

Found the hood filters soiled with dusty, greasy build up. To correct clean hood filters and clean on a more frequent schedule.

Found wrapped sleeves of cups stored on the floor in the storage cabinet under the self service pop machine. To correct store cups off the floor in a more protected manner.

Found the vent covers and surrounding ceiling area soiled with dusty build up. To correct clean ceiling and vent cover

Found packages of chocolate and carrot cake available for sale on the front counter display case with no ingredients available for consumer. Pic states cakes come in from a bakery without label. To correct have ingredients available for consumers upon request.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, a container of hummus, and corn mix with 10 day datemarks. Pic corrected by discarding hummus and corn mix, and has been instructed to monitor that employees are dating rte, tcs food with 7 day (day made + 6 days) datemarks.

Found the tiled floor in the kitchen and bathroom with holes and chipped tile. To correct repair/replace damaged tile.

Found the hood filters and light cover soiled in the cooking hood soiled with dusty build up. To correct clean as soon as possible and clean on a more frequent schedule.

(Critical) Found time/temperature control for safety (tcs) food, a container of cooked pasta 50°F in a "double" pan on the top portion of the left 2 door prep top cooler. Found tcs food, veggie sausage 50°F, and portioned par-cooked potaotes 50°F in a "double pan" on the top portion of the right 2 door prep top cooler. Pic states food was taken out recently, and corrected by placing all tcs food in the walk in freezer to quick chill back to 41°F. Pic has been instructed to discontinue double panning tcs food and to ensure tcs food is held at 41°F or lower. Note: both 2 door prep top coolers appear to be working as some items in cooler were at 41°F. ,

Found the floor under and around the rangetop/grill soiled with build up and accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

Found the walls behind storage shelves with scratched and damaged surfaces. To correct repair and finish walls to be smooth and easy to clean. Found ceiling tile in center of back kitchen out of place. To correct replace out of place ceiling tile.

Found plastic squeeze bottles of oil without label. To correct provide labels.

Found dented can on back kitchen shelf not clearly identified "as dented cans, return for credit". To correct provide a sign that identifies this shelf area as "dented cans, return to supplier for credit".

Found the cooking hood and filters, and back splash of range top soiled with greasy build up. To correct clean as soon as possible, and clean on a more frequent schedule.

Found the tiled floor in the back kitchen area, and in the public restrooms chipped with exposed surfaces. To correct repair/replace tiled floor, to be smooth and easy to clean.

Found stacked portions cups and lids stored upright on the counter top of the prep top cooler. To correct store cups and lids inverted.

Found a can of beans on the floor holding door open. To correct discontinue this practice of using canned food to hold door open. Pic was reminded to clean can before using.

Found covered soups cooling on the bottom shelf of the prep top cooler. Pic corrected by placing soups in the walk in cooler uncovered. Pic was reminded that tcs foods must be cooled from 135°F to 70°F in 2 hours and then from 70°F to 41 f in 4 hours.
