Found a frozen chunk of pork thawing on the drainboard of the 3 compartment sink. Pic corrected by placing frozen pork in sink under running water. Pic has been reminded to properly thaw frozen products, such as submerged under running water.

Found the tiled floor in the dining area and kitchen with cracks, chips and uneven surfaces. To correct repair tiled floor to be durable, smooth, and cleanable.

Found the walls and floor edges surrounding cooking equipment soiled with grease and build up. To correct thoroughly clean walls and floor and keep clean.

(Critical) Used a chlorine test kit and found the chlorine sanitizing solution below 25 ppm in a bucket in the 3 compartment sink. Employee corrected by adding chlorine to about 100 ppm (minimum 50 ppm and maximum 200 ppm). Pic has been instructed to monitor and ensure sanitizing solutions are at least 50 ppm at all times.

Found the floor (under equipment, edges and corners) on the cooking line soiled with greasy build up and accumulation. To correct thoroughly clean floors and keep clean.

Observed water on the basement floor possibly dripping from sewer/drain line on the ceiling of the basement. To correct locate source of leak and repair as necessary. Observed the spray rinse nozzle hose no longer secured to shelf and without air gap. Pic corrected by re-securing spray rinse hose to shelf which provided adequate air gap from the spray rinse nozzle and the rim of the 3 compartment sink. Observed the drain line from the 3 comparment sink draining into the floor drain receptacle without air gap. Pic corrected by resecuring drain line to the leg of the 3 comparment sink which provided adequate air gap from the drain line and the rim of the floor drain.

Found the doors in the entire facility open without screen doors. To correct either keep doors closed or provide screen doors if doors are to be left open.

Found the floor in the entire facility with cracks, chips and damaged surfaces. To correct repair floor in entire facility to be smooth, durable, and cleanable.

Found opened bags of ingredients (flour, rice, etc) on the raised floor in storage room floor. To correct store items on shelf at least 6 inches off the floor.

Observed flies in the kitchen area. To correct keep doors closed as much as possible and use fly control measures if needed.

(Critical) Found time/temperature control for safety (tcs) food, beef 48°F, various chicken 51°F to 59°F, dumpling 58°F, egg roll 55°F, eggs 53°F in the 3 door reach in cooler. Pic temporarily corrected by discarding tcs food, and has called for repair. Pic has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F or lower. ,

(Critical) Found tuna roll (raw tuna) on the front counter menu board without proper consumer advisory. To correct provide disclosure, asterisk (*) undercooked foods to a footnote stating the items contain raw or undercooked ingredients, and reminder footnote that states "consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness. Example: menu item: *spicy tuna roll *contains raw or undercooked ingredients. Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness. ,

Found the hood filters and the edges of the cooking hood soiled. To correct clean hood filters and edges of hood as soon as possible, and clean on a more frequent schedule.

Found no antichoking poster on the kitchen wall. To correct provide and post antichoking poster on kitchen wall.

Found cardboard under the fryer and the surrounding area soiled with accumulation and build up. Found the floor in the storage room floor soiled with accumulation. Found the light shield in the dining room area soiled with accumulation. To correct thoroughly clean floors and light shield, and keep clean.

Found the 3 door reach in cooler not holding tcs food at 41°F or lower. To correct repair/replace cooler to hold tcs food at 41°F or lower.

Found the tiled floor in the entire facility chipped and damaged. To correct repair/replace damaged tile in entire facility. Found various areas of ceiling with water stains and peeling paint on wall. To correct repair source of leak and repair/replace ceiling tile and walls.

(Critical) Found a spray can of household pesticide on the kitchen counter. Pic corrected by discarding household pesticide and was instructed to use only pesticide which states can be used in a food establishment.

(Critical) Observed an open cup of beverage on the counter top of the 2 door prep top cooler. Pic corrected by discarding beverage, and has been reminded that cups with lids and straws are allowed in food prep areas.

Observed employee working in the cooking/prep area not wearing hat/hair restraint. Employee corrected by wearing hair net.

Found no antichoking poster on the kitchen wall. To correct provide and post antichoking poster on kitchen wall.

Observed the presence of flies in the dining room and kitchen area. To correct keep doors closed, and continue treating flies.

Found various spots of the floor in the entire facility with cracked, damaged and peeling tile. To correct repair/replace tile floors.

Observed all 3 doors of restaurant propped open. Pic corrected by turning on air conditioning and closing all doors. Pic has been reminded that if doors are to be propped open, screen doors or other means to prevent pest entering building are required.

Found the hood and hood filters soiled with build up. Found the sides of the fryer and surrounding equipment soiled with greasy build up. To correct clean as soon as possible, and clean on a more frequent schedule to preclude accumulation.
