(Critical) Observed three foods in the bottom of the one door pepsi cooler in the kitchen that did not cool properly. First thing this morning, a large container of rice, a bag of cooked breaded chicken and a deep 1/2 pan of a pork based sauce/gravy to be at 43 to 49f. Other foods in the refrigerator were at 36 to 38f. Corrected by discarding. Chinese cooling directions were provided with this report

(Critical) An employee used his bare hands to some spices, green onion pieces and dried, hot peppers to a cooked food item. Corrected by discussing this issue with the employee and by using a utensil to dispense these items as needed

(Critical) Some frozen foods are again stored in white plastic to-go bags. Ensure only food-grade bags are used in direct contact with foods. Remove these foods and place them in food service bags

(Critical) Observed kitchen employees only washing and rinsing off equipment (cutting boards, bowls and sheet trays) after use. All equipment must be washed, rinsed and sanitized after use

(Critical) The array brand of chlorine bleach available in the facility does not have an epa registration, nor does it have use directions for sanitizing food contact surfaces. Purchase and use a bleach approved for sanitizing food contact surfaces

Observed many wet wiping cloths stored on the kitchen counters. To prevent bacterial growth, keep these wiping cloths stored in a container of sanitizer at the proper strength

Again, several sheet trays were observed stored upright on the floor and propped up against the wall. All of your food contact surfaces need to be stored in a protected manner and off of the floor

The prep refrigerator was operating at a slightly elevated temperature. The unit was adjusted to a colder setting during the inspection

Observed several broken / unused pieces of equipment (chest freezer, slicer, deep fryer, etc. ) being stored in the basement. Remove these items

A tray of thinly cut strips of raw pork was found in a back storage room on top of two containers thawing. This pork was already thawed. Ensure your foods are thawed in a refrigerator, as part of the cooking process, or under running water

The fish tank is now partially under the drain line for the three compartment sink. Shift the location of this tank so it is no longer under any part of the wastewater drain line

Found the bulk food bin of rice with the scoop handle in contact with rice. Should store the handle of the scoop extended above the food for proper storage of the scoop. Found bin of knives stored with tools in unclean bucket located on shelf above the kitchen food prep table. Should store knives in a clean approvable location to protect food contact surfaces between uses

Observed several wiping cloths placed on counter without sanitizing solution. Should store wiping cloths properly for food safety

Found several tea pots with water residue remaining in the empty tea pots store on shelf under the wait counter. To correct should air dry tea pots after warewashing

(Critical) Observed chicken water/broth in a metal pot stored on the counter near the chinese range to be at 80f. This liquid was being saved to use later. This product must be maintained above 135f or quickly cooled down and held at 41f and below. Corrected by discarding this broth

Observed whole chicken breasts that were placed in the chest freezer today (not fully frozen yet) to not be entirely covered with plastic wrap. Ensure all of the chicken is fully covered to protect it from misc. Contamination

Observed the pepsi glass door cooler in the kitchen to be operating at a slightly elevated temperature. The temperature control dial was adjusted from 2 to 3. 5/4. Monitor your food temperatures to ensure they are being held at 41f and below

Observed several food employees in the kitchen rinsing/washing their hands in the three compartment sink. All hand washing must occur in the designated hand washing sink

(Critical) Observed 3 to 4 woven wooden (bamboo) steaming baskets on the shelf above the dish washing sink. These baskets are not smooth and easily cleanable. Remove and replace these items with commercial metal steaming baskets

(Critical) Staff indicated they use the one compartment sink in the back of the dining room to wash plastic drinking cups and tea cups. These items must be sent to the kitchen so they can be washed, rinsed and sanitized in the three compartment sink

(Critical) Observed many kinds of food stored in direct contact with brown and white shopping bags (t-shirt bags). Only utilize clear food bags for direct contact with the food. To correct, remove foods from these bags and place food in the clear food service bags

Observed several wet wiping cloths stored on the kitchen counter. Please store your wet wiping towels in a container of sanitizer at the proper strength

The floor in the following areas is in need of cleaning: under the drain for the chinese cooking range, the floor in the basement by the grease trap, the floor by the bulk rice storage area, and the bottom few stairs to the non-existent upper level. Clean these areas now and more frequently in the future

Observed some mouse droppings on the bottom couple stair treads of the stairwell to the no-existent second floor. Thoroughly clean this area and perform regular inspections for the presence of pests

Observed two one gallon jugs of sauce stored on the floor. Keep all foods 6 inches off of the floor

Observed the exhaust hood filter located above the range top and the deep fryer to be damaged. Provide a replacement filter for this exhaust hood

Observed a large gap under the front door. Provide a new door sweep that seals the gap to prevent pest entry

Observed the dumpster covers to not be closed. Maintain the dumpster lids closed to prevent pest entry

Observed the gas fired rice cooker still located outside of the exhaust hood. As mentioned previously either switch to an electric rice cooker or have the exhaust hood made larger (permit required) so it covers the rice cooker

Observed the following foods in the basement to be stored under a wastewater pipe: garlic, ginger, onions and cabbage. Also, observed a full 5 gallon container of sauce to be stored right next to the hand washing sink. Corrected by covering this food
