(Critical) On follow up inspection observed no ants in the back room floor. Pic states ants have been treated and crack in building sealed to prevent ants from entering. Risk control plan for ants has been reviewed and should work if followed. Violation corrected.

(Critical) On follow up inspection found the sanitizing solution in the spray bottle at about 200 ppm. This violation was corrected on routine inspection, and reverified on follow up.

(Critical) On follow up inspection found the interior front shield of the ice machine clean. Pic states ice machine was cleaned. Risk control plan for soiled ice machine has been reviewed and should work if followed. Violation corrected.

(Critical) Used a chlorine test kit and found the sanitizing solution in the spray bottle clearly above 200 ppm. Pic corrected by making a fresh sanitizing solution to manufacturer's specification of 200 ppm.

(Critical) Found the interior front shield of the ice machine soiled with mold. To correct follow manufacturer's directions and clean ice machine. This is a repeated violation a risk control plan must be submitted.

(Critical) Found live ants on the floor in front of the pop syrup storage rack. To correct keep facility clean and dry, in good repair, and contact pest control company for treatment. This is a repeated violation and a risk control plan must be submitted.

Found single packs of advil and aleve, and nuun drinking tablets passed it's expiration date, on the front counter display shelf. Pic corrected by removing items from display shelf, and has been instructed to monitor and ensure items for sale are not passed it's expiration date and honestly presented.

Found the floor under equipment, edges and corners in the storage room soiled with accumulation of debris. To correct throughly clean floor and clean on a more frequent schedule.

(Critical) Observed numerous ants on the floor under the boxed pop storage rack. To correct clean floor, keep areas clean and dry, in good repair, and contact pest control company for treatment.

(Critical) Found some of the pop nozzles of the pop machine in the front counter area soiled with heavy accumulation. Found the interior front shield and surfaces of ice forming panel of the ice machine soiled with moldy build up. To correct follow manufacturers directions and clean pop nozzles and ice machine.

(Critical) Found a bottle of sanitizing tablets stored in drawer with clean utensils. Pic corrected by placing tablets in chemical storage area.

Found water dripping from the back of the ice machine. To correct locate source of leak and repair ice machine.

Found a wet mop left in the wringer of an empty mop bucket in the mop sink. To correct hang mop and allow mop to air dry.

Found single packs of aspirin expired on 2/13, tylenol on 7/12, and nuun drinking tablets on 2/14 on the front counter display shelf. To correct check food products for expiration dates, remove items passed expiration date from sale, as items for sale should not be passed it's expiration date.

(Critical) Found the drain line of the ice bin in the front counter area without adequate air gap. Found the drain line of the ice machine draining into a receptacle without air gap. To correct provide an air gap of at least 1 inch from the drain lines and the rim of the drain receptacle.

(Critical) Found no thermometer available to take food temperatures (cooking, hot and cold holding temperatures). To correct provide thermometer.

(Critical) Found no chlorine test kit available to check and verify proper sanitizer concentration. To correct provide chlorine test kit.

Found single use utensils ( wiping towels and gloves) stored under the drain line of the sinks. To correct either relocate items away from drain lines or shield drain lines.

Found no handwashing sign in front counter handsink area. To correct provide sign reminding employees to wash their hands.

Found the mop left in the mop bucket wringer. To correct when clean task has been completed, hang mop allowing it to properly air dry.

Found the floor under and around the ice machine in the storage room and the front counter ice bin, soiled with accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

(Critical) Found a spray can of ant pesticide not approved for use in a food establishment. To correct discontinue using ant pesticide and if desired replace with an ant pesticide that is approved for use in a food establishment. ,

(Critical) Found a moveable household sprayer on the 3 compartment sink which can be left below the flood rim of the sink. To correct provide information that sprayer has backflow protection or provide a way to keep the sprayer at least 1 inch above the flood rim of the sink at all times or remove sprayer. ,

(Critical) Found the interior front shield of the ice machine in the back storage room soiled with accumulation and build up. To correct follow manufacturers directions and clean ice machine, and clean on a schedule to preclude accumulation. ,

Found facility using a metal stemmed thermometer with a range of 50 to 550°F, which can not measure cold holding temperatures of 41°F or lower. To correct provide a thermometer that has a range of at least 0 to 220°F, to measure cold hold temperatures of 41°F and lower and hot holding temperatures of 135°F and above.

Found no antichoking poster on the kitchen wall. To correct provide and post antichoking poster on kitchen wall.

Observed mouse droppings on the bottom of cabinet shelves in the front counter and on wrapped packages of beverages on shelves in the back storage area. To correct eliminate any holes, cracks and gaps in facility, keep areas clean, and continue mice control measures. Observed the presence of ants on the bottom shelf of front counter cabinets. To correct keep areas clean and continue ant control measures.

(Critical) Used a chlorine test kit and found the sanitizing solution in the sanitizing spray bottle which is used to sanitize food contact surfaces clearly over 200 ppm. Employee corrected by re-mixing sanitizing solution to manufacturers specification of 200 ppm. Pic has been instructed to monitor and retrain employee making sanitizing solutions, and to use chlorine test kit to verify proper concentrations.

Found facility with a quat test kit, but using chlorine as a sanitizer, and no chlorine test kit available. To correct provide appropriate test kit to check sanitizing solution, that is provide chlorine test kit.

Found the bottom shelves of cabinets soiled with debris and cobwebs. To correct clean cabinets and clean on a more frequent schedule.

Found no covered receptacle for sanitary napkins in the ladies public restroom. To correct provide covered receptacle.

Found thermometer to measure food temperatures not available. To correct provide thermometer

Found the floor soiled with accumulation of debris. To correct thoroughly clean floors and clean on a more frequent schedule.

Found no handwashing sign in the "handwashing sink" area of the kitchen. Found no handwashing signs in the mens and ladies public restrooms. To correct provide signs reminding employees to wash their hands at all locations where they may wash their hands.

Observed accumulated residue on the exposed interior portions of the receiving plumbing drain for the three compartment sink (located inside the far right base cabinet). Please clean this drain pipe now and on a frequent schedule in the future

Observed the mop head resting in the bottom of the mop bucket. Please hang up your mop head above the mop sink to facilitate air drying and so any water that drips out can be drained away

The lid for the store room chest freezer is still damaged (bent upward slightly) such that it does not fully close. Excessive frost build-up has accumulated on the interior surfaces. Try to have this lid bent back down so it seals properly

(Critical) Provide a thread on hose connection vacuum breaker (a. S. S. E 1011 device) on the exterior hose connection located on the side of the building toward the parking lot

1) observed the lid for the chest freezer to be damaged (bend upward slightly) such that it does not fully close. Excessive frost build-up has accumulated on the interior surfaces. Try to have this lid bent back so it seals properly. 2) provide a replacement ice scoop as the current one is cracked. Consider an aluminum one that will be more durable

Observed several interior surfaces of the ice making machine and ice bin to be in need of cleaning at this time. Have a mechanical equipment repair technician or refrigeration maintenance employee turn off the unit, discard ice, disassemble unit as needed and then wash (hot soapy water), rinse, and then sanitize (50-100 ppm chlorine sanitizer) this equipment prior to re-installation. The cleaning of this equipment should be performed prior to each season and then approximately 3-4 months afterward to maintain the unit clean
