(Critical) Observed an open container of milk in the silver refrigerator with a last day of use label indicating 3-15. Corrected by discarding

(Critical) Observed some loose bait trays for mice in the kitchen corner base cabinet next to the dish washing machine. Provide an enclosed / tamper-proof bait station with cubed bait.

(Critical) A small package of deli meat that was thawed in the yellow refrigerator with out a use by date. Corrected by discarding.

Several single use plates and trays were observed to be stored upright and un-packaged in the base cabinet across from the yellow refrigerator. Provide storage bags to protect these food contact surfaces

(Critical) Observed three containers of chlorine bleach in the main kitchen. All three of these were a scented bleach product. Provide at least one container of regular, unscented bleach for sanitizing food contact surfaces. The bleach purchased for this task must have label instructions for sanitizing food contact surfaces

Many interior surfaces of the bar ice making machine have some slime growth on them. Follow the manufacturer's instructions to wash, rinse and sanitize this unit to remove this build-up

Observed a very small water leak from the underside of one of the bar sinks. Trouble-shoot the exact cause and repair/replace as needed to prevent the water leak

Health Report - 03/02/2012 (2 critical incident(s))

(Critical) Observed three covered and stacked containers of black eyed peas prepared the previous day to still be at 49 to 57f in the yellow domestic kitchen cooler. Also, observed a full glass pan of macaroni & cheese prepared the previous day to be at 56f. These items were discarded.

(Critical) Observed the white domestic roper refrigerator to be operating at an elevated temperature. Butter was found to be at 47f. Repair/adjust as needed to maintain foods at 41f or below. No potentially hazardous foods may be stored in this cooler until it can maintain foods at 41f or below. Currently there are no potentially hazardous foods located in this unit

Observed the soda gun spouts at the basement bar to be in need of cleaning at this time and more frequently in the future. Corrected by cleaning this items

The two residential refrigerators have standard glass light bulbs installed in them. Replace these with a bulb that has a shatter-proof coating

Observed the interior of the microwave oven to be in need of cleaning at this time

The white roper residential refrigerator in the back room is lacking an air thermometer. Provide a thermometer for this unit so the air temperature can be monitored

(Critical) Observed bbq/smoked meats (chicken, chunks of pork, and ribs) on trays and a plate tightly wrapped with plastic wrap that were cooked late last night. These foods were stored stacked in a residential refrigerator and found to still be at 46 to 62f. Corrected by discarding. In the future, utilize some additional rapid cooling techniques to expedite the chilling of these foods (a handout on rapid cooling was provided)

(Critical) Observed two dozen cartons of raw, in-shell eggs stored on the upper shelves of the refrigerator above ready to eat foods. Corrected by relocating the egg cartons to the bottom drawer of the refrigerator

Please provide an air thermometer in the residential refrigerator where the bbq meats were stored

(Critical) Provide an air gapped discharge for the bar sink used as an ice bin

Provide a door sweep for the western exterior door to the main floor kitchen

Ensure the bar blender mixing tip is cleaned at the end of each night the bar is operational

Please keep your stainless steel bar blending cup inverted to prevent miscellaneous contamination

Obtain chlorine test papers to be able to verify your sanitizer strength.

Provide commercial, non-absorbing bar matting to replace the existing shelf liner on the serving tray at the bar.

(Critical) Upper level refrigerator: observed apples, onions, potatoes and coleslaw mix & dressing in the bottom pull out drawers located below multiple packages of raw animal foods. Corrected by discarding the onions and potatoes (due to spoilage) and relocating the other items to the top shelf of the refrigerator

(Critical) 1) found a package of moldy pepper jack cheese in the upper level refrigerator. 2) found rotting onions and potatoes in the bottom drawer of the upper level refrigerator. 3) found small flies in the bottom of a bottle of cognac and brandy. All foods above were discarded. Please inspect the refrigerators frequently to purge any old foods, and keep your open liquor bottle spouts capped to prevent pest entry

Observed evidence of mouse activity (mouse feces and wood shavings) located in the bottom of the upper level kitchen corner cabinet. It is unclear if the mouse evidence is still present from last inspection (11-10-10) or if it was cleaned up and this is from subsequent activity. Regardless, please clean up and sanitize this area asap

No soap is provided in the women's restroom. Provide soap prior to opening tonight

1) observed a minor water leak in the men's restroom on the hot water line shut-off valve. Please repair. 2) observed standing water in the utility tub located in the basement. Please clear any clogs so waste water can drain away

Observed missing and detached base coving in the lower level bathrooms. Please repair to provide a cleanable and water proof surface that seals off the interior of the walls. Observed water that has seeped into the basement along the east side where the booths are located. Please take necessary steps to prevent water from seeping into the building.

The bar area floor (employee side) and under the ice machine are overdue for a thorough cleaning. Please clean these areas now and more frequently in the future
