Observed several wipe down rags on the counter at the bar. Wipe down rags shall be stored fully submerged in a sanitizing solution when not in use. (Critical) Observed numerous small flies (likely fruit flies) in the bar area. Additionally, multiple breeding and harborage areas were discovered. Eliminate all breeding and harborage areas and utilize a floor drying fan during the overnight hours to help dry out the floor

(Critical) 1) the dish washing employee (a fill-in employee) did not verify the sanitizer strength in the dish washing machine rinse water. 2) a server did not use chlorine test papers to check the wiping cloth solution. Train and ensure staff are checking the chlorine sanitizers for proper strength

(Critical) Observed five bottles of liquor with pour spouts on them and one open bottle of wine at the bar with dead flies in them. This liquor and wine was discarded. Consider obtaining a commercial liquor pour spout covers to keep in place at all times except when staff are actively pouring the liquor. Keep corks in all opened wine.

(Critical) The following items were found expired in the kitchen coolers: hollandaise sauce (10-3), mexican rice (10-6) and tuna salad (10-2). Corrected by discarding these foods

(Critical) Observed a bar tender handle lemon wedges with her bare hands when placing them on the rim of glasses. No bare hand contact with ready to eat foods is allowed. In the future, utilize tongs, sword spears or single use gloves for this task

(Critical) Upon arrival (prior to the restaurant being open) it was noticed the pail for the dish washing sanitizer was empty. An employee was using this machine to wash some kitchen equipment used during morning prep. A call was placed to ecolab and the technician returned the call to this facility. The technician will arrive at this facility within three hours to provide a replacement pail of dish washing machine sanitizer. Staff are not washing any dishes until the unit is providing proper sanitizer concentration

(Critical) Observed an off-brand of chlorine bleach present in the facility that staff use to prepare the wiping cloth sanitizer solution. This bleach lacks the required epa registration and usage directions for sanitizing food contact surfaces in restaurants. The label only has directions to bleach laundry and clean restrooms. Basement beer room: spilled soda syrup was present at the bag in a box rack and the pvc drain pipe at the floor sink next to the hot water heater are in need of cleaning. (Critical) Observed pan of rte deli meat (ham) and raw hamburger patties in bottom of prep unit cooler on cooking line with internal food temperatures of 49-51f (ric door was slightly ajar when i first arrived for inspection). Pic stated that rte ham and hamburger have been in cooler for 1. 5 hours. All other phf in top and bottom portions of prep unit cooler were cold holding at 41f. All phf must be cold held at 41f or below at all times. Pic corrected by placing food in wic at time of inspection. (Critical) Observed an accumulation of mold on interior faceplate in basement ice machine. All ice machines must be washed, rinsed and sanitized as per manufacturers directions and must be clean to sight and touch. To correct, please remove all ice from basement ice machine, then wash, rinse and sanitize all interior surfaces as soon as possible. Facility has 2 ice machines. Pic corrected by taking basement ice machine out of service and removing ice from machine at time of inspection. Staff washed, rinsed and sanitized ice machine at time of inspection. (Critical) Basement area (near ice machine, bar area, restroom hallway) recently had a sewage backup through the floor drains. Greasy food debris and water observed throughout basement (no blackwater or restroom sewage observed). Carpets in bar area and restroom hallway soaked with water. Owner has already had a licensed plumbing company (charlies sewer and drain) to snake drain on 3/15/13 (saw receipt), but a follow up visit is necessary to fix problem. Basement area is closed to customers and all carpeting will be replaced after backup is fixed. Facility does have a unisex restroom upstairs (near game room) for customer and employee use. Once drains are cleaned and repaired, all basement areas will need a thorough washing and sanitizing. Please complete as soon as possible. I will check this item at follow up inspection.
