A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under # 4-703. 11(c) shall meet the criteria specified under # 7-204. 11 sanitizers, criteria, shall be used in accordance with the epa-registered label use instructions to correct: provide a new bucket of sanitizer at the machine, corrected: person in charge hooked up a new bucket of sanitizer

(Critical) All three hand sinks at all three bars were void of soap to wash hands. Each hand washing sink or group of 2 adjacent hand washing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. (pf) to correct provide mountable soap dispensers for use at the bars and then retrain staff on when they need to be refilled. Partial correction: person in charge provided moveable hand pump soap to all bars. ,

(p) to correct discard the noted food into the garbage and check dates every morning before opening. Corrected: person in charge had the noted items discarded into the garbage.

Observed the following: 1. A white, chunky like water build up in the bottom of the long wooden bar cooler located at the main level middle bar. 2. Observed all surfaces in need of cleaning in all three bars to eliminate harborage conditions for fly's. These surfaces include and is not limited to all sides of equipment, behind equipment, all shelves, pop guns cords, etc. Non food-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. To correct: 1. Clean the noted cooler. 2. Clean all the surfaces. ,

Observed the long wooden bar cooler in the middle main level bar to have a build up of water in the middle sections. Equipment shall be maintained in a state of repair and condition to correct have the cooler repaired to eliminate the water build up. ,

Observed the slicer with food debris build up in the hard reach areas behind the blade. Equipment food-contact surfaces and utensils shall be cleaned: (1) except as specified in # (b) of this section, before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry; (p) (2) each time there is a change from working with raw foods to working with ready-to-eat foods; (p) (3) between uses with raw fruits and vegetables and with potentially hazardous food (time/temperature control for safety food); (p) (4) before using or storing a food temperature measuring device; (p) and (5) at any time during the operation when contamination may have occurred. Observed multiple bar fly's at all three bars. Note: middle and lower level bars had were observed with the most fly's. The premises shall be maintained free of insects, rodents, and other pests. The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the premises by: (a) routinely inspecting incoming shipments of food and supplies; (b) routinely inspecting the premises for evidence of pests; (c) using methods, if pests are found, such as trapping devices or other means of pest control as specified under ## 7-202. 12, 7-206. 12, and 7-206. 13;(pf) and (d) eliminating harborage conditions. To correct continue working with pest control company and deep/detail clean all bars.

Observed floors, walls and floor drains in need of cleaning to eliminate harborage conditions for fly's. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct clean all the noted areas and maintain all areas clean. ,

Observed mustard being stored in open metal can. Materials that are used in the construction of utensils and food-contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: (p) (a) safe; (p) (b) durable, corrosion resistant, and nonabsorbent; (c) sufficient in weight and thickness to withstand repeated ware washing; (d) finished to have a smooth, easily cleanable surface; and (e) resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. (Critical) Observed the following: a. In a non-working cooler two door up right cooler by the office area for an unknown amount of time: 1. Two containers of raw hamburger 47f. 2. Large container of cooked chicken 47f. 3. Heat treat olive 47f. 4. Cooked veggies 46f. 5. Salmon 46f. B. In the top loading reach-in cooler: 1. Cooked potato mix 50f for an unknown amount of time. 2. Cut cheddar cheese 50f for under 2 hours. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct: a. 1,2,3,4,5; b 1. Discard the noted food into the garbage. B 2. Move the noted food to the walk-in cooler. Corrected: person in charge discarded all the noted food into the garbage and moved the cheddar cheese to the walk-in cooler.

Observed the following bare wood in the facility: 1. Interior frame of the walk-in cooler in the kitchen. 2. Plywood backing behind the three compartment sink at the main level bar. 3. Wooden backing behind the bar in club above v2 bar. Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. To correct paint, stain or cover the noted bare wood to make it smooth and easily cleanable.

Observed the following parts of the facility in need of repair: 1. Ceiling in the middle leve storage room with office. 2. Hole in ceiling above the dishwashing machine in the kitchen. 3. Ceiling in basement leaking water from main level bar. Physical facilities shall be maintained in good repair. To correct: repair all the noted items.

Observed the following equipment in poor repair: 1. Two door upright cooler nearest to the office not holding cold food the closer you get to the bottom. 2. Main level bar keg cooler pooling water. 3. Gaskets torn on the two door upright cooler nearest to the office. 4. Gaskets torn on the two door upright cooler nearest to the dishwashing machine. Equipment shall be maintained in a state of repair and good condition. To correct: 1. Repair the noted cooler to hold cold food 41f or below. 2. Repair the noted cooler to not pool water. 3,4. Observed water dripping on the water supply line shutoff valve of the ice machine. To correct repair water supply line/shutoff valve. Found the drain line of the ice bin in the main level bar loosen from straps. Pic corrected by resecuring drain lines and providing air gap.

Found a hole in the wall by the main level walk in cooler door and on the interior wall by the light switch in the basement beer storage room. Found the entire back ceiling in the main level storage area with holes and water damage. Found the stairs leading to the main level storage area with loose and chipped tiles. Found the ceiling of the walk in freezer warped. Found the wall under the upper bar ceiling light with peeling paint. Found a large gap between the back of the bar counter and floor. Found ceiling tile missing in the main level ladies restroom. To correct repair noted areas and keep facility in good condition.

Found the fryer not completely under the cooking hood. To correct either adjust equipment to fit under hood or provide addition hood.

Found the floor in the upper, middle and lower bar with some accumulation and debris on the floor edges of bar under the counter.

Found no sign reminding employees to wash hands in the main level mens restroom. To correct provide sign.

Found the wall next to the walk in cooler damaged with holes. Found the ceiling in the store room with- water stains, open seams, holes and mold build up. Found the tiled floor in the walk in freezer with missing tile, damaged and exposed surfaces. Found the floor junction under and behind the bar counter in the main level bar without coving and not smooth. Found the tiled coving behind the bar counter in the lower level bar with missing coving and loose pieces. Found the wall surrounding the light switch in the lower level storage room damaged with exposed surfaces. To correct properly repair all noted areas to be smooth, durable and cleanable.

Found no handwashing signs in main level mens restroom, and ladies and mens restroom in the lower level. To correct provide and post signs reminding employees to wash their hands.

Found the bottom portion of the 2 door prep top cooler on the cooking line not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F.
